Periostin differentially induces proliferation, contraction and apoptosis of primary Dupuytren's disease and adjacent palmar fascia cells.

Dupuytren's disease, (DD), is a fibroproliferative condition of the palmar fascia in the hand, typically resulting in permanent contracture of one or more fingers. This fibromatosis is similar to scarring and other fibroses in displaying excess collagen secretion and contractile myofibroblast differentiation. In this report we expand on previous data demonstrating that POSTN mRNA, which encodes the extra-cellular matrix protein periostin, is up-regulated in Dupuytren's disease cord tissue relative to phenotypically normal palmar fascia. We demonstrate that the protein product of POSTN, periost

Surgical management of the hand in Apert syndrome.

In patients with Apert syndrome, the hands demonstrate many disturbances of soft tissue and bony structures. These include a short thumb with radial clinodactyly, complex syndactyly with a bony fusion involving the index, long and ring fingers, symphalangism and simple syndactyly of the fourth web space. The soft tissue anomalies involve the intrinsic muscles, the extrinsic tendon insertions and the neurovascular bundles. We have reviewed 52 patients who underwent surgical reconstruction of their hands. The aim of this study is to propose a better surgical management in the light of recent pub

Synostosis of the ring-small finger metacarpal in Apert acrosyndactyly hands: incidence and treatment.

This study reports on the incidence of a synostosis between the ring-small metacarpal bases in patients with Apert syndrome and describes a technique to resect the synostosis and insert silicone sheets to improve hand function. Records of 9 patients (18 hands) were evaluated. Average age at follow-up was 9.5 years. Three relationships between the ring-small metacarpals were observed. Type I hands (22%) had no abnormal interconnections; type II hands (33%) possessed a synostosis since birth; type III hands (44%) did not possess a synostosis at birth, but one later developed at average age of 79

[Fractures near the base of the first metacarpal bone--clinical outcome of 21 patients].

Fractures of the base of the first metacarpal are particularly common. The present examination was carried out in order to find out a correlation between the clinical outcome and the type of the fracture, the quality of reduction, the surgical procedure, and the extent of osteoarthrosis. From February 1992 to August 1997, 24 patients with Bennett fracture-dislocation, Rolando fracture und extraarticular fracture--eight patients in each group--were treated operatively. After a median interval of 33 months, 21 patients were reviewed. The evaluation was done using a new score with subjective (pai

Prediction of fracture risk by radiographic absorptiometry and quantitative ultrasound: a prospective study.

Recent developments in computer-assisted radiographic absorptiometry (RA) and quantitative ultrasound techniques (QUS) provide readily accessible and relatively inexpensive methods for assessing bone mineral status. However, few population-based studies have investigated the ability of RA and ultrasound to predict fracture risk prospectively. We explored the ability of RA and QUS to predict fracture risk among 560 postmenopausal women from the Hawaii Osteoporosis Study; average follow-up was 2.7 years. An incident vertebral fracture was defined as a decrease of more than 15% in vertebral heigh

[Late functional outcome of surgically treated mid-hand fractures].

The aim of surgical intervention for the treatment of metacarpal fractures is to restore the best possible function. This study of 61 patients who were treated utilizing different osteosynthesis techniques demonstrates out-standing long-term results. Conservative treatment may be adaeqaute for the treatment of uncomplicated and minimally dislocated fractures. For serial fractures and fractures involving joints, however, there is no alternative to osteo-synthesis.

Screw or mini-plate fixation in fractures of the first metacarpal. Experience with thirty-nine cases.

Re-establishing normal osteoarticular anatomy, and early mobilization are the principal elements in the successful treatment of fractures of the base of the first metacarpal. To achieve these goals in thirty-nine fractures rigid fixation was obtained by screw alone, or by a mini-plate. Seven Bennett's fractures have been treated by screw fixation when the size of the fragment has made it possible. Extra-articular fractures have been treated, either by screw alone, in oblique fractures (7 cases). Or by mini-plate, T or L shaped, in transverse fractures (23 cases). The use of these techniques ha

Further evidence for specific IFIH1 mutation as a cause of Singleton-Merten syndrome with phenotypic heterogeneity.

Singleton-Merten syndrome (MIM 182250) is an autosomal dominant inherited disorder characterized by early onset periodontitis, root resorption, osteopenia, osteoporosis, and aortic valve or thoracic aorta calcification. The disorder can have significant intrafamilial phenotypic variability. Here, we present a mother and daughter with Singleton-Merten syndrome harboring a previously described pathogenic missense mutation, c.2465G>A p.(Arg822Gln), in IFIH1 (interferon induced with helicase C domain 1), encoding MDA5 (Melanoma Differentiation-Associated protein 5). These data confirm the pathogen

Quality of life-oriented evaluation of late functional results of hand replantation.

BACKGROUND: Forearm amputation in different zones via a similar mechanism in a group of patients of similar age is associated with different possibilities of functional recovery. The degree of postoperative recovery of function is invariably partial compared to pre-amputation function; this kind of trauma inevitably leads to disability. The patients adapt to their new circumstances to a different extent. That is why a quality of life evaluation should be included in the assessment of replantation outcomes. The aim of the study was to evaluate functional outcomes in patients after replantation

Sensorineural deafness, abnormal genitalia, synostosis of metacarpals and metatarsals 4 and 5, and mental retardation: description of a second patient and exclusion of HOXD13.

In 1988 Pfeiffer and Kapferer reported on a patient with sensorineural deafness, psychomotor delay, hypospadias, cerebral manifestations, and bilateral synostosis of the 4th and 5th metacarpals and metatarsals. Synostosis of the 4th and 5th metacarpals and metatarsals is a very rare defect that has been described as an isolated Mendelian defect, as part of multiple congenital anomaly (MCA) patterns, and in different syndromes. Among a total of 2,023,155 liveborn infants in the Spanish Collaborative Study of Congenital Malformations (ECEMC), we observed only two cases with this type of metacarp

Metacarpophalangeal analysis in Crouzon syndrome: additional evidence for phenotypic convergence with the acrocephalosyndactyly syndromes.

Crouzon syndrome (CS) is characterized by premature craniosynostosis, orbital proptosis, and midfacial hypoplasia and is related to the acrocephalosyndactylies (ACS) with limb abnormalities. In CS the hands are considered to be normal, but a previous report indicated that there is consistent alteration in the proportions of the bones of the hands, as shown by metacarpophalangeal analysis (MCPP), in at least some cases of CS. The purpose of this study was to extend these findings by examining affected individuals and their supposedly unaffected first degree relatives. If CS does have an effect

Palatal dimensions and some inherited factors (body height and metacarpal index).

The relationship between the palatal dimensions and inherited factors such as body height and arachnodactyly, i.e., spider fingers, were studied in a sample of 87 patients with high and narrow palatal vaults, chosen from 1008 randomly selected patients at an orthodontic clinic. The patients with high palates were significantly taller than Swedes of the same sex and age (p < 0.001). Established arachnodactyly was not found in the examined sample. Sucking habits neither seem to influence crowding nor the palatal dimension at the molar region. Mouthbreathing strengthened but was not a pre-requisi
