Nurses in many parts of the United States have limited experience in caring for patients with Kaposi's sarcoma. The severity of the disease can range from a few lesions to extensive cases with fungating, encrusted wounds. This case study describes the wound management of a 43-year-old patient with an advanced case of Kaposi's sarcoma with elephantiasis of the lower extremities. Use of a collaborative care approach resulted in dramatic improvement of this patient's wounds. Although the patient's serum albumin level remained low, no secondary skin breakdown or infection occurred.

Angiosarcoma is a rare malignant tumor of the soft tissues (less than 1% of sarcoma). It may affect various parenchymae (bones, liver, breast), the soft tissues or the skin. In the latter case it may be associated with lymphedema. Onset in the arm is typical with postmastectomy lymphedema (Stewart-Treves syndrome). To elucidate the issue of lymphoedema and elephantiasis in our previous reports, all data on the two clinical conditions have been reviewed and analysed. The various characteristics of the swelling are described and factors that may influence the outcome of treatment with diethylcarbamazine (DEC) analysed. We report a case of idiopathic elephantiasis of penis. A 41-year-old man was refered to our hospital with a painful penile swelling and severe miction pain. He had no particular past or familial history, and had never been to tropical or semitropical region. Physical examination showed a prominent swelling and flection of his penis. OBJECTIVE: To assess the outcome of the surgical management of severe elephantiasis of male external genitalia.
PATIENTS AND METHODS: Twelve patients with gross oedema of their penis and scrotum were managed by excision of the lymphoedematous skin and subcutaneous tissue and a split-skin graft.
RESULTS: The cosmetic and functional results were satisfactory in all cases. There was no recurrence and all patients were able to resume sexual activity.
CONCLUSION: Surgical excision of affected tissue and split-skin graft is an effective management for severe lymphoedema of the scrotum and penis.

Filarial involvement of penis in endemic areas is not an uncommon entity. In the late stage it may lead to crippling "ramhorn" penis. A case of filarial penis is presented which was successfully managed by split thickness free graft.

A case of elephantiasis nostras in a lower limb is presented. A thorough programme of investigation could only reveal a single episode of erysipelas as the possible etiological origin. The therapeutic possibilities are briefly reviewed. The most important are early institution of adequate antibiotic treatment and antibiotic treatment of every recurrence of disease.
