Malignant hyperthermia is a pharmacogenetic disease of skeletal muscle characterized by hypermetabolism that occurs on exposure to a triggering agent or agents. The most common agents are halogenated inhalational anesthetics and succinylcholine, a depolarizing muscle relaxant. Patients who experience malignant hyperthermia are generally transferred to the ICU for ongoing treatment and monitoring for secondary complications of the disorder. Critical care practitioners must be both knowledgeable and competent to prevent and treat perioperative episodes of malignant hyperthermia. A thorough preop…

Malignant hyperthermia, although an uncommon condition, has dramatic consequences when it occurs. The exact cause of MH is unknown but it is triggered by agents such as suxamethonium and volatile anaesthetic agents, especially Halothane. The condition can occur at anytime during the perioperative period and practitioners need to be vigilant towards its signs and symptoms as well as fully aware of the need for prompt action should it occur. The management of MH also includes identifying those at risk and taking measures to minimise these risks during anaesthesia.

Major advances have occurred in the management of MH since this disorder was first described. Despite the initially high mortality, deaths secondary to MH have dramatically decreased over the past 15 years, which is due in large part to the discovery of efficacious therapy. It is also a result of ever-increasing awareness and appreciation of the syndrome by physicians, other health care providers, and patients. Research has provided many answers to questions about the pathologic mechanisms of this disorder, and an animal model exists for testing new therapies; however, many questions remain. I

In humans genetically predisposed to malignant hyperthermia, anesthesia can induce skeletal muscle rigidity, hypermetabolism, and high fever, which, if not immediately reversed, can lead to tissue damage or death. The corresponding condition in swine leads to stress-induced deaths and devalued meat products. Abnormalities in the Ca2+ release channel of skeletal muscle sarcoplasmic reticulum (the ryanodine receptor) have been implicated in the cause of both the porcine and human syndromes by physiological and biochemical studies and genetic linkage analysis. In swine, a single founder mutation …

Malignant hyperthermia is a hereditary trait characterized by hypercatabolic reactions induced by anesthetic drugs, or physical or emotional stress. Patients must be treated quickly and efficiently in order to prevent irreversible organ damage and death.
