Background: Giant hypothalamic hamartomas (HHs) are extremely rare lesions, for which the treatment is challenging. While minimally invasive treatments such as radiofrequency thermal coagulation and laser ablation have improved seizure outcomes, multiple operations are often required. This study investigated the value of one-stage stereo-array radiofrequency thermocoagulation based on stereotactic electroencephalography (SEEG) for pediatric giant HHs. Methods: We analyzed the clinical data of six patients with giant HHs (masses with a maximum diameter >30 mm) who underwent stereotactic electro

Variations in coronary sinus (CS) anatomy can make subclavian vein approach dif?cult or even impossible for LV lead delivery. A combination of interventional and electrophysiological methods is therefore the state of art technique for implantation of LV leads. A 52 year-old male patient with ischemic dilated cardiomyopathy (LVEF: 15%, QRS: 160 msec) who was symptomatic under optimal medical therapy was hospitalized for implantation of cardiac resynchronization therapy. Although right ventricular and atrial electrodes were implanted successfully, we had difficulty during placement of the left v

A 4-year-old girl with congenital profound deafness underwent cochlear implant surgery. Preoperative CT and MRI revealed that her inner ears had common-cavity or aplasia-type malformation. The bilateral internal auditory meatus were markedly narrowed. Audiometric examination demonstrated that only slight residual hearing remained in the low-frequency range and that a hearing aid would be of no benefit. Cochlear implantation was performed in her left ear. Because of the abnormal position of the facial nerve, the routine facial recess approach could not be performed. A canal-wall-down mastoidect

A major factor associated with recent improvements in the clinical performance of cochlear implant patients has been the development of speech-processing strategies based on high stimulation rates. While these processing strategies show clear clinical advantage, we know little of their long-term safety implications. The present study was designed to evaluate the physiological and histopathological effects of long-term intracochlear electrical stimulation using these high rates. Thirteen normal-hearing adult cats were bilaterally implanted with scala tympani electrode arrays and unilaterally st

BACKGROUND: A number of techniques are being investigated to accomplish bladder control recovery in paralyzed patients using the neurostimulation, but currently, all techniques are based on the dorsal implantation of the electrodes using a laminectomy.
METHODS: On 27 April 2006 we performed a laparoscopic implantation of a Finetech-Brindley bladder controller on the endopelvic sacral roots in a Th8 completely paralyzed woman who had previously undergone the removal of a Brindley controller due to an arachnoiditis after extrathecal implantation with intradural sacral deafferentation.

Radiotherapy of the head and neck can be associated with conductive and/or sensori-neural hearing loss. We report the case of a 67-year-old man who developed complete bilateral deafness caused by labyrinthitis and radiation-induced neuritis of the acoustic nerve after postoperative radiotherapy of a nasopharyngeal carcinoma. Two years postoperatively extensive clinical workup including computed tomography and magnetic resonance imaging showed no recurrence or secondary brain tumors. To facilitate sound perception a Combi 40 cochlear implant was implanted. Because of fibrosis the insertion dept

Transparent graphene-based neural electrode arrays provide unique opportunities for simultaneous investigation of electrophysiology, various neural imaging modalities, and optogenetics. Graphene electrodes have previously demonstrated greater broad-wavelength transmittance (∼90%) than other transparent materials such as indium tin oxide (∼80%) and ultrathin metals (∼60%). This protocol describes how to fabricate and implant a graphene-based microelectrocorticography (μECoG) electrode array and subsequently use this alongside electrophysiology, fluorescence microscopy, optical coherence tomogra

Intracortical microelectrodes have become a useful tool in neuroprosthetic applications in the clinic and to understand neurological disorders in basic neurosciences. Many of these brain-machine interface technology applications require successful long-term implantation with high stability and sensitivity. However, the intrinsic tissue reaction caused by implantation remains a major failure mechanism causing loss of recorded signal quality over time. Oligodendrocytes remain an underappreciated intervention target to improve chronic recording performance. These cells can accelerate action poten

PURPOSE OF REVIEW: To appraise and discuss the current available evidence on pulsed radio frequency (PRF), with an emphasis on published randomized controlled clinical trials.
RECENT FINDINGS: Pulsed radio frequency is a minimally invasive procedure used to treat a wide variety of chronic pain conditions. It is considered to be a safe and effective pain intervention with minimal side effects when performed by an experienced clinician with careful patient selection. Its mechanism of action is thought to be via neuromodulation, as 'pulses' of electric current are created at the electrode tip wi

In this work, we studied the combined effects of MR emissions, including gradient-induced electric fields and radiofrequency-induced heating, on vagus nerve activation in the presence of a cuff electrode. Our approach integrates electromagnetic and thermal simulations and neurophysiological modeling to assess these interactions. Preliminary results indicate that the radiofrequency-induced heating of the cuff electrode can further alter the activation threshold in the specific configuration investigated.Clinical Relevance- This study highlights the potential for radiofrequency-induced heating t

Recently the authors showed that a computational model of visual saliency could account for changes in gaze behavior of monkeys with damage in the primary visual cortex. Here we propose a neural prosthesis to restore eye gaze behavior by electrically stimulating the superior colliculus to drive visual attention. The saliency computational model is used to calculate the stimulation parameters from a real-time camera stream. Our simulations demonstrate that electrodes implanted in the superior colliculus at 1.0mm spacing are, in principle, able to recover specifically those visual attention beha

Deep brain stimulation (DBS) is a treatment using implanted electrodes to deliver electrical pulses to targeted areas of the brain. DBS holds great promise in psychiatry for the treatment of patients with treatment-resistant obsessive-compulsive disorder, depression and Gilles de la Tourette syndrome. Double-blind and open trials have shown that the treatment is effective in about half of these patients. Further research should focus on optimizing DBS with respect to target location for the various psychiatric disorders. More controlled double-blind trials are needed to corroborate the effecti

In the past decade deep brain stimulation (DBS)-the application of electrical stimulation to specific target structures via implanted depth electrodes-has become the standard treatment for medically refractory Parkinson's disease and essential tremor. These diseases are characterized by pathological synchronized neuronal activity in particular brain areas. We present an external trial DBS device capable of administering effectively desynchronizing stimulation techniques developed with methods from nonlinear dynamics and statistical physics according to a model-based approach. These techniques

This report describes the data of the Spanish Registry of Pacemakers, based on the European Pacemaker Identification Card, and the changes in the use of different stimulation modes since 1994. Of note is the significant and steady change towards more physiological modes of stimulation (which maintain atrioventricular synchrony). These modes accounted in 2003 for 69.9% among patients with sick sinus syndrome and 67.7% among patients with atrioventricular block. In the population less than 80 years of age these percentages were 78% for sick sinus syndrome and 78.7% for atrioventricular block. Th
