Biology of corneal fibrosis: soluble mediators, integrins, and extracellular vesicles.

Corneal fibrosis develops in response to injury, infection, postsurgical complications, or underlying systemic disease that disrupts the hom … d in corneal fibrosis in the context of different injury models-epithelial debridement, superficial keratectomy, and penetrating incision. We elaborate on the interplay of the major pro-fibrotic factors involved in corneal scar development (e.g., transforming growth factor-β1, thrombospondin-1, and ανβ6), and explore a novel role for extracellular vesicles se …

Corneal hemangiosarcoma in a cat.

A 10 year-old castrated male Domestic Short-hair cat with a history of chronic bilateral keratitis was referred for assessment of a red, elevated mass involving the left cornea. The rapid growth of the mass, over a month period in combination with pronounced vascularization and invasion of the corneal surface suggested an aggressive inflammatory or neoplastic process. Following keratectomy, the lesion was diagnosed histopathologically as a hemangiosarcoma. The tumor recurred locally within 3 weeks and enucleation was performed. Histopathologic examination of the globe confirmed the diagnosis a…

Superficial stromal keratitis in the dog.

Superficial stromal keratitis or pannus is a syndrome of corneal, conjunctival and third eyelid inflammation. Superficial stromal keratitis … titis can be controlled with topical corticosteroids, and only rarely is cryosurgery or superficial keratectomy required to remove excessive pigment and or granulation tissue. The precise aetiology of SSK is unknown, but is likely to be multifactorial, with sunlight being a significant factor. Corneal lipidosis and keratoconjunctivitis sicca can occur seconda …

Keratoepithelioplasty.

Keratoepithelioplasty is a new surgical procedure for the treatment of persistent epithelial defects in patients without healthy donor tissue in their fellow eyes. After a total superficial keratectomy, lenticules of donor cornea covered by epithelium are placed at the corneoscleral limbus. The epithelium spreads from the lenticules and covers the center of the cornea. In three of four patients with persistent defects of long duration, healing was successful and vision improved. The three successful procedures were in patients who had had chemical injuries. They retained useful visual function…

Refractive surgery and dry eye - An update.

Dry eye disease is the most common complication and a frequent cause of patient dissatisfaction after corneal laser refractive surgery, which includes laser-assisted in situ keratomileusis (LASIK), small-incision lenticule extraction (SMILE), and photorefractive keratectomy (PRK). It has a complex, multifactorial etiology and is characterized by a highly variable clinical presentation. A detailed preoperative screening and optimization of the ocular surface prior to refractive surgery are the key to minimizing the incidence and severity of postoperative dry eye. Diagnosis of postrefractive sur…

Update on contraindications in laser corneal refractive surgery.

More than 25 years of experience with refractive surgery techniques (since the FDA [Food and Drug Administration] of the United States approved the use of the excimer laser in the remodeling of the cornea in 1995), added to the technological advances in ophthalmology (femtosecond laser, topography-guided ablation, anterior segment imaging systems) and increased understanding of refractive errors, higher order aberrations, and corneal biomechanics make corneal laser refractive surgery a safe method and effective for the correction of these defects. However, to date, there are still certain circ

Topography-guided laser refractive surgery.

PURPOSE OF REVIEW: Topography-guided laser refractive surgery seeks to correct vision by altering the major refractive surface of the eye. Whereas results are not significantly different from current treatment options for primary surgery, topography-guided treatment is uniquely effective in eyes with corneal irregularity. This review highlights topography-guided ablations, emphasizing recent advances in treating highly aberrated eyes, including treatment for corneal ectasia in conjunction with collagen cross-linking (CXL). RECENT FINDINGS: Studies continue to document similar outcomes between
