[Episiotomy and prevention of perineal and pelvic floor injuries].

OBJECTIVES: The objective of this review was to assess the efficacy of episiotomy to prevent severe perineal tears, urinary incontinence, faecal incontinence and genital prolapse. MATERIAL AND METHODS: A systematic review on Medline database was performed between 1980 and 2005. One hundred seventy seven articles were selected. Trial quality was assessed on the following parameters: design (prospective, randomized, meta-analysis), sample size (>50) and relevant results. Finally, 43 articles were analysed. RESULTS: The routine use of episiotomy did not prevent severe perineal tears. It decreas…

Effect of episiotomy on the frequency of vaginal outlet lacerations.

The effect of midline episiotomy on the incidence and severity of vaginal outlet lacerations in spontaneous vaginal deliveries was studied in a retrospective manner by pairing women with and without episiotomy on the basis of age, parity and infant birth weight. When we controlled these three parameters, the patient with an episiotomy had fewer or less severe lacerations 33% of the time. However, when the episiotomy itself was counted as a laceration in the analysis, the number of patients benefitting from the procedure was reduced to 6%, with the nonepisiotomy patient having fewer lacerations…

Postpartum Vaginal Adhesion Following Episiotomy: Treatment With Tampon Therapy.

Postpartum vaginal agglutination/stenosis is a rare complication. It can lead to significant morbidity related to quality of life and sexual functioning. Documentation in the literature is limited, especially in low-resource settings. We present a case of complete vaginal stenosis in a 23-year-old primigravida homemaker from Udaipur, India, after a difficult episiotomy repair. The woman presented three months postpartum with dyspareunia and was found to have complete vaginal stenosis approximately 3 cm from the introitus on examination. The patient improved completely with conservative managem…

Repair of episiotomy and obstetrical perineal lacerations (first-fourth).

Perineal injury after vaginal delivery is common, affecting up to 90% of women. Perineal trauma is associated with both short- and long-term morbidity, including persistent pain, dyspareunia, pelvic floor disorders, and depression, and may negatively affect a new mother's ability to care for her newborn. The morbidity experienced after perineal injury is dependent on the type of laceration incurred, the technique and materials used for repair, and the skill and knowledge of the birth attendant. After all vaginal deliveries, a systematic evaluation including visual inspection and vaginal, perin

Clinical effects of different anesthesia methods in lateral episiotomy.

OBJECTIVES: To investigate the clinical effects of different anesthesia methods in lateral episiotomy. Providing the guidance of choosing the appropriate anesthesia method in clinical operation. METHODS: A total of 300 primiparas with vaginal delivery were enrolled into this study. These primiparas were divided into three groups (n=100, each), according to the different methods of anesthesia: group A (pudendal nerve block anesthesia + stepwise dissection and incisional local anesthesia), group B (bilateral pudendal nerve block anesthesia), and group C (pudendal nerve block anesthesia + local …

The role of mediolateral episiotomy during operative vaginal delivery.

The role of episiotomy with regard to prevention of anal sphincter injuries (OASIS) is under discussion. The recently published guideline of the WHO "Intrapartum care for a positive childbirth experience" states that the role of episiotomy during operative vaginal deliveries remains to be established. This guideline is based on the evidence coming from randomised clinical trials. However, since the turn of the century large observational studies have pointed out that adequately performed mediolateral episiotomies may play an important role in the prevention of OASIS during operative vaginal de…

Could a mediolateral episiotomy prevent obstetric anal sphincter injury?

OBJECTIVE: To analyse the significance of risk factors and the role of episiotomy in preventing obstetric anal sphincter injury at vaginal delivery. STUDY DESIGN: This is a retrospective cross-sectional study in the Norfolk and Norwich University Hospital in the UK. All caesarean sections and non-vertex presentations were excluded, which resulted in a study population of 10,314 deliveries. Obstetric anal sphincter injury (OASI) was defined as third or fourth degree tears to the anal sphincter muscles, with or without a tear involving the anal mucosa. First a univariate analysis was done to id…

Predictors of episiotomy use at first spontaneous vaginal delivery.

OBJECTIVE: To identify factors associated with the use of episiotomy at spontaneous vaginal delivery. METHODS: We studied 1576 consecutive term, singleton, spontaneous vaginal deliveries in nulliparas at Brigham & Women's Hospital between December 1, 1994 and July 31, 1995. The association of demographic variables and obstetric factors with the rate of episiotomy use were examined. Adjusted odds ratios (OR) and confidence intervals (CI) were estimated from multiple logistic regression analysis. RESULTS: The overall rate of episiotomy was 40.6% (640 of 1576). Midwives performed episiotomies a…

Does episiotomy prevent perineal trauma and pelvic floor relaxation?

OBJECTIVE: To compare the outcomes of the current practice of liberally or routinely employing episiotomy to prevent perineal tears and pelvic floor relaxation (control group) to a policy of restricting episiotomy use to specific fetal and maternal indications (experimental group). DESIGN: A randomized controlled trial (RCT). SETTING: Three university hospitals in Montreal. SUBJECTS: Seven hundred three low-risk women enrolled at 30 to 34 weeks of gestation were randomized late in labor to the designated trial arm, by parity, and followed up to 3 months postpartum. MAIN OUTCOME MEASURES: A…

Selected use of midline episiotomy: effect on perineal trauma.

A prospective, nonrandomized controlled study was performed to determine the effect of using midline episiotomy only for mothers who experienced fetal distress and/or operative vaginal delivery. Such a policy resulted in a significant decline in third- and fourth-degree lacerations in nulliparous women. This reduction was most pronounced in nulliparous women delivering infants larger than 3400 g. No third- or fourth-degree laceration occurred without antecedent episiotomy in any woman. A policy of using episiotomy selectively appears to lower the incidence of perineal trauma.

Episiotomies and obstetric anal sphincter injuries following a restrictive episiotomy policy in France: An analysis of the 2010, 2016, and 2021 National Perinatal Surveys.

BACKGROUND: The French guidelines have recommended a restrictive policy of episiotomy since 2005. We aimed to assess variations in the prevalence of both episiotomy and obstetric anal sphincter injury (OASI) from the 2010, 2016, and 2021 National Perinatal Surveys. METHODS AND FINDINGS: A total of 29,750 women who had given birth to a live infant by vaginal delivery were included. For instance, in 2021, 22.3% of women were over 35 years old, 17.7% were born outside of France, 11.3% had a body mass index (BMI) of 30 kg/m2 or higher, and 39.9% were primiparous. Episiotomy and OASI (third- and f…

Herbal Medicine in Episiotomy Wound Healing: A Systematic Review.

INTRODUCTION: Episiotomy is a common obstetric intervention following vaginal delivery and is frequently associated with pain, delayed wound healing, and postpartum discomfort. Inadequate healing of episiotomy wounds may result in infection, prolonged pain, and long-term perineal complications. Traditional and complementary medicine, particularly herbal therapies, has been increasingly investigated as a potential strategy to enhance wound healing and reduce pain after episiotomy. This systematic review aimed to evaluate the effects of herbal medicines on episiotomy wound healing and pain reduc…

Episiotomy: What angle do you cut to the midline?

OBJECTIVE: Although episiotomies are the commonest obstetric procedure performed the technique of performing one varies amongst obstetricians and midwives. The angle of episiotomy to the midline in particular has been shown to influence the risk of developing obstetric anal sphincter injuries (OASIS). In order to identify the differences in technique and to identify targets for training we undertook a practice survey of episiotomies to analyse the differences in technique between grades of obstetricians and midwives. STUDY DESIGN: A prospective practice survey of staff working on delivery sui…

Selective versus routine use of episiotomy for vaginal birth.

BACKGROUND: Some clinicians believe that routine episiotomy, a surgical cut of the vagina and perineum, will prevent serious tears during childbirth. On the other hand, an episiotomy guarantees perineal trauma and sutures. OBJECTIVES: To assess the effects on mother and baby of a policy of selective episiotomy ('only if needed') compared with a policy of routine episiotomy ('part of routine management') for vaginal births. SEARCH METHODS: We searched Cochrane Pregnancy and Childbirth's Trials Register (14 September 2016) and reference lists of retrieved studies. SELECTION CRITERIA: Randomis…

Decreased anal sphincter lacerations associated with restrictive episiotomy use.

OBJECTIVE: To determine whether restrictive episiotomy use was associated with decreases in anal sphincter lacerations and the risk of anal sphincter laceration attributable to episiotomy. STUDY DESIGN: This was a retrospective database study. Rates of episiotomy, anal sphincter laceration (third- or fourth-degree tear), and other confounding variables were compared among vaginal deliveries before (1999) and after (2002) restrictive episiotomy use was implemented at our institution. Logistic regression was used to estimate the odds ratio of anal sphincter laceration that was due to episiotomy…

Severe perineal lacerations in nulliparous women and episiotomy type.

OBJECTIVE: To determine the patient-related factors associated with severe perineal lacerations in nulliparous women and to evaluate the effect of episiotomy type on the risk of severe perineal tears. STUDY DESIGN: In all, 400 nulliparous women admitted in labor between June and December 2001 were prospectively enrolled. Maternal height, perineal length, fetal birth weight, fetal head circumference, and severe perineal lacerations (third and fourth degrees) were recorded. RESULTS: The rate of severe perineal lacerations was 2% (8/400); 3% with midline, 1% with mediolateral groups. In patient…

Episiotomy: does it have to hurt so much afterwards?

Episiotomy is one of the commonest surgical procedures in the UK. Many women suffer pain for several days sometimes weeks afterwards and dyspareunia may be a problem. Absorbable suture materials appear to be preferable to non-absorbable ones. Infiltrating the perineum with normal saline before suturing may help reduce postpartum perineal pain. Omitting sutures just below the skin surface may cause less pain. More research is needed. The skill of the operator seems to be a factor. Midwives need to evaluate their practice in repairing the perineum
