Ultra-Low Contrast Percutaneous Coronary Angiography and Intervention: How To Do It.

Excessive contrast volume continues to be one of the major modifiable risk factors for contrast-induced nephropathy in coronary angiography and percutaneous coronary intervention. Consequently, strategies aimed at minimising contrast exposure are increasingly relevant in contemporary interventional practice. This article describes a practical, step-by-step approach to contrast minimisation by outlining reproducible techniques to assist interventional cardiologists in safely reducing contrast volume without compromising procedural efficacy or clinical outcomes.

When Heart Team Really Counts: Radical Excision of Intracardiac Leiomyomatosis.

Intravenous leiomyomatosis (IVL) are rare and complex tumors, characterized by high rates of recurrences after surgical removal and the capability of multi-organ involvement including pulmonary embolization. Regarding the surgical treatment of Intracardiac Leiomiomatosis (ICL), only few articles have been published and no controlled data are available. A combined approach that involves a Team of Cardiologists, Heart Surgeons, Vascular surgeons and Radiologists seems to be successful in treating ICL.

Valvular Heart Disease in Pregnancy.

The number of reproductive age women with valvular heart disease is rising and accounts for one third of all heart disease among pregnant women. Severe, symptomatic left-sided cardiac lesions, particularly mitral and aortic stenosis, and mechanical heart valves, are associated with adverse maternal and fetal outcomes. Decreasing maternal and fetal risk requires shared decision-making among patients and the heart team, consisting of obstetricians, maternal-fetal medicine subspecialists, and cardiologists.

[European Society of Cardiology guidelines for the management of complex grown-up congenital heart disease].

The number of patients with Grown-Up Congenital Heart disease (GUCH) consulting adult cardiologists is steadily increasing. These patients have either a non-diagnosed congenital heart disease revealed at adulthood, or a diagnosed congenital heart disease for which one or multiple interventions have possibly been performed during childhood. In this article, we summarize the recommendations of the European Society of Cardiology of 2010 for complex congenital heart disease.

Cardio-obstetrics: Recognizing and managing cardiovascular complications of pregnancy.

Pregnancy can exacerbate known cardiovascular disorders and unmask previously unrecognized problems. Patients with congenital heart disorders, valvular disease, primary pulmonary hypertension, hypertensive disorders of pregnancy, and acquired peripartum cardiomyopathy need a collaborative interdisciplinary team that includes a cardiologist with specialty training in obstetrics.

Matt's story: learning from heartbreak.

The victims of medical error reach far beyond the patient. The aftermath forever changes the lives of the patient's family and physician alike. We explore the life and death of nineteen-year-old Matt, a stellar athlete and better son, and the cognitive bias that led to an unfortunate and consequential medical misdiagnosis. This story is one of family heartbreak, the harsh reality of second victim phenomenon, and ultimately lessons learned in compassion, vigilance, and candidness for the health care industry. We share this tragedy with the medical community in hopes that we improve the use of p

Echocardiography in low-resource rural tropical hospitals.

Cardiopathies are common in tropical regions and echocardiography could be of substantial value. However, it is difficult to practice. We suggest simple techniques with conventional instruments that are available in many rural hospitals, and describe the most common echocardiographic procedures and findings that can be undertaken with short training by non-cardiologist physicians.

Echocardiographic evaluation of tetralogy of Fallot.

Tetralogy of Fallot (TOF) is a cyanotic heart disease consisting of nonrestrictive ventricular septal defect, overriding aorta, pulmonary stenosis, and right ventricular hypertrophy. Early total correction is the treatment of choice and these patients with repaired TOF are increasingly seen in adult practice. This review addresses echocardiographic evaluation of TOF, corrected TOF, its sequelae and various complications. A working knowledge of TOF assessment is essential for all adult cardiologists and sonographers.

Strategies to manage the heart failure patient after transplantation.

Cardiac transplantation has emerged as an effective form of therapy for end-stage congestive heart failure (CHF), but patients are predisposed to both dilated and restrictive congestive physiology. The etiologies of post-transplant CHF are unique and complicated, most certainly reflecting a chronic rejection process that has yet to be elucidated. Successful identification of underlying pathophysiologic mechanisms and subsequent therapeutic approaches will require close coordination among cardiologists, cardiac surgeons, and immunobiologists.

[Subphrenic abscess and suppurative pericarditis as complications in children with perforated appendicitis].

The combination of subphrenic abscess with purulent pericarditis is a rare postoperative complication of perforated appendicitis in children, with severe clinical course, difficult to diagnose and high case fatality rate. A 7-year-old child with this complication, successfully diagnosed and treated is reported. The importance of complex therapy--surgical and intensive and collaboration between pediatric surgeons, anesthesiologists, cardiac surgeons and cardiologists for the favourable outcome is emphasized.

Current perspectives on the use of intracoronary imaging in interventional practice in the Asia-Pacific.

The perceptions of the Asia-Pacific interventional community towards intracoronary imaging (ICI) has not been systematically evaluated. This study aimed to evaluate adoption rates, operator perceptions, and attitudes towards ICI amongst the cardiology community in the Asia-Pacific. A web-based survey was conducted from April to September 2025 among cardiologists performing percutaneous coronary intervention from the Asia-Pacific region. Demographic characteristics as well as the availability, frequency of use, and perceptions of ICI were assessed. Among 672 survey responses, 71.4% of responden…

Interdepartmental cooperation in access-site management - verifiable persistent benefit for patients.

Background: Vascular access-site complications after catheterization remain a significant cause of morbidity and mortality. While patient- and procedure-related risk factors are well described, the impact of structured, interdisciplinary training programs is less well established. We evaluated the effect of a structured training program for cardiologists and interventional staff on access-site-related complications after transfemoral coronary interventions. Patients and methods: Over 36 months, we tracked all elective transfemoral coronary interventions at our institution. After an initial 6-m…

Z-score evaluation and inferences for management of aortopathy patients.

Echocardiographic Z-score models play a crucial role in defining cardiac pathology in paediatric patients. There are multiple models that practitioners utilize in the United States without guiding principles to standardize their use. Discrepant interpretations can occur depending on the model chosen, even if standardized Z-score cutoffs are applied. In this study, a survey was developed to assess pediatric cardiologists' use of Z-score systems when evaluating and treating patients with isolated bicuspid aortic valve. The majority of respondents reported using Z-score cutoffs to evaluate the de…

Thoracic Outlet Syndrome: A Comprehensive Review.

Thoracic outlet syndrome (TOS) comprises a range of conditions characterized by compression of the brachial plexus, subclavian artery, or subclavian vein as these structures traverse the thoracic outlet. Although TOS was first documented in 1860 by Willshire, it remains a diagnostic and therapeutic challenge-particularly for cardiologists evaluating upper-extremity ischemia, suspected arm emboli, or unexplained swelling. This article provides a cardiology-focused overview of TOS, emphasizing the condition's subtypes (neurogenic, venous, and arterial), key diagnostic approaches, comparative sur…

Aortic valve replacement in non-elderly: the gap between reality, guidelines and evidence.

OBJECTIVES: There are several treatment options for non-elderly adults (18-60 years) in need of aortic valve replacement (AVR): a mechanical or biological substitute, a homograft, the Ross procedure or nowadays even transcatheter aortic valve replacement. This study evaluated the current opinions and practices of cardiac surgeons and cardiologists concerning AVR strategies in this patient group. METHODS: A 43-item online survey was sent out to several scientific organizations, individual cardiac surgeons, cardiologists and residents/fellows worldwide. RESULTS: Two hundred twenty-two physicia…

A Case of Primary Aldosteronism Due to A Primary Adrenal Adenoma Diagnosed by Segmental Adrenal Venous Sampling (S-AVS) Using a Modified Catheter System and Lateral Cine Angiography.

BACKGROUND Before partial adrenalectomy for primary aldosteronism due to a primary adrenal adenoma, the aldosterone-producing tumor can be localized by segmental adrenal vein sampling (S-AVS). Cardiologists, who regularly perform percutaneous coronary intervention (PCI), or coronary angioplasty with stent, may not be familiar with the technique of S-AVS. A case of the use of S-AVS is reported in a patient who presented with primary aldosteronism and a right adrenal adenoma. CASE REPORT A 44-year-old man with a history of hypertension presented with a man in the posterior part of the right adre…
