Improved Pressure Equalization Ratio Following Mannitol Administration in Patients With Severe TBI: A Preliminary Study of a Potential Bedside Marker for Response to Therapy.

BACKGROUND: Performing a cerebrospinal fluid (CSF) drainage challenge can be used to measure the pressure equalization (PE) ratio, which describes the extent to which CSF drainage can equalize pressure to the height of the external ventricular drain and may serve as a correlate of cerebral edema. We sought to assess whether treatment with mannitol improves PE ratio in patients with severe traumatic brain injury (TBI) with elevated intracranial pressure (ICP). METHODS: We studied consecutive patients with TBI and brain edema on computed tomography scan and an external ventricular drain (EVD),

Update on the current knowledge of lymphatic drainage system and its emerging roles in glioma management.

The draining of brain interstitial fluid (ISF) to cerebrospinal fluid (CSF) and the subsequent draining of CSF to meningeal lymphatics is well-known. Nonetheless, its role in the development of glioma is a remarkable finding that has to be extensively understood. The glymphatic system (GS) collects CSF from the subarachnoid space and brain ISF through aquaporin-4 (AQP4) water channels. The glial limiting membrane and the perivascular astrocyte-end-feet membrane both have elevated levels of AQP4. CSF is thought to drain through the nerve sheaths of the olfactory and other cranial nerves as well

Methods of Monitoring Intracranial Pressure: A Review.

Monitoring intracranial pressure (ICP) is important in a variety of neurologic conditions, including aneurysmal subarachnoid hemorrhage and traumatic brain injury. Monitoring and controlling ICP can mitigate secondary injury of the brain. Of the invasive methods of monitoring ICP, the external ventricular drain is still considered the gold standard. However, microtransducers have been shown to be a reliable option with significantly lower risks of complications. Due to their reproducibility, and their limitations, they are not ready to replace invasive ICP monitoring techniques. This article r

Case report: Completely unroofed coronary sinus with a left superior vena cava draining into the left atrium studied by cardiovascular magnetic resonance.

A persistent left superior vena cava (LSVC) draining through a dilated coronary sinus into the right atrium is a relatively common congenital cardiovascular anomaly. It is readily identified by cardiovascular magnetic resonance (CMR). However, a LSVC draining into the left atrium (LA) and associated with unroofing of the coronary sinus, with resulting interatrial communication, is rare and may have important clinical consequences. As with any large atrial septal defect, it can be associated with a higher than expected incidence of pulmonary arterial hypertension, systemic embolization, and bra

Comparing the Level of Knowledge on External Ventricular Drain Care: A National Study of Neurosurgical and General ICU Nurses.

BACKGROUND: An External Ventricular Drain (EVD) is a catheter inserted into one or both of the lateral ventricles of the brain. The indications for inserting this drain include intracranial haemorrhage, severe traumatic brain injury, to monitor intracranial pressure and to drain excess Cerebral Spinal Fluid. To ensure safe care for patients with an EVD, nurses need evidence-based knowledge in providing quality care. AIM: The purpose of this study was to describe and compare nursing knowledge and practices regarding patients with EVDs between nurses in General Intensive (GICU) and Neurosurgica

[Dynamic distribution and clearance of 99mTc-DTPA in brain extracellular space].

OBJECTIVE: To explore the distribution and clearance of 99mTc labeled diethylenetriamine pentaacetic acid (99mTc-DTPA) in different brain regions of adult rats after administration through brain extracellular space (ECS) pathway. METHODS: After the injection of a volume of 2 μL and radioactive activity of about 3.7 MBq (100 μCi) of 99mTc-DTPA into the caudate nucleus and thalamus of SD rats through stereotactic positioning of rat brain, the single photon emission computed tomography/computed tomography (SPECT/CT) for small animals was used for imaging at different time points, and the dyna-mi

Microsurgical anatomy of the venous drainage of the mesencephalodiencephalic junction.

OBJECTIVE: The veins draining the posterior wall of the third ventricle and its adjacent structures and the posterior part of the midbrain have been the most neglected of the intracranial vascular structures in both the anatomic and neurosurgical literature. During our dissections of the pineal region and the quadrigeminal cistern, we did not always encounter topographic anatomy as described in previous articles. The purpose of this study is to describe the topographic anatomy and normal variations of the specific veins that drain the collicular plate and the pineal body and their adjacent str

Subdural empyema, brain abscess, and superior sagittal sinus venous thrombosis secondary to Streptococcus anginosus.

Streptococcus anginosus can be frequently isolated from brain abscesses, but is a rare cause of the liver, lung, and deep tissue abscesses. In this report, we present a patient with subdural empyema, brain abscess, and superior sagittal cerebral venous thrombosis as complications of rhinosinusitis whose purulent empyema sample yielded S. anginosus. A 13-year-old female patient was referred to our pediatric intensive care unit with altered mental status, aphasia, and behavioral change. On a brain computed tomography scan, subdural empyema extending from the left frontal sinus to the frontal int

Difficulties in Transvenous Embolization via the Cavernous Sinus: Buddy-Wire Anchoring Technique.

The cavernous sinus drains venous blood from most of the temporal lobe through the superficial middle cerebral vein (SMCV).1 Brain AVMs located on the surface of the temporal and parietal lobes often drain into the SMCV.2 Therefore, in cases treated with transvenous embolization, accessing the SMCV passing through relaying the cavernous sinus (CS), which can interfere with the deployment of the microcatheter by trapping the catheter in the CS. In this video article, we describe a bailout technique for this situation using another microguidewire. This additional guidewire provides support by se

Reverse brain herniation during posterior fossa surgery.

Posterior fossa tumors are commonly associated with obstructive hydrocephalus. Relieving the raised intracranial pressure by draining the cerebrospinal fluid presents the possibility of reverse herniation of the brain. A 5-year-old male child with a diagnosis of posterior fossa space-occupying lesion and hydrocephalus was scheduled for craniectomy in the prone position. After craniectomy, the surgeons placed an intraventricular shunt catheter to drain out cerebrospinal fluid in an attempt to reduce the tense brain so as to facilitate easy dissection of the tumor. The patient had sudden and sev

Glymphatic System as a Gateway to Connect Neurodegeneration From Periphery to CNS.

The classic concept of the absence of lymphatic vessels in the central nervous system (CNS), suggesting the immune privilege of the brain in spite of its high metabolic rate, was predominant until recent times. On the other hand, this idea left questioned how cerebral interstitial fluid is cleared of waste products. It was generally thought that clearance depends on cerebrospinal fluid (CSF). Not long ago, an anatomically and functionally discrete paravascular space was revised to provide a pathway for the clearance of molecules drained within the interstitial space. According to this model, C

Deletion of Lymphatic PD-L1 Protects Mice From Severe Autoimmune Encephalitis.

Lymphatic endothelial cells at the cribriform plate and in brain-draining cervical lymph nodes express PD-L1 during autoimmune encephalitis. Unexpectedly, deletion of lymphatic PD-L1 expression ameliorates disease symptoms and is associated with increased brain infiltration of regulatory CD8+ Foxp3+ T cells.

[A man in his thirties with recurrent brain abscesses].

BACKGROUND: Brain abscess is a life-threatening condition. Congenital cardiovascular malformations can create right-to-left shunting and be an underlying cause. CASE PRESENTATION: A young man was admitted due to headache and deteriorating general condition. He had a history of a surgically treated brain abscess 19 years earlier. Investigations now showed a new brain abscess. The patient was operated and received a peripherally inserted central catheter in his left arm for antibiotic treatment. A chest X-ray showed abnormal positioning of the catheter lying in a persistent left superior vena c

Gadolinium enhancement of cranial nerves: Implications for interstitial fluid drainage from brainstem into cranial nerves in humans.

Drainage of interstitial fluid and solutes from the brainstem has not been well studied. To map one drainage pathway in the human brainstem, we took advantage of the focal blood-brain barrier disruption occurring in a multiple sclerosis brainstem lesion, coupled with intravenous injection of gadolinium, which simulates an intraparenchymal injection of gadolinium tracer within the restricted confines of this small brain region. Using high-resolution MRI, we show how it is possible for interstitial fluid to drain into the adjacent trigeminal and oculomotor nerves, in keeping with a pathway of co

A hemodynamic model for layered BOLD signals.

High-resolution blood oxygen level dependent (BOLD) functional magnetic resonance imaging (fMRI) at the sub-millimeter scale has become feasible with recent advances in MR technology. In principle, this would enable the study of layered cortical circuits, one of the fundaments of cortical computation. However, the spatial layout of cortical blood supply may become an important confound at such high resolution. In particular, venous blood draining back to the cortical surface perpendicularly to the layered structure is expected to influence the measured responses in different layers. Here, we p

The use of intraventricular vancomycin in subacute brain abscess in an adolescent male: A case report.

INTRODUCTION: Brain abscess is a serious condition in children, leading to rapid deterioration, and permanent neurological damage associated with significant morbidity and mortality. Current management protocols for brain abscesses focus on intravenous antibiotics and surgical excision and drainage. CASE PRESENTATION: A 12-year-old adolescent male who had headache and photophobia and was diagnosed with multiple brain abscesses and was refractory to conventional medical and neurosurgical intervention. A single dose of 10 mg vancomycin was administered through endo-ventricular drain, resulting

Ruptured Suprasellar Dermoid Cyst Treated With Lumbar Drain to Prevent Postoperative Hydrocephalus: Case Report and Focused Review of Literature.

Background: Ruptured intracranial dermoid cysts are extremely rare. Standard treatment consists of endonasal decompression or craniotomy with evacuation and copious irrigation of subarachnoid spaces to remove any disseminated cystic contents. Disseminated fat particles in the subarachnoid space may be the cause of further sequalae, including the subsequent development of chemical meningitis and hydrocephalus. Here, we present a case of ruptured suprasellar dermoid cyst treated with craniotomy for emergent optic nerve decompression, followed by postoperative hydrocephalus successfully treated w

Methotrexate induced brain necrosis and severe leukoencephalopathy due to disconnection of an Ommaya device.

A 63-year old woman with breast carcinoma developed leucoencephalopathy and local brain necrosis after intraparenchymal infusion of methotrexate. This iatrogenic condition was caused by drain disconnection of the Ommaya device (bore hole type). The diagnosis of this rare complication of an Ommaya device was based on multiplanar MR-imaging. The case is discussed with regard to the complications of intraventricular chemotherapy by means of an Ommaya reservoir.
