[The correlations of dysplastic hip joint pathology with fetal breech presentation].

In the article are discussed the causes of hip joint instability at breech presentation. In the article are presented the results of examination of 116 probands with hip joint dysplasia, aged 5-15 years, and 69 breech presentation born probands, It has been carried out genetic-correlative analysis on the basis of population frequency and frequency among the probands' relatives, differentiated in accordance with the signs discussed. The principal cause of the hip joint dysplastic pathology formation at breech presentation is the mechanical factor.

External Cephalic Version-A Chance for Vaginal Delivery at Breech Presentation.

Background and Objectives: In recent years, the rate of caesarean section (CS) has increased constantly. Although vaginal breech delivery has a long history, breech presentation has become the third most common indication for CS. This study aims to identify factors associated with the success of external cephalic version (ECV), underline the success rate of ECV for breech presentation and highlight the high rate of vaginal delivery after successful ECV. Material and Methods: This retrospective observational study included 113 patients with singleton fetuses in breech presentation, who underwen

Breech presentation management: A critical review of leading clinical practice guidelines.

PROBLEM: Clinical practice guidelines are designed to guide clinicians and consumers of maternity services in clinical decision making, but recommendations are often consensus based and differ greatly between leading organisations. BACKGROUND: Breech birth is a divisive clinical issue, however vaginal breech births continue to occur despite a globally high caesarean section rate for breech presenting fetuses. Inconsistencies are known to exist between clinical practice guidelines relating to the management of breech presentation. AIM: The aim of this review was to critically evaluate and com

Cesarean Rate and Risk Factors for Singleton Breech Presentation in China.

OBJECTIVE: To determine the risk factors for breech birth, as well as the rates of cesarean section in singleton breech births in different Chinese provinces. STUDY DESIGN: This retrospective, cross-sectional study included 109,736 singleton preterm and term infants, with 4,535 presenting breech. The risk factors of singleton breech were calculated by using multiple logistic regression analysis based on a cohort design (4,535 breech, 103,484 cephalic presentation). RESULTS: The incidence of term singleton breech decreased from 8.34% to 2.17%, but overall breech cesarean rates ranged from 83.

[Assisted vaginal delivery using the vacuum extractor in frank breech presentation].

OBJECTIVES: When all the breech conditions for the acceptance of a vaginal breech birth are present, we occasionally practise a total breech extraction for non frank breech presentations. Similarly, for frank breech presentations, instead of using the fetal leg as tractor, we sometimes apply the vacuum extractor on the breech presentation in order to perform the first step of the total breech extraction. The vacuum extractor is not traumatic for the fetus and enable a quick extraction. This study was conducted to describe the technique we use in our ward. PATIENTS AND METHODS: A retrospective

[Obstetric and perinatal outcomes of a disreputable presentation: the nonfrank breech].

OBJECTIVES: A nonfrank breech is generally believed to be associated with abnormal labor and risk of cord prolapse. Owing to these risks, many authors consider such a presentation as a positive indication for cesarean section. The purpose of this study was to estimate the delivery characteristics of nonfrank breech with a gestational age over 37 weeks and particularities of this presentation. PATIENTS AND METHODS: This retrospective study included all deliveries with a nonfrank breech presentation and a gestational age over 37 weeks over a sixteen years period in our maternity ward. [Progress of labor and neonatal morbidity in primiparity with breech presentation].

Progress of labour in 100 consecutively delivered term ( > or = 37 weeks) primiparous breech births was analysed and correlated with fetal outcome according to different durations of first and second stages. All women had sonographic biometry prior to admission and epidural anaesthesia (PDA) early in established labour. Women with nonreassuring fetal heart rate tracings during any stage of labour were delivered by caesarean section, and were excluded. Birth weights were between 1980 g and 4090 g. There was no perinatal mortality, but eight neonates sustained birth associated trauma (6 Erb's pa

The effect of breech presentation on the accuracy of estimated fetal weight.

To determine whether fetal presentation affects the accuracy of ultrasonographic estimated fetal weight (EFW). This is a retrospective cohort study of singleton pregnancies that underwent ultrasonographic EFW within 3 weeks of delivery at a single institution from 1993 to 2008. Breech presenting fetuses were compared with those presenting cephalic. EFW using the Hadlock formula was compared with actual birth weight (ABW) and reported as mean difference and mean percentage difference. Differences were also considered categorically. Subgroup analyses were performed of women who delivered within

Changed prognosis of breech-presenting low birthweight infants.

We have compared the mortality and morbidity of low birthweight breech-presenting singleton infants born in the Coventry Maternity Hospital during the years 1973 and 1974 with those born during 1979 and 1980. During this period changes in management have included an increase in the caesarean section rate and the continuing development of methods of intensive care for the newborn infants. The neonatal mortality rate has fallen but the increased survival rate is accounted for largely by the survival of handicapped infants. The value of routine caesarean section in the absence of other obstetric

[Child development after abdominal version of the fetus from breech presentation to vertex presentation near term].

The prospective study presented here describes the results of follow-up examinations of the developmental stage of 116 children born between 1977 and 1980 in whom abdominal version of the foetus from breech presentation to vertex presentation was performed near term. The children were examined according to the Denver Developmental Screening Test at the age of 2 to 5 3/4 years in various fields (social contacts, minor motor functions and adaptation, speech and major motor functions). Children with abnormal findings were subjected to special examinations. Those with disturbed development were re

Planned caesarean section versus planned vaginal birth for breech presentation at term: a randomised multicentre trial. Term Breech Trial Collaborative Group.

BACKGROUND: For 3-4% of pregnancies, the fetus will be in the breech presentation at term. For most of these women, the approach to delivery is controversial. We did a randomised trial to compare a policy of planned caesarean section with a policy of planned vaginal birth for selected breech-presentation pregnancies. METHODS: At 121 centres in 26 countries, 2088 women with a singleton fetus in a frank or complete breech presentation were randomly assigned planned caesarean section or planned vaginal birth. Women having a vaginal breech delivery had an experienced clinician at the birth. Mothe

Association between breech presentation in twins and developmental dysplasia of the hip.

BACKGROUND: Breech presentation is considered one of the main risk factors for developmental dysplasia of the hip (DDH) in singleton pregnancies; however, data regarding twins are limited. The aim of this study was to evaluate whether breech presentation in twins is associated with abnormal hip ultrasound findings and to assess the influence of delivery mode, sex and gestational age. METHODS: Obstetric and ultrasound data from twin births at Frankfurt University Hospital between 2010 and 2014 were retrospectively analyzed. A total of 130 twins in whom at least 1 infant with breech presentatio

Breech presentation delivery care: A review of childbirth semiology, mechanism and care.

OBJECTIVE: To review the concepts underlying breech presentation delivery as well as the semiology and the obstetric maneuvers contributing to a successful perinatal maternal outcome. MATERIALS AND METHODS: Based on a hypothetical scenario to set the stage for a practical approach to the topic, an explanatory paper built on a narrative review is created in order to examine the principles related to diagnosis, mechanism of delivery and maternal care, emphasizing maneuvers to ease fetal extraction. RESULTS: Breech presentation delivery must be managed through the vaginal canal when already in

Preterm Breech Presentation: A Comparison of Intended Vaginal and Intended Cesarean Delivery.

OBJECTIVE: To study the association of the intended mode of delivery and perinatal morbidity and mortality among breech fetuses who are delivered preterm. METHODS: We conducted a nationwide cohort study of women with a singleton pregnancy in breech presentation who delivered preterm (26 0/7-36 6/7 weeks of gestation) in the years 2000-2011. We compared perinatal outcomes according to the intended and actual mode of delivery using multivariate logistic regression analysis. We performed subgroup analyses of gestational age and parity. RESULTS: We studied 8,356 women with a preterm singleton br

Randomized trial of anaesthetic interventions in external cephalic version for breech presentation.

BACKGROUND: Successful external cephalic version (ECV) for breech presenting fetus reduces the need for Caesarean section (CS). We aimed to compare the success rate of ECV with either spinal anaesthesia (SA) or i.v. analgesia using remifentanil. METHODS: In a double-phased, stratified randomized blinded controlled study we compared the success rates of ECV, performed under spinal anaesthesia (SA), i.v. analgesia (IVA) using remifentanil or no anaesthetic interventions. In phase I, 189 patients were stratified by parity before randomization to ECV, performed by blinded operators, under SA usin

[Obstetrical prognosis of breech presentation admitted in advanced first stage of labour].

AIM: Fear of dystocia during breech delivery brings obstetrical teams to choose elective caesarean sections. However, some patients with breech presentations will be in labour before the date scheduled and caesarean delivery during labour increases maternal morbidity. Therefore, this situation raises the question about the relevance of labour trial for breech presentations admitted in labour. Our study aimed to determine the obstetrical prognosis of breech presentations on admission in the labour ward following their degree of cervical dilation. MATERIALS AND METHODS: We conducted an observat
