BACKGROUND: Left bundle branch area pacing (LBBAP) may restore a more physiological pattern of ventricular activation in patients with conduction delay; however, QRS narrowing alone may incompletely characterize electrical resynchronization. Ultra-high-frequency ECG (UHF-ECG) provides quantitative markers of ventricular activation timing and dyssynchrony.
OBJECTIVE: To quantify paired OFF-to-ON changes in conventional ECG and UHF-ECG metrics during LBBAP in patients with baseline wide QRS and to assess the relationship between paced R-wave peak time (RWPT) and residual UHF-ECG dyssynchrony.

We present an illustrative case of a patient with intermittent left bundle branch block (LBBB) that underwent electrophysiological study with detailed multielectrode recordings of the right and left septum. The case elucidates the nature and location of conduction block and provides further insight into the underlying mechanisms of LBBB.

AIMS: P-wave morphology and PR interval have both been previously associated with atrial fibrillation (AF). We hypothesized that incomplete right bundle branch block (IRBBB) would be associated with early-onset lone AF.
METHODS AND RESULTS: We conducted a case-control study comparing electrocardiographic (ECG) markers from patients with early-onset lone AF and from a healthy control population. We included 187 patients with early-onset lone AF and 383 healthy controls. Sixty-two lone AF patients were excluded from the study because of AF at the time of enrolment or because of the use of antia

We report a rare combination of a right atriofascicular Mahaim fiber and two left-sided atrioventricular accessory pathways in a 57-year-old female presenting with an antidromic atrioventricular reciprocating tachycardia. Radiofrequency ablation was first targeted at the left lateral accessory pathway that served as the retrograde limb of the tachycardia. After elimination of the left lateral pathway, a bystander left posterolateral pathway was detected, and it too was successfully ablated. Although no tachycardia was reinducible, the Mahaim pathway was ablated because of its short effective r

We report the pathologic changes of the cardiac conduction tissue (CS) in 33 cases of sudden cardiac death. The cases were divided into two groups: those with severe atherosclerotic coronary lesion (> 75% cross-sectional area stenosis of at least one coronary artery), 15 cases, and those without significant coronary disease, 18 cases. In the first group, we observed major changes of the CS in eight cases. The changes consisted of abnormalities in His bundle position (branching bundle in pars membranacea or left-sided bundle), atrioventricular (AV) node fibrosis, and interatrial lipoma. Other a

A survey is given on the method of the transvasal intracardiac ablation in the treatment of atrial and ventricular tachyarrhythmias. Experimental fundaments, indication, method and possible complications are reported. Own results on five patients with His bundle ablation are described. A possible further development of the method is the enlargement of the His bundle ablation to the ablation of focal arrhythmias, Kent's fibres and ventricular tachycardias without evocation of a total atrioventricular block. Alternative techniques of the cauterization, the laser ablation and the high frequency c

A 1.5-year-old, female-spayed mix-breed dog was presented with recurrent episodes of shaking and excessive panting attributed to drug-refractory ventricular arrhythmia (VA) characterized predominantly by incessant periods of ventricular bigeminy. The VA had a narrow QRS morphology, suggestive of an origin near the His bundle or fascicular system. Diagnostic evaluation found no structural heart disease or underlying etiology. Three-dimensional electroanatomic mapping and radiofrequency catheter ablation were pursued. Voltage mapping demonstrated normal bi-ventricular voltage (≥1.5 mV) without a

Left bundle branch area pacing is an innovative technique employed to maintain left ventricular synchrony. However, the operator can face challenges in penetrating the lead into the septum during screwing. Two pitfalls-helix-only penetration and shallow penetration-hinder lead penetration. The research, conducted in 2 phases on patients and swine hearts, tested 2 techniques to overcome these issues. The results highlight the impact of endocardial involvement and suggest that the optimal technique involves intentionally creating an endocardial breach rather than simply unscrewing and rescrewing

PURPOSE OF THE REVIEW: Dyssynchrony occurs when portions of the cardiac chambers contract in an uncoordinated fashion. Ventricular dyssynchrony primarily impacts the left ventricle and may result in heart failure. This entity is recognized as a major contributor to the development and progression of heart failure. A hallmark of dyssynchronous heart failure (HFd) is left ventricular recovery after dyssynchrony is corrected. This review discusses the current understanding of pathophysiology of HFd and provides clinical examples and current techniques for treatment.
