A Systematic Review of Screw and Suture Button Glenoid Augmentation Constructs.

BACKGROUND: Glenohumeral dislocations often lead to glenoid bone loss and recurrent instability, warranting bony augmentation. While numerous biomechanical studies have investigated fixation methods to secure a graft to the glenoid, a review of available constructs has yet to be performed. PURPOSE: To synthesize the literature and compare the biomechanics of screw and suture button constructs for anterior glenoid bony augmentation. STUDY DESIGN: Systematic review. METHODS: A systematic review was conducted in accordance with the PRISMA (Preferred Reporting Items for Systematic Reviews and M

Lag screw fixation of mandibular angle fractures.

This article presents a technique of applying lag screws for treating fractures of the mandibular angle. A review of 30 patients who had lag screws placed to treat such fractures showed that it is an extremely useful, but technique-sensitive, method of providing rigid internal fixation. The advantages and complications of this technique over bone-plate fixation are discussed.

Study of fixation of a mandibular plate for favourable fractures of the mandibular angle: numerical predictions.

Fractures of the mandibular angle have been well-described and, in most societies, their incidence is decreasing. In this study we analysed the stabilisation of fractures using a single plate (standard or optimised model). The finite element model was developed based on a mandibular computed tomographic scan, together with a miniplate from DePuy Synthes and an optimised plate. Using the finite element model we looked in turn at the four screws for fixation of the standard plate, and the six screws for the optimised plate, in a complete and an incomplete favourable fracture of the mandibular an

Mechanical Analysis of Ultrasound-Activated Pins and Resorbable Screws: Two Different Techniques to Fixate Osteosynthesis in Craniosynostosis Surgery.

PURPOSE: Ultrasound activation of resorbable pins directly into drilled holes of the calvarium was introduced to overcome the time-consuming installation in the resorbable osteosynthesis fixation in craniosynostosis surgery. There is paucity in the data comparing the mechanical properties of resorbable screws and ultrasound-activated pins produced by different manufacturers. The aim of this experimental study was to compare the mechanical properties of ultrasound-activated pins and resorbable screws. METHODS: A mechanical testing machine was used to characterize the mechanical performance of

Pedicle screw fixation with kyphoplasty decreases the fracture risk of the treated and adjacent non-treated vertebral bodies: a finite element analysis.

Adjacent vertebral fractures are common in patients with osteoporotic vertebral compression fractures (OVCFs) after kyphoplasty. This finite element study was to examine whether short segment pedicle screw fixation (PSF) with kyphoplasty may decrease the fracture risk of the treated and adjacent non-treated vertebrae after kyphoplasty for OVCFs. By simulating cement augmentation with or without short segment pedicle screw fixation (PSF), two tridimensional, anatomically detailed finite element models of the T10-L2 functional spinal junction were developed. The insertion of pedicle screws into

A Comparison of Torque Forces Used to Apply Intermaxillary Fixation Screws.

PURPOSE: When establishing intermaxillary fixation (IMF) using bone screws, fracture of a screw is a potential complication. This study was conducted to investigate the forces that arise at bone screw insertion and to determine safety margins between torque for manually tightened insertion and torque until breakage for 3 different IMF screw systems, which could ultimately favor the use of 1 IMF screw system based on decreased risk of complications. MATERIALS AND METHODS: IMF screws were placed into porcine mandibles by 3 oral and maxillofacial surgeons. The porcine mandibles were evaluated fo…

[Biomechanical studies of the role of the interfragmentary traction screw in plate osteosynthesis exemplified by a short oblique tibial shaft fracture].

In oblique fractures of the shaft of the tibia, plate osteosynthesis is combined with an interfragmentary traction screw to enhance the stability. Depending on the position of the fracture, the screw is introduced separately or via a plate hole near to the fracture. The maximum resistance to bending and the plastic deformation of plate osteosyntheses were studied in the tibiae of human corpses, with or without traction screw; in some cases, pure traction screw osteosyntheses were used in this study. It was found that the additional traction screw enhanced the stability by 52%, whereas plastic

Influence of hooks and a lag screw on internal fixation plates for lateral malleolar fracture: a biomechanical and ergonomic study.

BACKGROUND: For internal fixation of AO classification Type B lateral malleolar fracture, insertion of lag screws into the fracture plane and fixation with a one-third tubular plate as a neutralization plate are the standard treatment procedures. The one-third tubular plate is processed to a hook shape and hung on the distal end of the fibula. In this study, to compare the function of the hook and lag screws of a one-third tubular plate and LCP for osteosynthesis of lateral malleolar fracture, mechanical indices of internal fixation were compared among the one-third tubular plates with lag scr

Implants and sinus-inlay graft in a 1-stage procedure in severely atrophied maxillae: prosthodontic aspects in a 3-year follow-up study.

The aim of this 3-year prospective study was to evaluate the prosthetic treatment in 2 groups of maxillary edentulous patients with similar age and gender distribution: a study group of 39 patients treated with intra-sinus block bone grafts and implants in a 1-stage procedure, and a control (reference) group of 37 patients treated with implants and no grafting. In the study group, bone volumes were regarded as insufficient for implant treatment unless a bone grafting procedure was performed (posterior alveolar bone height was less than 5 mm). Self-tapping Nobel Biocare implants were used in bo

The use of acetabular screws in total hip arthroplasty and its influence on wear and periacetabular osteolysis in the long-term follow-up.

PURPOSE: The cementless implantation of hip replacement cups may be performed with and without the additional use of acetabular screws. If the surgeon uses screws or not depends on variable factors. In general, the use of screws is intended to increase the primary stability of the cup. Whether screws increase the initial stability of the cup construct, or even reduce it in part, is the subject of considerable debate in the literature. It is also unclear whether the additional screws lead to increased wear or increased periacetabular osteolysis over the long-term course. METHODS: Two hundred e

A comparison of the biomechanical properties of three different lumbar internal fixation methods in the treatment of lumbosacral spinal tuberculosis: finite element analysis.

There are various internal fixation methods in treating lumbosacral spinal tuberculosis. The study compared the stability and stress distribution in surrounding tissues/implants, such as discs, endplates and screw-rod internal fixation system, etc. when applying three different lumbar internal fixation methods to treat lumbosacral spinal tuberculosis. A finite element model was constructed and validated. The spinal stability was restored using three methods: a titanium cage with lateral double screw-rod fixation (group 1), autologous bone with posterior double screw-rod fixation (group 2), and

Interfragmentary compression forces of scaphoid screws in a sawbone cylinder model.

Various screws have been developed to stabilise fractures of the scaphoid. Commonly used are the Herbert, the HBS, the 3-mm AO and the Acutrak screws. Not long ago a new screw, the Twin Fix, was introduced. This is cannulated and similar in shape and appearance to the classical Herbert screw. In our test series we compared the maximum achievable compression forces of the Twin Fix screw with that of three other screws (AO, HBS and Acutrak screws). To avoid the variations of density, stiffness and rigidity in natural bone, a polyurethane sawbone-based test setup was used. The test series include

Incarcerated Cannulated Cancellous Screw Removal during Total Hip Arthroplasty with a Novel Trick: A Case Report.

Salvage total hip arthroplasty (THA) may be required to manage femoral neck fracture implant failures, avascular necrosis and secondary hip arthritis. Cannulated cancellous screws can become stripped or incarcerated during the initial implantation and pose hardware removal issues. We present a novel technique for safe screw removal in a 62-year-old female patient with a painful right hip. She had undergone cancellous screw fixation for a fracture of the neck of femur ten years ago. There was avascular necrosis with screw cut out leading to secondary hip arthritis necessitating THA. Comparative evaluation of a patient-specific customised plate designs and screws for partial mandibular reconstruction.

Mandibles with odontogenic tumors are often partially reconstructed with a metallic bone graft analogue with dental roots, crowns, along with a customized plate fixed with monocortical or bicortical screws, following resection of the tumor. In this study, two different designs of patient specific customized Ti reconstruction plates, solid and plate with holes, were considered. Fixation through both bicortical and monocortical screw types were investigated. FE models of the reconstructed mandibles were developed to analyse the influence of the plate-screw type combination on the load transfer a

Prospective Evaluation of Magnetic Resonance Imaging Features of Magnesium-Based Alloy Screw Resorption in Pediatric Fractures.

BACKGROUND: The resorption of magnesium-based alloy bioabsorbable screws results in the release of hydrogen gas, which can mimic infection and enter the growth plate. The screw itself and the released gas may also affect image quality. OBJECTIVE: The evaluation of magnetic resonance imaging (MRI) findings during the most active phase of screw resorption is the objective, with particular focus on the growth plate and to assess for the presence of metal-induced artifacts. MATERIAL AND METHODS: In total, 30 prospectively acquired MRIs from 17 pediatric patients with fractures treated with magne

Finite element method-based simulation on bone fracture fixation configuration factors for biodegradable embossed locking compression plate.

As an evolution, biodegradable implants need to maximize mechanical performance thereby may lead to confusion in selection of the biodegradable material and implant design to the fracture site. This requires selecting a unique fixation configuration to fit within the fractured bone, factors of which can be bone-plate clearance, interfragmentary gap, alteration in screw fixation position and variation in the number of screws whose configuration optimization can re-maximize the mechanical performance of the biodegradable implant. Therefore, these factors have been optimized based on the induced
