Fibrous dysplasia mimicking a pediatric endolymphatic sac tumor.

A 15-year-old female presented for evaluation of progressive hearing loss over a year. Computed tomographic imaging revealed a 11 x 6 × 6 mm osseous lesion with 'groundglass' appearance within the left posterior petrous bone lateral to vestibular aqueduct suspicious for an endolymphatic sac tumor. Surgical excision revealed fibrous dysplasia on histological analysis. Fibrous dysplasia of the temporal bone is not uncommon, but few cases of extension causing sensorineural hearing loss exist in the literature. We describe the first reported case of fibrous dysplasia mimicking an endolymphatic sac…

Histopathology after endolymphatic sac surgery for Ménière's syndrome.

BACKGROUND: The putative goal of sac surgery in Ménière's syndrome is to promote the flow of endolymph from the labyrinth to the endolymphatic sac and thereby relieving hydrops. There are scant published histopathologic data whether sac surgery actually accomplishes this goal. OBJECTIVE: To determine whether sac surgery relieves hydrops by examining the histopathologic changes in temporal bones obtained from individuals who had undergone sac surgery during life for Ménière's syndrome. METHODS: Temporal bones were examined from 15 patients who had sac surgery. Data on the presence and severit…

Delayed facial nerve palsy after endolymphatic sac surgery.

Data on delayed facial nerve palsy (DFNP) following endolymphatic sac enhancement surgery are limited. We conducted a retrospective chart review to determine the incidence, possible predisposing factors, treatment, and prognosis of DFNP in such cases. We reviewed the records of 779 patients who had undergone endolymphatic sac surgery for intractable Ménière disease from January 1997 through December 2007 at a tertiary care otologic referral center. We found 5 cases (0.64%) of postoperative DFNP. The length of time between surgery and the onset of DFNP ranged from 7 to 20 days (mean: 11). Paral…

Endolymphatic sac revision for recurrent intractable Meniere's disease.

Meniere's disease can be observed uniquely in revisions of the conservative surgical procedure for endolymphatic sac enhancement, which preserves the labyrinth, unlike destructive procedures. Genetic anatomic abnormalities in patients lead to malabsorption of endolymph and symptoms of Meniere's that are reversed by endolymphatic sac enhancement, but disabling symptoms eventually may recur in a few cases and require revisional surgery. We observe developing pathophysiologic conditions intraoperatively and have modified our techniques to accommodate the redeveloped pathogenesis we observe and av…

Current and historical perspectives on endolymphatic sac surgery.

The 70-year history of the Ear, Nose and Throat School, Bordeaux, is inextricably linked to the achievements of Georges Portmann who pioneered surgery of the endolymphatic sac (ELS) and who contributed to the reputation of the institution. This review comprises three parts: the experiment work of Portmann on the ELS, the research into clinical techniques that has been conducted at the School; and current viewpoints on Meniere's disease. We will also present a summary of the results of a comparison of ELS surgery and neurectomy of the vestibular nerve.

Revision endolymphatic sac surgery for recurrent Meniére's disease.

A total of 44 cases of revision endolymphatic sac surgery (RESS) were studied. Overall, results of RESS were comparable to the primary endolymphatic sac surgery (PESS). Furthermore, early failure rate (around 6 months after PESS) is significantly higher compared to late failure (approximately 2 years after PESS). In addition, important revision findings which showed saccular fibrosis and new bone growth in the long-term make possible the theory that there is a difference in pathological locus between early and late failures of PESS. Bulky implant devices (such as inner ear valve) had the effec…

Long-term results of revision endolymphatic sac surgery.

The results of revision endolymphatic sac surgery on 30 patients with unilateral Menière's disease are presented. Good control is achieved in 73.3 per cent of cases at six months, falling to 56.7 per cent at one year and only 33.3 per cent by four years. The long-term results are disappointing but are more favourable if the initial surgery was successful, if the hearing remains fluctuant and improves on glycerol dehydration prior to the revision procedure and also if the sac is intubated. The current place of endolymphatic sac surgery is discussed and revision surgery only recommended for care…

Results of endolymphatic sac surgery for Ménière's disease.

From 1976 to 1982, 72 patients with Ménière's disease had endolymphatic sac surgery. Sixty of the 72 were reviewed after a mean period of 33 months from operation; in these, sac surgery (regardless of type) provided abolition or improvement of vertigo in 75%, and 70% of these had hearing improvement or stabilization. Twelve had failure of vertigo control and required either labyrinthectomy or vestibular neurectomy, usually within one year of the first operation. In a smaller subgroup followed for a mean period of 51 months, vertigo control was achieved in 60% and hearing maintenance in 40%. Ou…

Endolymphatic sac valve implant surgery. I: The technique.

A resurgence of interest in surgery on the endolymphatic sac (ELS), not only for relief of vertigo but also for stabilization or improvement of hearing, has emerged. With the concept of operating early in the course of the disease after the diagnosis is well established, various modifications of existing procedures on the ELS have been utilized in an attempt to improve the overall results. One such major modification is the unidirectional inner ear valve implant modified from the eye valve for glaucoma. This valve implant has produced impressive one-year results for both hearing improvements a…

History of the Endolymphatic Sac: From Anatomy to Surgery.

OBJECTIVES: To evaluate the historical descriptive origins of the anatomy of endolymphatic sac (ELS), its initial dilation description, its association with tumors, and lastly its surgical implication. METHODS: A collection of historical and contemporary references, and selected textbooks were compiled spanning the 18th century to present day with specific focus on the description of the ELS, ELS dilation, and ELS tumors (ELSTs). RESULTS: Domenico Cotugno describes the ELS in 1760. Carl Hasse introduces the term endolymphatic sac (saccus endolymphaticus) in 1873, which has remained in use ti…

Endolymphatic sac drainage for the treatment of Ménière's disease.

OBJECTIVE: To describe the results obtained with endolymphatic sac drainage in patients with Ménière's disease. METHOD: A retrospective case review study was conducted of 95 Ménière's disease patients who underwent endolymphatic sac drainage in a tertiary care referral centre, after failing a long course of medical management. The main outcome measures were vertigo control and hearing preservation. RESULTS: In patients with unilateral disease, vertigo control was obtained in 94.3 per cent of patients. A significant improvement in cochlear function was seen in 14 per cent of patients, and hea…

Endolymphatic sac decompression as a treatment for Meniere's disease.

OBJECTIVES/HYPOTHESIS: Endolymphatic sac decompression is a surgical treatment option for patients with medically intractable Meniere's disease. However, effectiveness is debated because published data show great variability. Outcome-based research studies are useful in incorporating the patient's perspective on the success of treatment. To further assess effectiveness of endolymphatic sac decompression, we performed a prospective study to examine both symptom-specific and general health outcomes. STUDY DESIGN: Prospective, observational outcome study. METHODS: Nineteen patients with endolym…

Effects of VHL deficiency on endolymphatic duct and sac.

The von Hippel-Lindau (VHL) disease is caused by VHL germ line mutation. Inactivation of the wild-type copy of the VHL gene leads to up-regulation of hypoxic response and tumor formation within central nervous system (CNS), kidneys, pancreas, adrenal glands, epididymis, broad ligament, and the endolymphatic sac/petrous bone. Endolymphatic sac tumors (ELST) have been proposed to be derived from endolymphatic sac epithelium, but other possible structures of origin have been implicated. To clarify the anatomic and cellular origin of ELSTs, we did a morphologic and molecular pathologic analysis of…

[The endolymphatic sac: its roles in the inner ear].

The endolymphatic sac is a non-sensory organ of the inner ear. It is connected to the endolymphatic compartment that is filled with endolymph, a potassium-rich fluid that bathes the apical side of inner ear sensory cells. The main functions ascribed to the endolymphatic sac are the regulation of the volume and pressure of endolymph, the immune response of the inner ear, and the elimination of endolymphatic waste products by phagocytosis. Functional alteration of these functions, leading to deficient endolymph homeostasis and/or altered inner ear immune response, may participate to the pathophy…

Vascular occlusion in the endolymphatic sac in Meniere's disease.

In 2 patients with severe Meniere's disease (MD), there was histologic evidence of occlusion of the vein of the vestibular aqueduct (VVA). This finding coincided with total or partial occlusion of numerous small vessels around the endolymphatic sac (ES), flattening of epithelium, extensive perisaccular fibrosis, and signs of new bone formation. Ultrastructural analysis of the occluding material showed foci with dense connective tissue, calcification, lipid deposits, and layers of basement membrane, sometimes concentrically arranged. The exact nature of the occluding material was unknown. In an…

[Adenocarcinoma of the endolymphatic sac in children. A case report].

We report a case of an adenocarcinoma of the endolymphatic sac in a 13 years old child, which was fortuitously discovered at the time of radiological investigation of growth delay. Arising from the end of the endolymphatic sac, this rare tumour shares a similar clinic expression with other cerebellopontine angle pathologies. Radiological investigation reveals a moth eaten appearance to the posterior temporal bone and specific bony inclusions. With MRI there is an intense and/or intermediate signal on T1, a hyper signal on T2 and T1 enhancement with injection of gadolinium. Treatment is surgica…

[Anatomy of the endolymphatic sac and its clinical significance].

Using 28 formalin fixed temporal bones and materials of 26 cases of endolymphatic surgery for menieres' disease, the location, vascular distribution and size of the endolyphatic sac (ELS) were investigated. ELS were measured with surgical microscope in all the cadaver specimen with a mean length of 6.9393 +/- 2.0429 mm and mean width of 5.8821 +/-1.6522 mm. All the specimens showed no clear capillary distribution. Only in 24 of the 26 ELS surgeries for Menieres' disease the ELS were found. The mean length was 6.8125 +/- 2.2449 mm, the mean width 5.6666 +/- 1.5084 mm and the vascular distributi

Effect of ototoxic drug administration to the endolymphatic sac.

Aminoglycosides were applied to the endolymphatic sac of guinea pigs, and the inner ears were studied by light microscopy. In group 1 gentamicin sulfate application on the lateral surface of the endolymphatic sac produced sensory cell atrophy limited to the macula sacculi. In group 2 gentamicin injection into the sac by extradural and intradural approaches often showed lesions in the maculae sacculi and to the sac from a lateral opening resulted in a similar pattern of degeneration but with less severity in comparison to the injection method. Mild to severe endolymphatic hydrops was sometimes …
