A 79-year-old man presented to the emergency department with a 1-week history of worsening confusion, falls and hearing impairment. We report a case of bismuth-induced pityriasis rosea-like drug eruption. Although historical accounts of bismuth hypersensitivity exist, contemporary reports are lacking. Given the frequency of bismuth administration, a modern review of this phenomenon would seem prudent.

The effect of colloidal bismuth subcitrate (De-Nol) on symptoms, Helicobacter pylori status and histological features was studied in 35 patients with non-ulcer dyspepsia. Pain (34 cases) and gas bloat (18) were the predominant symptoms. H pylori was present in 26 (74.3%) patients. Gastritis and duodenitis were present in 29 of 32 and 22 of 31 cases respectively in whom biopsies were available. Relief in symptoms after treatment was seen in 29 (82.8%) cases. Thirty-eight testicular hydroceles were treated by injection with 0.2-0.4 g of bismuth phosphate. The follow-up was completed in 32 cases, 17 (53%) of which showed complete regression although one patient had to wait 24 months for full regression. Eight patients (25%) showed clinical improvement and one patient (3%) had residual scrotal tenderness after otherwise successful treatment. Six patients (19%) did not improve and were treated surgically. The method may present an alternative to surgery in elderly patients with testicular hydrocele.

The active ingredient in Pepto-Bismol (PB) (Norwich-Eaton), a common antidiarrheal, is bismuth subsalicylate. The absorption of salicylate after oral PB was studied in six fasted men. Plasma concentrations of total salicylate and the urinary excretion profile of salicylate were determined as a function of time and dose. After 60 ml PB, 500.1 +/- 33.6 mg (mean +/- SD) salicylate were recovered in urine, representing 95.0 +/- 6.4% of salicylic acid equivalents in 60 ml of the formulation.
