OBJECTIVE: To determine the impact of controlled incremental changes in circuit blood flow resistance to the oxygenator on hemodynamics in the fetal lamb supported by the EXTrauterine Environment for Neonatal Development (EXTEND) system.
METHODS: A prospective study of oxygenator circuit clamping was conducted on 7 lambs on EXTEND. Echocardiography was performed at four levels of circuit flow (250, 225, 200 and 300 mL/kg/min). Hemodynamic parameters including cardiac output, velocities and pulsatility indexes (PI) were measured, and physiological parameters were documented.

Renal arteriovenous fistulae (AVF) are a rare condition that can be acquired, congenital, or idiopathic. Idiopathic AVF are the rarest type of renal AVF. Traditionally, renal AVF were treated with surgery, but the mainstay of treatment has shifted to an endovascular approach by coil embolization. Embolization procedures are often difficult secondary to the "high flow" of the AVF, and there is the substantial risk of distal embolization of coils. We present a case in which an idiopathic renal AVF was successfully embolized with an Amplatzer vascular plug.

BACKGROUND: Recent histological studies of the aortic wall of patients with tetralogy of Fallot (TOF) have shown massive degeneration of the tunica media of the aorta. We used laser Doppler flowmetry (LDF) with a high energy (20 mW) laser to measure perfusion of the femoral head intraoperatively in 32 hips. The relation between flow velocity and the pressure decrease is evaluated in Blalock-Taussig shunts used in congenital heart surgery and is related to the flow conditions and geometries of the shunts studied. The authors propose that the flow conditions within the shunt as well as shunt dimensions need to be taken into account when using Doppler velocimetry to predict pressure drops across these shunts.
