Treatment of camptocormia with continuous subcutaneous infusions of apomorphine: 1-year prospective pilot study.

Camptocormia is defined as a marked dystonic flexion of the trunk in the sagittal plane. Camptocormia responds poorly to botulinum toxin injections, manipulation with dopaminergic treatment, and deep brain stimulation. We designed a prospective pilot study to assess the effect of apomorphine infusions on camptocormia. Five patients were enrolled. All five patients responded well to this treatment. The fact that camptocormia responds so well to apomorphine may be explained by the sustained stimulation of the ventrolateral striatal D1 receptors, alleviating this type of dystonia.

Age and activity but not lumbar spinal stenosis and muscle fatigue affect sagittal spinal alignment: A pilot study.

BACKGROUND: Ageing is associated with changes in spinal alignment. Lumbar spinal stenosis may alter spinal alignment due to pain, muscle atrophy and fatigue. While spinal alignment and motion are crucial in lumbar spinal stenosis, the effects of age, activity, and muscle fatigue on spinal alignment remain unclear. This study investigates these factors using motion capture analysis. METHODS: In 11 patients with lumbar spinal stenosis (5 M/6 W; age, 71 ± 9 years; body mass index, 29 ± 5 kg/m2), 10 older controls (5 M/5F; 65 ± 5 years; 25 ± 6 kg/m2), and 10 young controls (5 M/5F; 26 ± 2 years;

Radiomics-powered radiographic image analysis for enhanced mechanical complications prediction and surgical planning in adult spine deformity.

BACKGROUND CONTEXT: Radiomics, a technique employing machine learning (ML) to extract quantitative features from processed radiographic images, holds promise for improving clinical prediction models. It offers the potential to comprehensively characterize spinal shape and alignment. We hypothesized that processed image (PrIm) algorithms outperform traditional radiographic measurements (TRM) and scores in predicting postoperative mechanical complications (MC) in adult spinal deformity (ASD). PURPOSE: The aim was to compare the performance of PrIm algorithms to TRM-GAP score in the prediction o

Spine surgeon versus AI algorithm full-length radiographic measurements: a validation study of complex adult spinal deformity patients.

INTRODUCTION: Spinal measurements play an integral role in surgical planning for a variety of spine procedures. Full-length imaging eliminates distortions that can occur with stitched images. However, these images take radiologists significantly longer to read than conventional radiographs. Artificial intelligence (AI) image analysis software that can make such measurements quickly and reliably would be advantageous to surgeons, radiologists, and the entire health system. MATERIALS AND METHODS: Institutional Review Board approval was obtained for this study. Preoperative full-length standing

Influence of changes in pelvic anteversion during gait on walking ability and physical function in patients with adult spinal deformity: A cross-sectional study.

BACKGROUNDS: Evaluation of gait posture using a three-dimensional motion analysis system (3DMAS) revealed that elderly patients with adult spinal deformity (ASD) experience pelvic anteversion while walking. The purpose of this study was to investigate the influence of changes in pelvic anteversion during gait on walking ability and physical function in patients with ASD. METHODS: Fifty-four patients with ASD aged 50 years or older who were admitted to our hospital between March 2016 and December 2021 were included in the study. The 6-min walking distance (6MWD) was used to evaluate walking ab

Bone Mineral Density and Related Scores in Parkinson's Disease: A Systematic Review and Meta-Analysis.

BACKGROUND: Parkinson's disease (PD) is the second most common degenerative neurologic disorder in older adults, and increasing attention has been paid to bone health in PD. Although several studies have shown that patients with PD have a lower bone mineral density (BMD) than do non-PD controls, there have been no systematic reviews in recent years. METHODS: PubMed, Medline, and Web of Science were used to search relevant studies up to May 2020. BMD, BMD T score, and BMD Z score of patients with and without PD were statistically analyzed. Meta-analysis was conducted using Review Manager versi

Reversible postoperative blindness caused by bilateral status epilepticus amauroticus following thoracolumbar deformity correction: case report.

Postoperative vision loss (POVL) is a devastating complication and has been reported after complex spine procedures. Anterior ischemic optic neuropathy and posterior optic neuropathy are the 2 most common causes of POVL. Bilateral occipital lobe seizures causing complete blindness are rare and have not been reported as a cause of POVL after spine surgery with the patient prone. The authors report the case of a 67-year-old man without a history of seizures who underwent a staged thoracolumbar deformity correction and developed POVL 6 hours after surgery. Imaging, laboratory, and ophthalmologica

Long-term outcome of laminectomy for cervical ossification of the posterior longitudinal ligament.

OBJECT: Although laminectomy is an effective surgical technique for the treatment of multilevel cervical stenotic lesions, postoperative kyphosis and neurological deterioration have been frequently reported after laminectomy. Hence, laminectomy without fusion is seldom performed nowadays. However, the clinical impression from the long-term follow-up of patients who had undergone laminectomy does not support that postoperative kyphosis is common in patients with ossification of the posterior longitudinal ligament (OPLL). In this paper, the authors assessed the long-term outcome of laminectomy f

REEP1 Accumulation Disrupts ER Integrity and Drives Spinal Motoneuron Degeneration in Distal Hereditary Motor Neuropathy.

REEP1 contributes to the shaping of the endoplasmic reticulum (ER) through conserved transmembrane hairpins and a long C-terminal amphipathic helix. REEP1 loss-of-function causes hereditary spastic paraplegia due to degeneration of cortical motoneuron axons. Patients with deletion of REEP1 exon5 (Δexon5), which deletes part of its amphipathic helix, however, develop muscle atrophy due to degeneration of spinal motoneuron axons (distal hereditary motor neuropathy/dHMN). It is known that REEP1 knockout mice exhibit simplified ER structures in cortical motoneurons. Here, we show that these neuron

Modified ACDF Technique for the Treatment of Centrum Focal Ossification of the Posterior Longitudinal Ligament: A Case Report.

BACKGROUND: Anterior cervical discectomy fusion (ACDF) is a surgical procedure used to treat cervical spondylosis with anterior spinal cord compression. However, there are limitations to traditional ACDF and posterior indirect decompression when the anterior source lesion is in the center of the cervical vertebra. CASE PRESENTATION: On June 8, 2022, our department treated a patient with cervical spondylotic myelopathy-whose high posterior longitudinal ligament (OPLL) occupied the central position of the vertebral body-with modified ACDF. The preoperative surgical plan was designed based on th

Intrathecal ligaments and nerve root tension: possible sources of lumbar pain during spaceflight.

UNLABELLED: Lumbar intrathecal ligaments have recently been demonstrated to randomly bind dorsal nerve roots to the dura within the lumbar vertebral column. Lengthening of the vertebral column and associated lumbar back pain experienced by astronauts is common in microgravity. This study was designed to investigate the relationship of lumbar intrathecal ligaments in spinal lengthening as a possible mechanism for back pain. METHODS: A two-part study was designed using 36 vertebral columns from embalmed cadavers. There were 12 vertebral columns studied in mid-sagittal section to demonstrate the

Evaluating Surgical Strategies for Adolescent Idiopathic Scoliosis: A Meta-Analysis of Posterior Spinal Fusion and Vertebral Body Tethering.

Adolescent idiopathic scoliosis (AIS) is a three-dimensional spinal deformity commonly affecting adolescents worldwide. Surgical management traditionally involves posterior spinal fusion (PSF), which achieves stable deformity correction but may limit spinal motion and growth potential. Vertebral body tethering (VBT) has emerged as a motion-preserving alternative aimed at modulating spinal growth while correcting curvature. This systematic review and meta-analysis compared PSF and VBT in terms of deformity correction, complication and revision rates, and postoperative functional outcomes. Data
