A palliative care resource for professional carers of people with learning disabilities.

People with learning disabilities who have a life-threatening illness, are as entitled as other members of the population to receive good palliative care in their home of choice. However, professional carers of people with learning disability are generally unaware of the meaning of palliative care, and how they can access palliative care support. More importantly, they may feel they are not capable of caring for a resident with a life-threatening illness in the home environment. This article uses a case study to help illustrate the value of compiling a resource booklet for professional carers

Does nutrition support have a role in managing cancer cachexia?

PURPOSE OF REVIEW: Cachexia is a negative prognostic factor in cancer patients. The pathogenesis is related to a variable combination of reduced food intake and metabolic changes. However, whether nutritional support may contribute to effectively prevent and treat cachexia remains a debated issue. RECENT FINDINGS: Consistent evidence demonstrates that anabolic windows of opportunity occur during the clinical trajectory of cancer patients. Also, the use of specific nutrients, namely omega-3 fatty acids, may enhance the efficacy of nutritional support when tumor-driven inflammatory response is

Death revisited: rethinking death and the dead donor rule.

Traditionally, people were recognized as being dead using cardio-respiratory criteria: individuals who had permanently stopped breathing and whose heart had permanently stopped beating were dead. Technological developments in the middle of the twentieth century and the advent of the intensive care unit made it possible to sustain cardio-respiratory and other functions in patients with severe brain injury who previously would have lost such functions permanently shortly after sustaining a brain injury. What could and should physicians caring for such patients do? Significant advances in human o

Living with incurable cancer at the end of life--patients' perceptions on quality of life.

This study describes incurably ill cancer patients' perceptions of the quality of life at the end of life. Purposeful sampling was used to get as comprehensive descriptions as possible about quality of life. Five patients participated in repeated focus groups. Content analysis showed that the patients emphasized what they could perform and enjoy in daily life, not their shortcomings. Five themes were developed: "valuing ordinariness in life," "maintaining a positive life," "alleviated suffering," "significant relations," and "managing life when ill." The themes describe that the patients could

Successful use of non-invasive positive pressure ventilation in a complicated flail chest.

The current advanced trauma life support manual states that patients with significant hypoxia namely, SaO2 <90% on room air as a result of pulmonary contusion should be intubated and ventilated within the first hour of injury. Recently, several researchers have shown improved outcomes when patients with acute respiratory failure are managed with non-invasive positive pressure ventilation NIPPV. Trauma patients may also benefit from this therapy. We report a case of 15-year-old boy with isolated flail chest and pulmonary contusion, who was intubated in the emergency room, and was managed succes

Respiratory failure in the patient with AIDS: is critical care warranted?

The issue of whether or not the life-sustaining treatments (LST) of critical care is warranted for patients with the acquired immunodeficiency syndrome (AIDS) experiencing acute respiratory failure has been raised. Factors that have contributed to this issue include the AIDS epidemic, the high costs of critical care, limited intensive care resources, high mortality, prolonged suffering, and the effects on caregivers in delivering futile care. This article examines this issue from the ethicality of the decision-making process regarding LST in context with medical futility, patient autonomy, and

Ambivalent experiences of people in treatment for tuberculosis-HIV coinfection: a meta-synthesis of qualitative studies.

This study examined the scientific literature, by means of a systematic review with meta-synthesis (PROSPERO No. CRD42023410141), to discover the experiences of people undergoing treatment for tuberculosis-HIV coinfection. Ten databases and reference lists were searched for original articles with qualitative data, published in Portuguese, English and/or Spanish up to 2022. The 11 studies included yielded a meta-theme comprising four themes: (i) tuberculosis-HIV coinfection diagnosis and treatment have impact on the life of the affected person; (ii) personal, social and economic vulnerabilities

Creating a Pathway to Health-Care Professions for Historically Marginalized Students.

Developing a diverse talent pool starting at the high-school level, while students are making future education and career decisions, should be a national priority, given the need to build a diverse health-care workforce. This article describes a 6-week immersive simulation-based summer program to introduce 20 junior high-school students (13-15 years old) to the range of health professions. Because precollege students typically receive limited exposure to clinical settings, high-fidelity simulation is an excellent surrogate for providing realistic experiences in health care. Students heard lect

Percutaneous reverse Potts shunt improves right ventricular function in patients with suprasystemic idiopathic pulmonary arterial hypertension. There are no data regarding the anesthesia in this high-risk procedure. We report our experience of the anesthetic management for the creation of percutaneous reverse Potts shunt in children with suprasystemic idiopathic pulmonary arterial hypertension. This study included 10 patients presenting with symptomatic idiopathic pulmonary arterial hypertension despite undergoing medical treatment. All patients underwent gradual induction of anesthesia to mai
