10-year survival of total ankle arthroplasties: a report on 780 cases from the Swedish Ankle Register.

BACKGROUND AND PURPOSE: There is an ongoing need to review large series of total ankle replacements (TARs) for monitoring of changes in practice and their outcome. 4 national registries, including the Swedish Ankle Register, have previously reported their 5-year results. We now present an extended series with a longer follow-up, and with a 10-year survival analysis. PATIENTS AND METHODS: Records of uncemented 3-component TARs were retrospectively reviewed, determining risk factors such as age, sex, and diagnosis. Prosthetic survival rates were calculated with exchange or removal of components…

Soft tissue problems in ankle fractures treated surgically. A prospective study of 154 consecutive closed ankle fractures.

We performed a prospective registration of primary soft tissue injuries and perioperative soft tissue complications the first 3 months after surgery in ankle fractures treated by open reduction and internal fixation. Open fractures and polytraumatized patients were excluded. The 154 consecutive patients (90 women) with an average age of 54.5 (S.D. 18.3) years were registered. Primary soft tissue injuries according to Tscherne's classification were noted in 22 patients (14.2%). Major perioperative soft tissue complications requiring revision occurred in five patients (3.2%). Minor perioperative…

Early ankle mobilization, Part I: The immediate effect on acute, lateral ankle sprains (a randomized clinical trial).

A random clinical trial was performed to determine the immediate effects of early ankle mobilization on active duty military members with moderate and severe lateral ankle sprains. Sixty-four subjects were randomly allocated to one of three treatments: a plaster cast, an air-stirrup (Aircast Inc., Summit, New Jersey), or an elastic wrap. Each intervention lasted 2 weeks followed by 3 weeks of rehabilitation. Subjects who were allowed early mobilization returned to work and running significantly sooner than those placed in plaster casts (p = 0.029). Swelling (p = 0.009) and pain (p < 0.001) dec…

Sensor Design for Ankle Resiliency Training.

INTRODUCTION: Lateral ankle sprains (LASs) are among the most common musculoskeletal injuries and are a leading cause of limited duty days among service members. Rehabilitation and prevention strategies focus on strengthening the peroneus longus (PL) muscle as a protection mechanism, however clinical assessments of PL engagement during training are qualitative and require expert evaluation. Existing surface electromyography (sEMG) technologies allow noninvasive estimations of muscle activation, but are limited when energy from neighboring muscles (crosstalk) reaches detection sites at anatomic…

Principles of Revision Total Ankle Arthroplasty.

Total ankle arthroplasty (TAA) has become a recognized solution for patients suffering from end-stage ankle arthritis. Despite advancements in implant design and surgical techniques over the past decade, the long-term survivorship of these implants remains ill-defined. A notable deficiency exists in published data regarding failure rates and the underlying causes of implant failures. As indications for TAA expand-with more complex deformities, the incidence of revision surgeries is expected to rise. Accordingly, surgeons need to be familiar with the principles of total joint revision to succes…

Current Concepts in Ankle Fracture Management.

Ankle fractures are one of the more common musculoskeletal injuries that are treated by foot and ankle specialists. A thorough understanding of managing these injuries requires the ability to differentiate between stable and unstable fractures. The current literature supports the nonoperative management of stable Weber B ankle fractures, whereas unstable fractures have much better outcomes with surgical intervention. Specifically, we review the fixation strategies for the lateral, medial, and posterior malleolar fractures respectively. Finally, we discuss the current trends in postoperative ma…

Current Trends in Total Ankle Replacement.

Ankle arthritis can result in significant pain and restriction in range of motion. Total ankle replacement (TAR) is a motion-preserving surgical option used as an alternative to total ankle arthrodesis to treat end-stage ankle arthritis. There are several generations of TAR techniques based on component design, implant material, and surgical technique. With more recent TAR implants, an attempt is made to minimize bone resection and mirror the native anatomy. There are more than 20 implant devices currently available. Implant survivorship varies among prosthesis types and generations, with impr…

Total Ankle Arthroplasty in Young Patients.

With continuing advancements in total ankle arthroplasty (TAA), it is quickly becoming the procedure of choice for older patients with end-stage ankle arthritis. Multiple studies have been conducted on younger patients who have undergone TAA with promising results, but is it the procedure of choice? Considerations of TAA versus ankle arthrodesis, TAA implant longevity, outcomes of revision TAA, and whether patients should be offered an arthrodesis with plans for conversion to arthroplasty may help elucidate whether pursuing ankle arthroplasty in a younger, more active population is the correct…

Arthroscopic Treatment of Anterior Ankle Impingement.

Arthroscopic reduction of tibiotalar osteophytes results in good to excellent results in the vast majority of patients. Pain is primarily due to synovial hypertrophy and anterior tibiotalar entrapment associated with the osteophytes. Osteophytes may be due to repetitive trauma such as sports, or associated with subtle or overt ankle instability. A minimally invasive approach results in rapid recovery and less risk than open interventions. In cases where anterior osteophytes have coexisting ankle instability and in many cases ancillary procedures such as ankle stabilization are performed.

Unstable Ankle Fractures in Neuropathic Diabetics.

Connolly recognized as early as 1998 that a displaced ankle fracture in a neuropathic diabetic puts that patient at risk for an amputation. It is well appreciated that the risk of a poor clinical outcome secondary to failure of the surgical construct, deep wound infection and osteomyelitis or a combination of both, is greatly increased in neuropathic diabetic patients, that is, those that are insensate to the Semmes-Weinstein 5.07 (10 g) monofilament, as compared with sensate diabetics or similar non-diabetic patients. Despite this understanding, there is little objective evidence to guide tre…

Current Techniques in Total Ankle Arthroplasty.

Total ankle replacement (TAR) continues to increase in popularity as a motion-preserving option to ankle arthrodesis. TAR is indicated for primary, posttraumatic and inflammatory arthropathies as an alternative procedure to tibiotalar arthrodesis. Proper patient selection is paramount to a successful outcome in TAR. Contraindications to TAR include the presence of neuropathy, active infection, severe peripheral arterial disease, inadequate bone stock, and severe uncorrectable coronal plane deformity. This article is a brief overview of techniques and PEARLS on how to address a well-aligned ank…

ANKLE DISTRACTION ARTHROPLASTY: A SYSTEMATIC REVIEW.

Epidemiological data suggest 9-15% of ankle joint osteoarthritis (AOA) in the general population. One of the methods of delaying radical intervention is ankle joint distraction arthroplasty of the ankle joint (ADA), including a combination of various techniques. The lack of publications summarizing the maximum possible clinical data on ADA for more than 50 years of the method's history justifies the need for a review. A systematic review of ankle distraction arthroplasty followed the 2020 PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-analyses) protocol guidelines. The inclu…

Sport activity levels following ankle fusion.

PURPOSE: Ankle arthrodesis (AA) is frequently employed in the treatment of end-stage ankle arthritis, which is common following trauma and athletic injuries. While AA remains a popular therapeutic option, little data exists about activity and sporting capacity following AA. The objective of this research was to determine functional outcomes and sporting activity levels in patients following Ankle Arthrodesis. METHODS: Validated questionnaires were emailed to 35 patients with a history of AA at an average follow-up of 52 months. Functional outcomes were assessed using the Foot and Ankle Disabi…

Patient Outcomes Following Ankle Fracture Fixation.

BACKGROUND: Several studies have reported on potential negative predictive factors of functional outcomes after ankle fracture fixation. However, there is minimal patient-reported data on long-term outcomes. This study aimed to evaluate potential risk factors leading to a poor patient-reported functional outcome at 2 and 5 years following ankle fracture fixation. METHODS: We conducted a prospective cohort study over a 5-year period on patients undergoing open reduction and internal fixation for unstable ankle fractures. Patient demographics, medical comorbidities, fracture pattern, and fixati…

[Controversial issues of chronic ankle instability].

Chronic ankle instability is a chronic sports injury disease characterized by recurrent sprains, feeling of instability, pain, swelling and other symptoms after an initial sprain. Research on the pathological mechanism of chronic ankle instability still cannot explain the diversity of symptoms. In the past, the surgical indications for chronic ankle instability only emphasized mechanical joint laxity. However, most patients with chronic symptoms after a lateral ankle sprain may not have a typical complaint of mechanical instability. More patients presented with persistent pain or functional in…

[Ligamentous ankle injury: an underestimated trauma?].

Ligamentous injuries of the ankle joint are among the most common injuries in sports, with landing on the opponent's foot or direct contact counting among the most common injury mechanisms. Initial measures most notably include a clinical examination, supplemented by X-rays, sonography or MRI as required. In most cases, conservative treatment is the treatment of choice. At the beginning, pain and swelling have to be reduced. Then function must be regained before patients may return to sports after a return-to-play test. Some injury patterns are more complex with concomitant injuries, and despi…

[Os subtibiale: That odd ankle pain].

Os subtibiale is a low prevalence accessory bone of the ankle. This bone is located in the posterior colliculus of the tibial medial malleolus, both in paediatric and adult ages. It can cause pain, redness and/or swelling, which can lead to a mistaken diagnosis of avulsion fracture. Adequate anatomical knowledge is crucial. First, we present the case of a school-aged boy, seen at the outpatient clinic for a 2-month history of pain in both inner ankles after an injury. Second, we present the case of an adult patient with a 3-day history of right medial ankle pain, with no previous injury, evalu…

Primary Arthrodesis for Diabetic Ankle Fractures.

BACKGROUND: Treatment of ankle fractures in patients with diabetes is associated with increased complication rates. Ankle arthrodesis is considered a salvage procedure after failed ankle fracture fixation, yet primary ankle arthrodesis has been proposed as a treatment option for patients with significant diabetes-related complications. To date, the characteristics of patients who undergo primary ankle arthrodesis and the associated outcomes have not been described. METHODS: A retrospective review was performed of 13 patients with diabetes who underwent primary arthrodesis for traumatic ankle …

Chronic Lateral Ankle Instability: Surgical Management.

Surgical management for chronic lateral ankle ligament instability is useful when patients have failed nonoperative modalities. Open anatomic reconstruction is an effective method of stabilization. Ankle arthroscopy is a recommended to address intra-articular disorder before stabilization. An anatomic approach provides full range of motion, stability, and return to sport and activity. Allograft or suture tape augmentation can be useful for patients with generalized ligamentous laxity, patients with high body mass index, and elite athletes. Allograft reconstruction may be especially useful in r…
