PURPOSE: To examine the vascular density in different retinal layers and in the choriocapillaris in eyes with retinal vein occlusions (RVO).
METHODS: Applying optical coherence tomography angiography (OCTA), we examined patients with unilateral RVOs and normal individuals of a control group.
RESULTS: The study group included 48 patients with unilateral RVO and the control group 17 normal individuals. Eyes affected by RVO as compared to the contralateral unaffected eyes (all P < 0.001), and the contralateral unaffected eyes as compared to the eyes of the control group (P < 0.05), showed a low…

Two illustrative cases of chronic branch retinal vein occlusion are presented with multimodal imaging, including commercially available optical coherence tomography angiography. In these two patients, retinal ischemia and collateral vessels were well imaged without the need to use traditional fluorescein angiography. Optical coherence tomography angiography provides useful information for the diagnosis and management of patients with branch retinal vein occlusion and other retinal vascular diseases.

CASE REPORT: Branch retinal vein occlusion (BRVO) is believed to arise at arteriovenous crossing sites. Surgical dissection of the arteriovenous sheath has been proposed as a treatment option, yet induction of a posterior vitreous detachment may be as important in obtaining a successful surgical outcome, suggesting that vitreoretinal traction may play a role in the development of BRVO. A retrospective review of 3 patients presenting with clinical features of BRVO and evidence of vitreoretinal traction at the occlusion site was conducted.
COMMENTS: All patients presented with mild vitreous hem…

BACKGROUND: Various complications for choroidal melanoma have been reported. However, there are no reports on choroidal melanoma complicated with central retinal vein occlusion.
CASE REPORT: A 37-year-old male showed an elevated yellowish white tumor of about 8 discs in diameter, and involving the optic disc in the left eye. Serous retinal detachment and subretinal exudate were noted around the tumor. Central retinal vein occlusion occurred concurrently. Fluorescein angiography revealed blocks due to hemorrhage and fluorescence leakage from the retinal vein. Dilation of retinal capillary vess…

On the basis of literature review the role of fibrinolysis in the pathogenesis of central retinal vein thrombosis is presented. It has been proved by many authors that fibrinolytic activity is depressed in patients with vascular thrombosis, due to increased levels of fibrinolytic inhibitors mainly plasminogen activator inhibitor (PAI 1), and/or decreased levels of tissue plasmogen activator (t-PA). Low levels of t-PA and increased levels of PAI-1 have been found in states of special risk of thrombosis: obesity, diabetes mellitus, postoperative states, rheumatoid arthritis, malignancies and oth…

Behçet's disease is a multisystem disorder affecting the skin, mucous membranes, eye, joints, central nervous system, and blood vessels. One of the known vascular complications of Behçet's disease is venous thrombosis or aneurysm formation. We report, herewith, a patient with Behçet's disease who developed radial artery aneurysm, deep venous thrombosis, and bilateral central retinal vein thrombosis. To our knowledge, this is the first report of bilateral central retinal vein thrombosis in association with Behçet's disease.

Little is known about the behaviour of central retinal vein occlusion (CRVO) in the young. We studied the course and visual outcome in 25 patients with CRVO aged less than 40 years with special reference to the association with underlying systemic disorders. Eight patients had ischaemic CRVO while 17 had non-ischaemic CRVO. Systemic disorders associated with ischaemic CRVO included end-stage renal disease in 3, and hypercholesterolaemia, beta-thalassaemia and hypertension in 1 each. All patients with ischaemic CRVO had unusually severe disease and a rapid downhill course. Management included p…

Waldenström's macroglobulinemia is a rare progressive neoplastic plasma cell disorder. Hyperviscosity is a major component in primary macroglobulinemia due to the excessive production of IgM paraprotein, a tumor by-product. The ocular manifestations of hyperviscosity can include congestion of the conjunctival vessels, dilation, segmentation and tortuosity of retinal vessels, retinal hemorrhages and serous retinal detachment. This case of bilateral central retinal vein occlusion represents an uncommon complication of Waldenström's macroglobulinemia.

Electroretinograms (ERGs) were measured in 15 patients with central retinal vein occlusion (CRVO). Seven of the patients had neovascularization of the iris (NVI) at the time of testing, and two developed NVI within one month of testing; six did not have NVI or any other form of neovascularization (NV) and were no longer considered to be at risk for NV from their present occlusion. The ERGs were recorded as a function of the stimulus intensity and to a 30-Hz flickering stimulus. A Naka-Rushton-type function was fit to b-wave amplitudes, measured as a function of stimulus intensity, to evaluate …

Seventeen patients with recurrent retinal vein occlusion were investigated for underlying medical conditions and compared with 61 patients with single retinal vein occlusion (26 with central, 35 with branch vein occlusion). The two study groups were comparable for age, sex, and weight. Patients with recurrence had a significantly increased prevalence rate of hypertension (88% versus 48%: p less than 0.01), with a trend to increased hyperlipidaemia (47% versus 33%) compared with patients with a single episode. A significantly raised mean systolic (p less than 0.05) but not diastolic blood press…

PURPOSE: The rate and magnitude of spontaneous visual recovery are very poor in central retinal vein occlusion (CRVO). In the first follow-up year, the Central Vein Occlusion Study Group reported that only 6% of eyes recovered > or = 3 lines of vision and none recovered > or = 8 lines of vision. Retinal endovascular surgery (REVS) is vitrectomy followed by cannulation of retinal vessels with injection of tissue plasminogen activator. After reports of one surgeon's experience suggested that the procedure promotes recovery of vision, other surgeons began to offer REVS to their patients. This rep…

BACKGROUND: The influence of retinal artery circulation on central retinal vein occlusion (CRVO) is not well known.
CASE: A 73-year-old man with hypertension complained of transient visual loss in his left eye. At initial examination, ophthalmoscopy of his left eye showed mild dilatation and tortuosity of the retinal vein and a few dot hemorrhages. Fluorescein angiography disclosed normal arm-to-retina circulation time and no leakage of dye, but delayed retinal circulation time. These findings indicated venous stasis retinopathy of CRVO. In spite of intravenous infusion of prostaglandin E1 an…

A 81-year-old woman developed central retinal vein occlusion (CRVO) in her left eye subsequent to central retinal artery occlusion (CRAO). She noticed sudden visual loss in the left eye. At initial examination, her left visual acuity was 0.03, and only a small visual field was preserved at temporal area. The left fundus showed findings of mild non-ischemic CRVO. In addition white clouded retina was recognized at the left posterior pole which indicated CRAO. FAG showed remarkably prolonged arterial circulation, but no retinal capillary obliteration. Then retinal hemorrhage increased rapidly and…
