Plagiocephaly and Developmental Delay: A Systematic Review.

OBJECTIVE: Deformational plagiocephaly (includes plagiocephaly and brachycephaly) is a common pediatric condition. Infants who present with altered head shape often experience developmental delay. It is uncertain how common developmental delay is in infants with plagiocephaly and how sustained this is, when present. This review explores the association between plagiocephaly and developmental delay to guide clinical practice. STUDY DESIGN: A systematic review was conducted. MEDLINE, EMBASE, CINAHL, and PEDro databases were searched. Data from relevant studies were extracted regarding study: sa

[A clinical analysis of 101 infants with plagiocephaly].

OBJECTIVE: To investigate the possible causes of plagiocephaly in infants and the therapeutic effect of postural correction training on plagiocephaly. METHODS: A total of 101 infants who were diagnosed with plagiocephaly were enrolled. According to the age at diagnosis, these infants were divided into 1-4 month group (31 infants), 5-8 month group (40 infants), and 9-12 month group (30 infants). The possible causes of plagiocephaly were analyzed in three groups. The cranial vault asymmetry index (CVAI) before and after postural correction training was compared in three groups. RESULTS: Of the

Uvular malformation in the presence of deformational plagiocephaly.

BACKGROUND: Deformational plagiocephaly is cranial asymmetry caused by external forces on the skull. Deformational plagiocephaly is seen in 5% to 48% of healthy newborns. Incomplete uvular fusion, in contrast, is one of many uvular malformations. The incidence of all degrees of incomplete uvular fusion is approximately 1% in healthy children. Bifid uvula is a malformation that is often considered a microform cleft palate or a marker for submucous cleft palate. METHODS: This is a retrospective study of patients with deformational plagiocephaly seen at the Upstate Cleft and Craniofacial Center

Orthotic (helmet) therapy in the treatment of plagiocephaly.

OBJECT: The goal of this study was to review the current literature on orthotic (helmet) therapy use in the treatment of deformational plagiocephaly. METHODS: PubMed was used to search English articles using the medical subject headings "deformational plagiocephaly" and "orthosis," and "deformational plagiocephaly" and "helmet." RESULTS: Forty-two articles were found. There were no Class I studies, 7 Class II studies, 1 Class III study, and 13 Class IV studies. Cranial orthoses have been shown to be effective in treating deformational plagiocephaly. It continues to be debated as to whether t

[Positional plagiocephaly in infants can be prevented].

The prevalence of positional plagiocephaly is increasing. A systematic database search (Cochrane, MEDLINE, Embase, Cinahl and PEDro), critical appraisal and systematic evaluation of the quality of 14 selected studies show that the following are certain risk factors for the development of positional plagiocephaly: supine sleeping position, positional preference, one-sided handling of the child, male sex, delayed motor development, reduced neck mobility, too little "tummy time", firstborn, multiple births and premature children. Recommendations for prevention are provided.

Development in toddlers with and without deformational plagiocephaly.

OBJECTIVE: To determine whether the heightened risk of developmental delays seen in infancy in patients with deformational plagiocephaly (DP) continues into the toddler years. DESIGN: Longitudinal study comparing the development of children with and without DP, with assessments in infancy (mean age, 7 months) and at age 18 months. SETTING: Infants with DP were recruited from a large craniofacial center, and unaffected infants were recruited from a research registry. PARTICIPANTS: The study included 227 children with DP and 232 children without previously diagnosed DP. MAIN EXPOSURE: Diagno

Motor development of infants with positional plagiocephaly.

Concurrent with recommendations to place infants to sleep in supine, there has been a dramatic increase in the number of infants with positional plagiocephaly (PP). Recent evidence suggests that infants who have decreased exposure to prone position may have a higher incidence of PP and may be at risk for a delay in the acquisition of certain motor skills. The purpose of this study was to compare motor development between infants with PP and matched peers without PP. We also examined differences in infant positioning practices when asleep and awake between the two groups. Twenty-seven infants w

Neurologic findings in infants with deformational plagiocephaly.

This study evaluates the neurologic profiles of infants with deformational plagiocephaly. Forty-nine infants with deformational plagiocephaly between the ages of 4 and 13 months (mean age, 8.1 months) are evaluated, along with 50 age-matched control subjects (mean age, 8.1 months). A modified version of the Hammersmith infant neurologic assessment was performed on each infant. A caregiver completed a questionnaire regarding the infant's prematurity, development, and health to date. Results are analyzed using t test. There is a statistically significant difference in overall neurologic assessme

MDCT diagnosis of the child with posterior plagiocephaly.

OBJECTIVE: In this article, we review the normal anatomy and development of the posterior skull base and describe distinguishing imaging features of the two most common causes of posterior plagiocephaly: posterior deformational plagiocephaly and unilateral lambdoid synostosis. We also describe three unusual cases of posterior plagiocephaly, including asymmetric premature fusion of the anterior and posterior intraoccipital synchondroses, with diagnoses enabled by volume-reformatted MDCT. CONCLUSION: Three-dimensional reformatted MDCT enables accurate diagnosis of common and rare causes of post

Development of plagiocephaly in rabbits with unicoronal synostosis.

Human unicoronal synostosis results in plagiocephaly of the cranial vault due to predictable compensatory growth patterns of the contralateral coronal, sagittal, and ipsilateral squamosal sutures. The present study describes the development of plagiocephaly and tests compensatory growth predictions in a naturally occurring rabbit model of uncorrected unicoronal synostosis. Cranial vault and sutural growth data were collected from serial x-ray films in 70 normal rabbits and 19 rabbits with congenital unicoronal synostosis from 1.5 to 18 weeks of age. One-way analysis of variance results reveale

Skeletal and functional craniofacial adaptations in plagiocephaly.

The present report aims to contribute to our understanding of craniofacial development in plagiocephaly. A previously unreported dry skull with plagiocephaly and two clinical cases with unoperated plagiocephaly are presented. The clinical cases were followed longitudinally with roentgencephalometry in lateral, frontal, and axial projections. In addition, in one of the cases, electromyographic analysis of the temporal, masseter, sternomastoid, and trapezius muscles was carried out. The dry skull revealed premature closure of the sphenofrontal suture in addition to the coronal suture. Furthermor

[Early treatment of orbital asymmetry in plagiocephaly].

Plagiocephaly results from premature unilateral closure of the coronal suture with subsequent frontal and orbital asymmetry and inequality of ocular levels requiring complex osteotomies if treatment is delayed. From 1976, early treatment from the age of approximately 6 months has been instituted by means of bilateral frontal remodelling. Results in 25 neonates in whom the operation was performed demonstrated that not only was the frontal correction maintained with growth, but that principally the facial asymmetry was reduced and later orbital surgery was never necessary. More complicated probl

Unilateral isolated frontosphenoidal craniosynostosis causing frontal plagiocephaly.

Synostotic frontal plagiocephaly is most commonly caused by frontoparietal (unicoronal) synostosis, but may be caused by other fusions along the coronal hemiring. Frontosphenoidal synostosis is a rare cause of frontal plagiocephaly, with only five previously reported cases. We present the case of an 8-month-old male infant with frontal plagiocephaly caused by unilateral isolated frontosphenoidal synostosis mimicking frontoparietal (unicoronal) synostosis. The clinical and radiological features included flattened ipsilateral forehead, retruded ipsilateral maxilla, a midline chin, contralateral
