Comparison of Large Language Model Performance on the United Kingdom Neurology Specialty Certificate Examination.

Background Large language models (LLMs) are increasingly used in medical education; however, a gap remains in the literature comparing the performance of different LLMs on neurology-specific higher-specialty training assessments. This study aimed to compare the performance of three leading LLMs, including ChatGPT (OpenAI, San Francisco, CA, USA), Claude (Anthropic, San Francisco, CA, USA), and Gemini (Google LLC, Mountain View, CA, USA), on UK Neurology Specialty Certificate Examination (SCE) sample questions. Materials and methods This comparative cross-sectional performance evaluation used 8…

Utilization of a HIPAA-compliant large language model chatbot in an academic pediatric medical center.

Despite growing interest in LLM chatbots, little evidence exists on usage patterns in hospital systems. This paper presents a mixed-methods case study of "InternalGPT," a HIPAA-compliant LLM chatbot made available to employees of the institution. Survey responses and 14 months of system utilization data were analyzed across diverse professional roles in the institution. Of approximately 15,800 hospital employees, 2,149 (13.6%) requested InternalGPT access. Motivations for sign-up aligned with professional roles: operations professionals prioritized administrative automation, clinicians focused

Assessment of CDASI scoring by a multimodal large language model: a comparative study with expert assessors.

Large language models (LLMs) with multimodal capabilities may support automated assessment of cutaneous disease activity in dermatomyositis (DM). We evaluated the performance of Claude v3.5 Sonnet in scoring the Cutaneous Dermatomyositis Disease Area and Severity Index (CDASI) compared with expert rheumatologists. Thirty published DM cases with available clinical images were retrospectively analyzed. Two expert rheumatologists independently scored CDASI activity and damage domains. Claude v3.5 Sonnet assessed the same images using structured prompting based on CDASI definitions. Agreement was …

Development of a Framework for Evaluating Large Language Model Safety and Reliability: a Proof-of-Concept Evaluation.

Large language models (LLMs) are entering clinical decision support faster than methodology can characterise their safety. Aggregate accuracy treats all errors as interchangeable and cannot support safe deployment under Software as a Medical Device (SaMD) and EU AI Act frameworks. To develop and demonstrate a framework for evaluating large language model safety and reliability using catastrophic-failure frequency and response reproducibility, incorporating a pre-specified error taxonomy, difficulty-stratified analysis, and two-layer response consistency. The framework was applied to 54 diagnos…

Pilot evaluation of a large language model-mediated pre-consultation interview for vestibular disorders.

OBJECTIVE: Efficient and high-quality history taking is central to vestibular diagnosis, but it is often constrained by limited consultation time and variable patient understanding of questionnaires. Large language models (LLMs) can help structure interviews and support diagnosis by proposing differential diagnoses; however, evidence of their use in neurotology remains scarce. Here, we developed and evaluated an LLM-based system that automatically conducts pre-consultation interviews for dizziness symptoms. METHODS: We developed a prototype interview application with text input and speech out…

CeRTS: certainty retrieval token search in large language model clinical information extraction.

OBJECTIVE: Large language models (LLMs) must effectively communicate their uncertainty to be viable in clinical settings. As such, the need for reliable uncertainty estimation grows increasingly urgent with the expanding use of LLMs for information extraction from electronic health records. Previous token-level uncertainty estimators have only used token probabilities within a single output sequence. Here, by leveraging the constraints of JSON output structure, we instead consider all likely sequences and their respective probabilities to obtain a more robust measure of model confidence. We de…

Clinical Large Language Model Evaluation by Expert Review (CLEVER): Framework Development and Validation.

BACKGROUND: The proliferation of both general purpose and health care-specific large language models (LLMs) has intensified the challenge of effectively evaluating and comparing them. Data contamination plagues the validity of public benchmarks, self-preference distorts LLM-as-a-judge approaches, and there is a gap between the tasks used to test models and those used in clinical practice. OBJECTIVE: In response, we propose CLEVER (Clinical Large Language Model Evaluation-Expert Review), a methodology for blind, randomized, preference-based evaluation by practicing medical doctors on specific …

Automatic Minds: Cognitive Parallels Between Hypnotic States and Large Language Model Processing.

The cognitive processes of the hypnotized mind and the computational operations of large language models (LLMs) share deep functional parallels. Both systems generate sophisticated, contextually appropriate behavior through automatic pattern-completion mechanisms operating with limited or unreliable executive oversight. This review examines this convergence across three principles: automaticity, in which responses emerge from associative rather than deliberative processes; suppressed monitoring, leading to errors such as confabulation in hypnosis and hallucination in LLMs; and heightened conte…

On the development and validation of large language model-based classifiers for identifying social determinants of health.

The assessment of social determinants of health (SDoH) within healthcare systems is crucial for comprehensive patient care and addressing health disparities. Current challenges arise from the limited inclusion of structured SDoH information within electronic health record (EHR) systems, often due to the lack of standardized diagnosis codes. This study delves into the transformative potential of large language models (LLM) to overcome these challenges. LLM-based classifiers-using Bidirectional Encoder Representations from Transformers (BERT) and A Robustly Optimized BERT Pretraining Approach (R…
