Large language models and global health equity: a roadmap for equitable adoption in LMICs.

UNLABELLED: Large language models (LLMs) have been proposed to address global health inequity by providing accessible and high-quality health care, particularly in low- and middle-income countries (LMICs). However, despite the early enthusiasm following the release of GPT, development and deployment of LLMs have remained heavily concentrated in high-income countries (HIC), raising concerns that such technology may worsen existing health disparities instead of alleviating them. The most recent LLMs, which include features such as lower cost, and open-source framework, show promise in rebalancin

Coping with sickle cell disease: a profile and perspective of a pioneer self-help group.

Sickle cell anemia is a chronic, debilitating disease that is passed genetically from generation to generation. It is a disease marked by periods of well-being and crisis, and it has a profound effect on all bodily organs, shortening the lifespan of its victims. The disease also has far-reaching effects on family functioning and relationships. Support for affected families and individuals is therefore a vital component of any management regimen. In the 1960s, the idea for the Association for Sickle Cell Anemia Research (ASCAR) was implemented. The group was spearheaded by Dr. Roland B. Scott an

Scalable, sustainable cost-effective surgical care: a model for safety and quality in the developing world, part I: challenge and commitment.

BACKGROUND: With an estimated backlog of 4,000,000 patients worldwide, cleft lip and cleft palate remain a stark example of the global burden of surgical disease. The need for a new paradigm in global surgery has been increasingly recognized by governments, funding agencies, and professionals to exponentially expand care while emphasizing safety and quality. This three-part article examines the evolution of the Operation Smile Guwahati Comprehensive Cleft Care Center (GCCCC) as an innovative model for sustainable cleft care in the developing world. METHODS: The GCCCC is the result of a unique

Corporate boxing matches for charity: a personal point of view.

Boxing and other combat sports receive constant exposure and support in the media. Corporate boxing matches are a popular method of raising money for charity. However, charity fights are more dangerous for the contestants than other sanctioned bouts. In a world where head trauma and concussion in sports is under constant scrutiny, the ethics of raising funds by deliberate concussive forces to the head needs to be reassessed.

The implications of US experiences with early childhood interventions for the UK Sure Start Programme.

BACKGROUND: The UK Government introduced a large-scale early childhood intervention programme, Sure Start, in 1999. Sure Start is to be further expanded, to achieve national coverage. US experience is highly relevant for anticipating challenges that this expansion will raise. METHODS: This is a focused, narrative review. We examine the impact, funding, quality-improvement and programme objectives of Head Start and Early Head Start programmes. RESULTS: (1) Early childhood interventions can make a significant difference to children's life chances; (2) expansion without adequate funding threate

Voluntary rewards mediate the evolution of pool punishment for maintaining public goods in large populations.

Punishment is a popular tool when governing commons in situations where free riders would otherwise take over. It is well known that sanctioning systems, such as the police and courts, are costly and thus can suffer from those who free ride on other's efforts to maintain the sanctioning systems (second-order free riders). Previous game-theory studies showed that if populations are very large, pool punishment rarely emerges in public good games, even when participation is optional, because of second-order free riders. Here we show that a matching fund for rewarding cooperation leads to the emer

Following the money: Mapping the sources and funding flows of alcohol and other drug treatment in Australia.

INTRODUCTION AND AIMS: The structures of health systems impact on patient outcomes. We present and analyse the first detailed mapping of who funds alcohol and other drug (AOD) treatment and the channels and intermediaries through which funding flows from the funding sources to treatment providers. DESIGN AND METHODS: The study involved a literature review of AOD treatment financing and existing diagrammatic representations of the structure of the Australian health system. We interviewed 190 key informants to particularise the AOD treatment sector, and undertook two case examples of government

Critical Shortfall in NIH Support for Medical Physics Research.

This report summarizes changes in federal research funding to the medical physics community between FY24 and FY25. By linking the AAPM membership database with NIH RePORTER records, we quantified the distribution of NIH funding for projects led by AAPM researchers. Although total NIH funding to AAPM members remained relatively stable across the two years, the composition of that funding shifted substantially. Competing (new and renewal) awards declined 50%, driven largely by an 80% collapse in new R01 grants from the National Cancer Institute (NCI). In contrast, noncompeting continuation award

Are NHS cleft services in England ready for delegation to integrated care systems?

Cleft care services in the UK have been nationally funded since centralisation 25 years ago and during this time have been able to demonstrate improved clinical outcomes. Integrated care systems have been introduced into legislature as part of the Health Care Act of 2022 and will be responsible for the paradigm shift of allocating funds on a regional basis for cleft care services in England from 2024. The proposed population-based funding formulas present an opportunity to improve current inequities in cleft care, including access to speech therapy and adult services. However, the regional foo

[Comparative evaluation of maternity hospitals in Auvergne: from planning to contracting].

Two reforms of public hospitals have been launched by the French government in 1991 and 1996 aimed at lowering costs and increasing the quality of services and ultimately the safety of patients. As concerns maternity hospitals, several new rules have been imposed upon. For example, those who performed less than 300 births a year should be closed. The basic idea was to concentrate technical resources and human skills in middle-size and important hospitals for saving money, and simultaneously, raising the safety level for mothers and babies. However, negative adverse effects fastly appeared: to

The competition and cooperation mechanism of policy goals in China's diagnosis-related group: a quantitative analysis based on the BERTopic and Lotka-Volterra models.

BACKGROUND: Aiming at the practical problems such as upcoding and decomposing hospitalizations emerging in China's Diagnosis-Related Group (DRG) reform, existing studies mostly attribute such issues to aspects like behavioral motivations, which fail to explain the root causes of the problems. METHODS: This paper takes policy goals as the research starting point and constructs a three-dimensional analytical framework of "identifying goals-quantifying relationships-explaining mechanisms". Based on 377 DRG policy texts covering the central, provincial and municipal levels from 2009 to 2025, this

Why public funding for non-invasive prenatal testing (NIPT) might still be wrong: a response to Bunnik and colleagues.

Bunnik and colleagues argued that financial barriers do not promote informed decision-making prior to prenatal screening and raise justice concerns. If public funding is provided, however, it would seem to be important to clarify its intentions and avoid any unwarranted appearance of a medical utility of the testing.

Analyzing media coverage of the global fund diseases compared with lower funded diseases (childhood pneumonia, diarrhea and measles).

BACKGROUND: Pneumonia, diarrhea and measles are the leading causes of death in children worldwide, but have a disproportionately low share of international funding and media attention. In comparison, AIDS, tuberculosis and malaria--diseases that also significantly affect children--receive considerably more funding and have relatively high media coverage. This study investigates the potential relationship between media agenda setting and funding levels in the context of the actual burden of disease. METHODS: The news databases Lexis Nexis, Factiva, and Google News Archive were searched for the

"A National Review of Crowdfunding for Plagiocephaly Helmets".

ObjectiveCustom prescription helmets for plagiocephaly may be a significant financial burden for families, especially when not covered by insurance. This study aims to identify factors that influence the success of crowdsourcing campaigns for this therapy.DesignGoFundMe campaigns were collected by searching terms such as "plagiocephaly" and "baby helmet." Two reviewers analyzed each campaign for variables, including demographic data, story elements, and photo characteristics. Univariate logistic regression was used to determine each variable's impact on success, defined as attaining ≥75% of a

From progress to precarity: how funding shifts threaten HIV gains in Nigeria.

Sustained financing remains critical to the global HIV response, particularly in low- and middle-income countries (LMICs) where external aid supports treatment and prevention programs. Recent shifts in donor priorities have raised concerns about the continuity of essential HIV services. Using Nigeria as a case study, this paper examines how declining international HIV funding threatens HIV control efforts through increased transmission risk, treatment interruption, and strain on fragile health systems. Available evidence indicates that funding reductions have disrupted community-based HIV test
