Isolated mediastinal necrotizing granulomatous lymphadenopathy due to cat-scratch disease.

We report a patient suffering from cat-scratch disease limited to mediastinal lymphadenitis. Although rare, cat-scratch disease should be considered in the differential diagnosis of mediastinal lymphadenitis, especially when patients were exposed to cats.

Clinical factors associated with invasive intervention and outcomes of drainage in pediatric tularemia.

UNLABELLED: Tularemia is an important zoonotic infection in endemic regions, and delayed diagnosis may be associated with suppurative lymphadenitis and need for invasive intervention. Although antimicrobial therapy remains the cornerstone of treatment, the optimal management of suppurative lymphadenitis remains uncertain. This study aimed to characterize pediatric tularemia and evaluate treatment outcomes, with particular emphasis on drainage-based management of suppurative lymphadenitis. This retrospective, single-center study included children with laboratory-confirmed tularemia. Demographic…

Endoscopic retrograde appendiceal irrigation for children with recurrent abdominal pain and appendiceal fecaliths: a retrospective cohort study.

BACKGROUND: Recurrent abdominal pain (RAP) in children with appendiceal fecaliths and mesenteric lymphadenitis poses a diagnostic and therapeutic challenge, often mimicking appendicitis. The role of interventional endoscopy in this setting is undefined. METHODS: We retrospectively analyzed children with RAP, sonographically confirmed appendiceal fecaliths, and ileocecal lymphadenitis who underwent ultrasound-guided endoscopic retrograde appendiceal irrigation (ERAI) from 2021 to 2023. Technical success, complications, immediate symptom resolution, and follow-up outcomes were assessed. RESULT…

Ultrasonographic evaluation of cervical lymph nodes in Kawasaki disease.

OBJECTIVE: Kawasaki disease (KD) is one of the common causes of cervical lymphadenopathy during early childhood. The purpose of this study was to compare the ultrasonographic feature of cervical lymph nodes in patients with KD, bacterial lymphadenitis, and infectious mononucleosis. DESIGN: We studied 22 patients with KD, 8 with presumed bacterial lymphadenitis, and 5 with Epstein-Barr virus infectious mononucleosis. We examined the cervical nodes by ultrasonography using a 7.5-MHz or 10-MHz transducer of a B-mode sector scanner in all patients with a chief complaint of fever and a visible cer…

Clinical Characteristics and Outcomes of Severe Adverse Reactions to Bacille Calmette-Guérin (BCG) Vaccination in China: A Single-Center Retrospective Study.

OBJECTIVES: This study investigated the clinical features, treatment approaches, and outcomes of severe Bacille Calmette-Guérin (BCG) vaccine complications in Chinese children. INTRODUCTION: BCG is the only available vaccine for tuberculosis (TB) prevention but can cause serious complications such as local abscesses, lymphadenitis, and disseminated BCG. Research on their management in China is limited. METHODS: This observational study reviewed 95 children with severe BCG-related complications treated at a tuberculosis-specialized hospital in Guangdong, China (2016-2020). Of these, 33 (34.7%…

Subcutaneous surprise.

Melioidosis is a zoonosis caused by the accidental pathogen Burkholderia pseudomallei, which is endemic in Southeast Asia and northern Australia. The mortality of melioidosis is 20-50% even with treatment. Suppurative lymphadenitis caused by melioidosis has been rarely encountered by clinicians practicing in endemic areas. In the majority of previously described patients, the infected lymph nodes were in the head and neck region, except for four patients who presented with unilateral, inguinal lymphadenitis. Hence, we report a case of unilateral suppurative inguinal lymphadenitis caused by B. …

[Isolated adenopathy in leishmaniasis due to Leishmania infantum].

INTRODUCTION: Leishmaniasis is rarely manifested by isolated lymphadenitis in the mediterranean area, only 13 cases have been reported. OBSERVATION: An isolated cervical lymphadenitis was observed in a 37 year-old man living in the north of Tunisia. The fine needle aspiration of the enlarged lymph node found Leishmania in both direct examination and culture. Isoenzymatic typing identified Leishmania infantum MON-1. The evolution was slowly favourable without treatment. DISCUSSION: Despite the rare number of cases described, looking for leishmaniasis should be systematic when confronted with …

Constitutive AKT activation in follicular lymphoma.

BACKGROUND: The phosphoinositide 3- kinase (PI3K) pathway is involved in the growth of various human cancers, including lymphoid malignancies. However its role in the pathogenesis of follicular lymphoma (FL) has not been yet described. METHODS: To clarify this point, biopsy tissue samples from 38 human FL cases were investigated for PIK3CA somatic mutations in exon 9 and 20 using direct sequencing. The same samples were analyzed using western blotting and immunohistochemistry to detect expression of AKT, phosphorylated AKT (pAKT), and PTEN proteins. Two cases of benign lymphadenitis were used…

Mycobacterial Infections of the Head and Neck.

Mycobacteria are important causes of head and neck infections. Mycobacterial lymphadenitis may be caused by both Mycobacterium tuberculosis and a variety of nontuberculous myocbacteria. Changes in the epidemiology of tuberculosis have caused a shift of the peak age range of tuberculous lymphadenitis from childhood to ages 20 to 40 years. Short-course chemotherapy is highly effective. Mycobacterium avium has become the most common cause of nontuberculous lymphadenitis, but new mycobacterial species are increasingly recognized. Treatment consists primarily of complete surgical excision, although…

Epidemiology and Outcomes of Mycobacterium abscessus Infections in Immunocompetent Children in Singapore.

We describe Mycobacterium abscessus infections in immunocompetent children with cervicofacial lymphadenitis and localized soft tissue infections. The presence of multi-nodal disease was significantly associated with refractory infection in cervicofacial lymphadenitis. Almost all children were treated with both antimicrobials and surgery, and close to half required repeat surgery.
