Implanting phakic IOLs (PIOLs) in phakic eyes is regarded as an established option in refractive surgery for many years when treating high myopia, hyperopia or ametropia. Even for eyes which show a combination of refractive errors (including astigmatism) this can be an appropriate option. The surgery itself requires certain anatomical and refractive requirements, since, as with any intraocular procedure, there are different possibilities of complication depending on the chosen IOL. Therefore, any potential candidate for refractive surgery should be treated in accordance with the current recomm

Cataract surgery combined with glaucoma surgery.

PURPOSE OF REVIEW: This review evaluates the surgical management of glaucoma in patients undergoing cataract surgery. Combining both procedures present challenges in balancing intraocular pressure (IOP) control, minimizing postoperative complications, and achieving optimal refractive outcomes. RECENT FINDINGS: Recent studies highlight the effectiveness of combined glaucoma and cataract surgeries, with traditional filtering surgeries and MIGS showing significant IOP reduction. The abundance of long-term studies shows that MIGS can offer an effective and safer alternative when carefully tailore

Ablation design in relation to spatial frequency, depth-of-focus, and age.

PURPOSE: To consider factors relevant to the design of wavefront-aberration-based customized ablations. METHODS: Review. RESULTS: Ablations that seek to eliminate all wavefront aberrations, both second- and higher-order, may not be optimal for all patients. This is particularly the case for presbyopes. Their main requirement will normally be for extended binocular depth-of-focus to yield adequate distance and near vision with good retinal contrast at lower spatial frequencies, rather than the highest levels of acuity and modulation transfer function at a single distance. For many presbyopes,

Making sense of refractive surgery in 2001: why, when, for whom, and by whom?

Surgical alteration of the focusing or refractive properties of the eye has been performed on millions of patients. An array of procedures to correct myopia, hyperopia, astigmatism, and presbyopia have been introduced over the past 25 years with varying degrees of success. Improved technology has increased patient and physician satisfaction and enthusiasm. Currently available surgical procedures can be categorized as incisional, surface-altering, lamellar, and intraocular. The choice of procedure depends on individual patient indications and contraindications based on results of ocular examina

Surgical Options for the Refractive Correction of Keratoconus: Myth or Reality.

Keratoconus provides a decrease of quality of life to the patients who suffer from it. The treatment used as well as the method to correct the refractive error of these patients may influence on the impact of the disease on their quality of life. The purpose of this review is to describe the evidence about the conservative surgical treatment for keratoconus aiming to therapeutic and refractive effect. The visual rehabilitation for keratoconic corneas requires addressing three concerns: halting the ectatic process, improving corneal shape, and minimizing the residual refractive error. Cross-lin

Surgically induced astigmatism made easy: calculating the surgically induced change in sphere and cylinder for corneal incisional, corneal laser, and intraocular lens-based surgery.

Net cylinder and spherocylinder formats characterize individual keratometries and prescriptions but must be converted to dioptric vectors to allow for calculations and statistical analyses. Næser's polar value system was specifically developed for the analysis of the surgically induced refractive change along the surgical meridian. This study provides a short description together with a practical manual and a computer program for the use of this dioptric vector method. Measurement techniques, vector equations, statistical methods, and terminology are reviewed. The analysis is identical for cor

Review of current status of refractive lens exchange and role of dysfunctional lens index as its new indication.

Advances in phacodynamics and intraocular lenses (IOLs) has given second life to clear lens extraction (CLE) or refractive lens exchange (RLE) in recent years for the treatment of patients with high degrees of myopia, hyperopia, and astigmatism who are unsuitable for laser surgery. Furthermore, presbyopia treatment with RLE supplemented with multifocal or accommodating IOLs gives the dual benefit of correcting refractive errors with eliminating the need for cataract surgery. RLE should be consistent and effective for a good refractive outcome along with safety during the surgical procedure and

Artificial intelligence in the diagnosis and management of refractive errors.

Refractive error is among the leading causes of visual impairment globally. The diagnosis and management of refractive error has traditionally relied on comprehensive eye examinations by eye care professionals, but access to these specialized services has remained limited in many areas of the world. Given this, artificial intelligence (AI) has shown immense potential in transforming the diagnosis and management of refractive error. We review AI applications across various aspects of refractive error care - from axial length prediction using fundus images to risk stratification for myopia progr

[Correction of refractive errors by subtractive corneal surgery: History and perspectives].

The goal of this review is to trace the history of refractive correction, from the earliest optical devices to modern refractive surgical techniques. Following the invention of spectacles in the Middle Ages, the first corneal procedures, such as Sato's and Fyodorov's keratotomies and Barraquer's keratomileusis, highlighted the limitations of early mechanical approaches. In the 1980s, the advent of the excimer laser revolutionized refractive surgery with the introduction of PRK (photorefractive keratectomy) and subsequently LASIK (Laser-Assisted In Situ Keratomileusis). The development of the f

Corneal Topography for Intraocular Lens Selection in Refractive Cataract Surgery.

The purpose of this review is to evaluate the usefulness of corneal topography to select premium intraocular lenses (IOLs), including aspherical IOLs, toric IOLs, and multifocal IOLs, in refractive cataract surgery. Corneal topography can detect corneal regular astigmatism, corneal irregular astigmatism (higher-order aberrations [HOAs]) including spherical aberration, and corneal shape abnormalities after corneal refractive surgery. Surgeons can explain to the patients with significant corneal HOAs about its effect on postoperative visual function before surgery. Multifocal IOLs should not be

Ultrasound biomicroscopic assessment of the cornea following excimer laser photokeratectomy.

Excimer laser photokeratectomy is used as a refractive tool and in the treatment of superficial corneal disease. Ultrasound biomicroscopy is a new method of ultrasound imaging developed in our laboratories that allows subsurface imaging of ocular structures at microscopic resolution. We used this imaging method to examine 12 patients following excimer laser keratoablation. Following treatment, the normal appearance of a highly reflective Bowman's membrane below the epithelial echo was absent in the treatment zone. Corneal thinning was also noted. Postoperative scarring in the treatment area co

Aberrometry: basic science and clinical applications.

This paper addresses the concept, measurement and applications of wave aberrations. The ocular optics is not perfect. Apart from conventional low order aberrations (such as defocus and astigmatism) retinal images are degraded by other high order aberrations. Aberrometers typically measure ray deviations at the retinal plane, as a function of pupil position, i.e. local derivatives of the wave aberration. The paper discusses state of the art technology to measure the aberrations of the eye (cornea and crystalline lens), both monochromatic and polychromatic. The aberrometers described include the

Intracorneal Inlays for the Correction of Presbyopia.

The current nonsurgical approaches for the correction of presbyopia are spectacles or contact lenses or the use of pharmaceuticals to create an artificial pupil and/or to stimulate residual accommodation. Refractive surgical procedures use a monovision approach and/or a multifocal corneal laser ablation with or without the induction of negative spherical aberration to improve near vision. More recently, new surgical approaches include intracorneal inlays. Inlay approaches include increasing corneal curvature alone, implanting a multifocal inlay, or by implanting a small aperture device that fu…

[The basics of refractive lens exchange].

Removal of the crystalline lens and its replacement by an intraocular lens (IOL) is known as refractive lens exchange (RLE). RLE is performed to correct high ametropia or aniseikonia. Crystalline lens removal always leads to a complete loss of accommodation; therefore, RLE should mainly be used for patients with beginning or existing presbyopia. RLE patients usually have a good best-corrected visual acuity, which is the default value for the postoperative uncorrected visual acuity. To reach this goal, microincisional and astigmatism-neutral implantation techniques as well as special IOL optic
