A Variable Stiffness Gripper with Reconfigurable Finger Joint for Versatile Manipulations.

A reconfigurable dexterous gripper is designed which can switch states, including rigidity and flexibility, for different application scenarios. Moreover, the stiffness of the fingers in the flexible state can also be tuned for different objects. Three fingers are connected to the revolute joints of the palm, and each finger has a reshape mechanism with a slider moving up and down to lock or release the fingertip joint. When the slider moves upward, the gripper works in the rigid state and the fingers are actuated by the servos. When the slider moves downward, the gripper works in the flexible

Clinical management of finger joint capsulitis/synovitis in a rock climber.

This case study presents a 23-year-old male recreational rock climber, who climbed an average of 3-4 times per week and presented with finger joint capsulitis/synovitis after increasing his climbing intensity and training from moderate to high over 6 months, which led up to injury. During the exam, the diagnosis was ruled in with clinical orthopedic testing. Additional movement analyses revealed improper gripping mechanics contributing to asymmetric finger loading. A comprehensive rehabilitation program was developed based on the concept of a progressive framework that included unloading of th…

A new technique for metacarpophalangeal joint replantation after four-finger amputation.

Although finger amputations are not life-threatening, they are functionally and psychologically problematic for the patient. Successful replantation of amputated fingers results in better appearance and functional outcome. Therefore, the patient's satisfaction and psychological status improve. Usually, the first step of replantation involves bone and joint fixation. Various fixation methods can be used. Rapid and rigid fixation allows the surgeon to focus on repairing the soft tissues. In this report, we describe four-finger metacarpophalangeal (MCP) joint replantation with a new technique whe

Verification of Finger Joint Stiffness Estimation Method With Soft Robotic Actuator.

Stroke has been the leading cause of disability due to the induced spasticity in the upper extremity. The constant flexion of spastic fingers following stroke has not been well described. Accurate measurements for joint stiffness help clinicians have a better access to the level of impairment after stroke. Previously, we conducted a method for quantifying the passive finger joint stiffness based on the pressure-angle relationship between the spastic fingers and the soft-elastic composite actuator (SECA). However, it lacks a ground-truth to demonstrate the compatibility between the SECA-facilit…

Experimental investigation of finger dynamics before and after metacarpophalangeal joint arthroplasty.

PURPOSE: The purpose of this study was to quantify the changes in the arc of digital flexion before and after metacarpophalangeal (MCP) silicone arthroplasty with a 30 degrees preflexed design. METHODS: Index, middle, and ring fingers of 4 fresh-frozen cadaver hands were used. Each hand was attached (palmar side up) to a custom test apparatus. The tendon was drawn by a small winch-type servomotor. Micropotentiometers that were attached to the centers of rotation of the MCP, distal interphalangeal, and proximal interphalangeal joints measured angular displacement before and after MCP arthropla

[Pyrocarbon prosthesis for finger interphalangeal joint replacement. Experience after one year].

The long-term results of finger joint replacement, particularly the proximal interphalangeal (PIP) joint, have mostly been disappointing according to a review of the international literature and our own experiences. The recently developed pyrolytic carbon implants (Ascension Orthopedics, Austin, Texas) for replacing the PIP joint were evaluated retrospectively regarding range of motion, functionality, and relief of pain. In our hospital 17 PIP joints were replaced with pyrolytic carbon implants in 16 patients by an offset stem technique inserted without cement. This study included 13 patients;…

Comparing biomechanical investigations about different wiring techniques of finger joint arthrodesis.

INTRODUCTION: Multiple operative techniques are currently used for finger arthrodesis in clinical practice. The present study was designed to compare the biomechanical characteristics of typical arthrodesis techniques used in daily practice. MATERIAL AND METHODS: Osteosynthesis techniques comprising wire cerclage, thread cerclage (PDS) or intraosseous wire suture were compared in a biomechanical experiment for resistance against bending loads. The mentioned techniques were applied to artificial specimens with resected articular surfaces or by using the cup-and-cone procedure. In this process,

Proximal interphalangeal joint release in Dupuytren's disease of the little finger.

We present a prospective study, with 3-year follow-up, of the role and outcome of fasciectomy plus sequential surgical release of structures of the proximal interphalangeal joint in Dupuytren's contracture of the little finger. Our treatment programme involves fasciectomy for all patients followed by sequential release of the accessory collateral ligament and volar plate as necessary. Of the 19 fingers in the study, eight achieved a full correction by fasciectomy alone, and in these cases there was a fixed flexion deformity of 6 degrees at 3 months and 8 degrees at 3 years. The remaining 11 fi

Rotatory subluxation of the metacarpophalangeal index finger joint: a case report.

We report a case of rotatory subluxation of the metacarpophalangeal joint (MCPJ) of the finger. A 40-year-old man sustained an open injury to his index finger following an explosive injury. Radiographs showed rotatory subluxation of the index finger MCPJ. The index finger extensor digitorium was found interposed in the MCPJ, with a complete tear of the radial collateral ligament. Treatment was by open reduction and repair of the collateral ligament and the extensor tendon. A high level of clinical suspicion is needed to diagnose this entity.

A biomechanical analysis of the rheumatoid index finger after joint arthroplasty.

OBJECTIVE: To determine the mechanisms responsible for the recurrence of ulnar drift after metacarpophalangeal joint arthroplasty in the rheumatoid hand. DESIGN: A three-dimensional biomechanical model of the index finger joints was used to predict the implant loads during several activities of daily living. BACKGROUND: Post-operative clinical evaluation of Sutter metacarpophalangeal prostheses shows a high incidence of fracture and recurrent deformity. METHODS: A six-component force transducer in conjunction with a six-camera motion analysis system were used to obtain kinematic and externa…

Silicone lymphadenopathy and synovitis. Complications of silicone elastomer finger joint prostheses.

We report two complications of silicone elastomer finger joint prostheses. In one patient, the prostheses broke, with silicone particles present in synovium ("detritic synovitis"). In another patient, silicone particles were found in an axillary lymph node five years after insertion of prostheses in the ipsilateral hand (prostheses were intact at the time). Microscopically, silicone particles in synovium and lymph node were identical to particles abraded from a new prosthesis.

Three-dimensional assessment of finger individuation reveals finger- and joint-specific selective motor control deficits in pediatric cerebral palsy.

BACKGROUND: Children with cerebral palsy (CP) exhibit impaired selective motor control (SMC) that contributes to poor hand function, but current clinical assessments lack the sensitivity to detect finger- and joint-specific deficits to guide rehabilitation strategies. This study aimed to determine the internal consistency and validity of an objective, instrumented assessment of selective finger control (individuation) in children with CP, and to examine its relationship to clinical measures of upper limb function. METHODS: A custom-designed device recorded 3-dimensional isometric forces concu

Irreducible Volar Rotatory Subluxation of the Proximal Interphalangeal Joint of the Finger.

We describe 4 cases of irreducible volar rotatory subluxation of the proximal interphalangeal (PIP) joint of the finger that required open reduction. All of the patients had radiographically proven (in lateral-view radiographs) volar rotatory subluxation of the PIP joint, without fracture. The causes of irreducibility were interposition of the lateral band about the condyle of the middle phalanx in 2 cases, interposition of the collateral ligament in 1 case, and scarring of the injured central slip in 1 case. Rupture of the collateral ligament of one side was found in all cases. Acceptable res

Collateral ligament laxity of the finger metacarpophalangeal joints: an in vivo study.

PURPOSE: To assess the normal degree of laxity of the collateral ligaments (CLs) of the finger metacarpophalangeal (MCP) joints and to compare side-to-side differences in CL laxity. METHODS: One hundred subjects had measurements of the degree of laxity in the radial (RCL) and ulnar (UCL) collateral ligaments of the MCP joints of each digit on both hands with the joints at neutral and at 30° and 90° of flexion using a custom-made measuring device. Statistical analysis was performed to assess both the difference in laxity for each ligament at each position of flexion and between sides for analo

Simultaneous dorsal dislocations of the carpometacarpal joints of all four fingers.

A 57-year-old right-hand-dominant woman was involved in a motor vehicle collision. Upon examination, her right hand was markedly deformed and swollen, with limited range of movement. Plain radiographs revealed dorsal dislocations of the index, long, ring, and small finger carpometacarpal joints and an avulsion fracture of the dorsal aspect of the capitate. Closed reduction was unsuccessful. Closed reduction under general anesthesia was successful on the carpometacarpal joint of the ring and small fingers, however, the long and index fingers remained irreducible. An open approach revealed that
