BACKGROUND: Ureteroscopic lithotripsy (URSL) is a common surgical treatment for ureteral stones. Although flexible ureteroscopy can be used to treat ureteral and renal stones in a single procedure, rigid ureteroscopy can only be used to treat ureteral stones; nonetheless, rigid ureteroscopy remains mainstream in Taiwan owing to its late introduction and flexible ureteroscopy is not covered by national health insurance. Hematuria is a common complication that occurs when the scope passes through stricture sites or when mucosal damage occurs during lithotripsy, but this is usually self-limited.

INTRODUCTION: Flexible ureteroscopes are not tailored for pediatric ureteral size and may not pass at first attempt into the pediatric ureter. Administration of tamsulosin preoperatively in adults has been shown to facilitate ureteral access sheath placement. Several studies have shown tamsulosin to be safe and effective when utilized for medical expulsive therapy in pediatric patients, but its utility for preoperative ureteral dilation has not been studied to date.

OBJECTIVE: To summarize the experiences of pneumatic lithotripsy under ureteroscope for pyonephrosis due to calculus obstruction.
METHODS: Twelve patients with pyonephrosis due to calculus obstruction were treated using endoscopic manipulation such as ureteroscopy, pneumatic lithotripsy and indwelling double J catheter.

There were 62 patients referred to our institution for evaluation of upper urinary tract filling defects. These patients had undergone any of several diagnostic evaluations, including computerized tomography, B-mode ultrasound scanning and urinary cytology studies with or without upper tract brush biopsy. A case is reported of inverted papilloma of the renal pelvis in a patient with previous transitional cell carcinoma of the bladder. Upper tract tumors may be diagnostic problems, and the importance of obtaining tissue in a closed fashion has been illustrated. This is probably the most important advantage of the ureteroscopic approach to these tumors. Ureteroscopy is safe and reliable and allows examination of about 95 per cent of all patients evaluated. Use of a rigid ureteroscope for extraction of 19 ureteric and 6 renal pelvic stones is reported. One of the 24 patients had bilateral ureteric stone. The extraction was successful in 22 cases, including 9 after ultrasonic disintegration of stone.
