Massage-induced brachial plexus injury.

BACKGROUND AND PURPOSE: There are few reports in the literature of adverse effects resulting from massage therapy (MT) and no reports of brachial plexus injury (BPI) associated with MT. This case report describes an uncommon case of BPI that developed after a session of MT and reviews previously published reports of peripheral nerve injury following MT. CASE DESCRIPTION: A 58-year-old Asian woman developed sudden unilateral paralysis of her left shoulder girdle after a session of MT. A diagnosis of acute BPI was suspected due to her recent history and the results of several examinations. The

Cutaneous silent period in fibromyalgia.

OBJECTIVES: The pathogenesis of widespread pain and increased tenderness in Fibromyalgia (FM) are still unknown. Recently, the role of central nervous system hyperexcitability is emphasized in pathogenesis of FM. The central sensitization was demonstrated with decrease in nociceptive flexion reflex (NFR) threshold in patients with FM. The NFR and cutaneous silent period (CuSP) are excitatory and inhibitory parts of the same spinal protective reflex, respectively. The aim of this study was to evaluate the changes in CuSP in FM. PATIENTS AND METHODS: Twenty-eight patients with FM and 18 healthy

A case of diabetic neuropathy combined with Guillain-Barre syndrome.

A 59-year-old man was admitted with numbness, pain, and a tingling sensation in both lower legs. He was initially diagnosed with diabetic peripheral neuropathy based on a symptom questionnaire and a quantitative sensory test. Despite symptomatic treatment of diabetic neuropathy, he complained of worsening sensory symptoms and additional motor weakness in both lower extremities. As the motor weakness of both extremities became more aggravated over time, brain and spine imaging tests and a nerve conduction test were performed. The nerve conduction study revealed motor and sensory axonal neuropat

Pitfalls in electrodiagnosis of Guillain-Barré syndrome subtypes.

OBJECTIVE: To electrophysiologically classify an Italian Guillain-Barré syndrome (GBS) population into demyelinating and axonal subtypes, to investigate how serial recordings changed the classification and to underline the pitfalls in electrodiagnosis of GBS subtypes. METHODS: The authors applied two current electrodiagnostic criteria sets for demyelinating and axonal GBS subtypes in 55 patients who had at least two serial recordings in three motor and sensory nerves. RESULTS: At first test, the electrodiagnosis was almost identical with both criteria: 65-67% of patients were classifiable as

Eletrophysiological study of the caudal nerve on developing rats.

PURPOSE: To non-invasively study the sensory nerve conduction of the caudal nerve of normal developing rats. METHODS: Twenty normal Wistar male rats served as subjects. Caudal nerve conduction studies were performed at 60 days from birth and weekly at end of six consecutive weeks. The caudal nerve was stimulated distally and nerve potentials were recorded proximally on the animal's tail using common 'alligator' clips as surface electrodes. RESULTS: The amplitude and the conduction velocity of the caudal nerve sensory action potential increased linearly from 29+/-6 microV to 85+/-13 microV an

Case of acute motor conduction block neuropathy (AMCBN).

We describe a 21 year-old man with an acute development of weakness whose clinical and serial electrophysiological findings were atypical for Guillain-Barré syndrome. Electrophysiological data suggested a diagnosis of "acute motor conduction block neuropathy" (AMCBN). The 6 months of disease duration and the electrophysiological follow-up, which never showed axonal degeneration until complete clinical recovery, raise the issue of the relationship between AMCBN and acute motor axonal neuropathy (AMAN).

[Effects of electroacupuncture at shu-points of the five zang-organs on electrophysiologic function of sciatic nerve in the rabbit of Guillain-Barre syndrome].

OBJECTIVE: To probe into the mechanism of electroacupuncture at shu-points of the five zang-organs for treatment of Guillain-Barre syndrome (GBS). METHODS: Eighty healthy giant-ear white rabbits were randomly divided into 4 groups, a blank group, a model group, an electroacupuncture (EA) group and an immunoglobulin group, 20 rabbits in each group. The internationally recognized P 2 immune rabbit model was used in the study. The EA group were treated with EA at shu-points of the five zang-organs and the immunoglobulin group with intravenous injection of immunoglobulin 80 mg/kg/day. The sciatic

Reversible respiratory failure due to rhabdomyolysis associated with cytomegalovirus infection.

A 58-year-old woman presented with muscle weakness, whole body myalgia, and dyspnea. On admission, neurological examination showed proximal muscle weakness in the extremities. The weakness gradually extended to the bulbar and respiratory muscles, necessitating an artificial ventilator. Serum CK level was markedly increased (33,774 IU/L; normal <150 IU/L) and myoglobinuria was noted in urinalysis. There was no sign of renal failure. Nerve conduction study was normal, but needle EMG showed myopathic changes in the weak muscles. Serological studies for virus titers showed more than a four-fold in

The role of neurodiagnostic studies in nerve injuries and other orthopedic disorders.

Neurodiagnostic techniques, such as nerve conduction studies, needle electromyography studies, intraoperative nerve monitoring, and evoked potentials provide useful information for practicing orthopedic surgeons to help localize central from peripheral nervous system lesions. For peripheral nerve lesions, it helps localize the level of the nerve dysfunction (for example, root versus plexus versus peripheral nerve). These techniques are well established and routinely used. Newer techniques have emerged that aim to simplify the technical process of performing these studies and help reduce the di

Radial nerve injuries associated with humeral fractures.

A radial nerve injury associated with a humeral shaft fracture is an important injury pattern among trauma patients. It is the most common peripheral nerve injury associated with this fracture. Although treatment for this injury pattern is a controversial subject among upper-extremity surgeons, certain principles of management need to be applied in all cases. As our understanding of the pathoanatomy of the humerus and surrounding neurovascular structures has evolved, surgeons have adapted their strategies to improve outcome and avoid long-term morbidity. The principles of management and the cl

Neurophysiologic studies in early-onset cerebellar ataxia.

The discovery of the gene for Friedreich's ataxia (FRDA) has not only broadened the FRDA phenotype, but has also identified patients with early-onset cerebellar ataxia who resemble FRDA clinically but who do not carry a mutation in the frataxin gene. In order to identify subgroups that may represent a uniform underlying disorder, we performed neurophysiologic studies, including nerve conduction studies, electromyography, and transcranial magnetic stimulation, in 15 patients with a slowly progressive, unexplained, early-onset cerebellar ataxia (EOCA). In addition, sural nerve biopsy data were a

[Chronic inflammatory polyneuropathies].

There is an increasing number of peripheral nerve disorders with inflammatory and immune mechanisms involved. The precise diagnosis is of utmost importance, since these patients can be successfully treated. Unfortunately, there is no specific marker for any disease of this group. The diagnosis therefore relies on the appropriate consideration of the clinical, neurophysiological and laboratory data, which requires in-depth knowledge of these diseases. In this paper we review the diagnostic criteria and treatment strategies for the major types of chronic inflammatory polyneuropathies.

Slowed conduction velocity of the median sensory nerve across the carpal tunnel in normal adults.

OBJECTIVE: To examine the difference of using onset or peak latency in the segmental conduction study of the median nerve in normal adults without carpal tunnel syndrome. DESIGN: We investigated 60 hands of 30 young healthy adults (15 men and 15 women; mean age, 27.1 yrs; range, 18-36 yrs). We performed the mid-palm segmental conduction study using antidromic and orthodromic techniques in the median sensory nerves of the second and third digits and the onset and peak latencies of the proximal (wrist-palm) and distal (palm-digit) segments were measured. The distoproximal latency ratios were co

Diagnosing mild Carpal Tunnel Syndrome with interpolation.

Carpal Tunnel Syndrome (CTS) is the most common peripheral nerve entrapment. In the diagnosis of Carpal Tunnel Syndrome, velocity is determined by either measuring the average velocity over a segment by dividing the distance by the difference in latencies. Polynomial interpolation can determine the velocity at any point along a nerve. Applying interpolation techniques correctly classified 58/60 electrophysiologically proven CTS cases (96.7%) and 36/38 normals (94.7%). Of 7 cases with CTS by standard sensory criteria alone, 6 (86%) had abnormal motor conduction using the interpolation technique
