OBJECTIVE: To evaluate the results of Mitrofanoff continent urinary diversion in a group of women with persistent severe incontinence after vesicovaginal fistula (VVF) secondary to obstructed labour.
PATIENTS AND METHODS: Seven women with severe incontinence following a VVF were offered the Mitrofanoff procedure after all other attempts had failed to restore continence. In three patients a caecocystoplasty formed the urinary reservoir and in four the bladder was used. In all seven patients the appendix was used as the conduit for self-catheterization. The mean (range) follow-up was 10 (3-14)

OBJECTIVE: To present a modified technique and early outcomes of a continent catheterizable vesicostomy in pediatric patients with either flaccid neurogenic bladder or intractable voiding dysfunction and large capacity bladder.
METHODS: Six patients underwent the procedure from October 2014 to December 2015. A 4-cm Pfannenstiel incision was made, avoiding intraperitoneal dissection. After adequate mobilization, a 2-cm vertical flap at the dome of the bladder was identified and tubularized over a 12Fr catheter with 4-0 vicryl suture. The tubularized flap was then intussuscepted into the bladde

Bacteria resistant to both the agents deployed to prevent infections and those used to treat infections would be formidable nosocomial pathogens. The aim of this paper is to review the evidence that gram-negative bacteria resistant to antibiotics and biocides have emerged and been responsible for catheter-associated urinary tract infection. A study of patients undergoing intermittent bladder catheterization revealed that the frequent application of the antiseptic chlorhexidine to the perineal skin prior to the insertion of the catheter was effective against the normal gram-positive skin flora

Vesicourethral anastomotic stenosis (VUAS) is a complication of radical prostatectomy characterized by fibrotic narrowing at the surgical connection of bladder and urethra. It is distinct from bladder neck contractures (BNC) as BNC occur with prostate in situ and VUAS occurs after a surgical anastomosis is created after removal of the prostate. Clinically, VUAS may present with obstructive lower urinary tract symptoms, fistula, urine leak, or paradoxically improving continence due to the narrowing of the vesicoureteral junction. There are multiple risk factors for VUAS from preoperative, intra

Detrusor underactivity (DUA) is defined as a voiding contraction of reduced strength and/or duration, which prolongs urination and/or prevents complete emptying of the bladder within a 'normal' period of time. This issue is associated with voiding and postmicturition urinary symptoms, and can predispose to urinary infections and acute urinary retention. The aetiology of DUA is influenced by multiple factors, including ageing, bladder outlet obstruction, neurological disease, and autonomic denervation. The true prevalence of this condition remains unknown, as most data come from referral popula

A review of 160 male patients admitted to the Shepherd Spinal Center was completed to determine the influence of urodynamic findings and choice of bladder management program on the risk of developing upper urinary tract distress following acute spinal injury. Within the context of this investigation, upper urinary tract distress was defined as presence of hydronephrosis, febrile urinary tract infection, urolithasis or vesicoureteral reflux. Three groups were identified according to the urodynamic findings and the bladder management program. Thirty-four subjects with preservation of detrusor fu

We analyzed the incidence and risk factors associated with urinary retention following transurethral resection of the bladder tumors (TURBT). This retrospective study included a total of 218 TURBT cases we experienced from September 2021 to April 2023. Twenty-six patients (12%) experienced urinary retention after the procedure. The use of a thick urethral catheter (20 Fr or larger) post-surgery (p=0.013) was identified as a significant risk factor. Of the 26 patients with urinary retention, 18 were able to recover spontaneous urination, while the remaining 8 required either permanent urethral

There is evidence that there is no difference in the incidence of urinary tract infections between multiple-use catheters that are cleaned after use and single-use catheters for intermittent catheterization. Despite the very significant economic and environmental advantages of multiple-use catheters, the single-use option is still promoted. The increasing focus on sustainability in urology may prompt a shift in practice to multiple-use catheters or other novel solutions in this setting. PATIENT SUMMARY: In intermittent catheterization, a patient or their caregiver periodically inserts a cathet

The aim of this study was to compare the accuracy of bladder volume measurements using a portable ultrasound machine to measurement by catheterization. For 13 consecutive weeks, all patients admitted to the stroke unit at Royal Perth Hospital were studied by both methods when urinary retention was suspected. The accuracy of ultrasound bladder volume measurements and interobserver reliability were evaluated. Ninety ultrasound examinations were performed prior to catheterization, 70 were by one nurse, and 20 independently by two nurses. Correlation of ultrasound measurements with actual catheter

BACKGROUND: Clean intermittent self-catheterisation (CISC or ISC) is used by patients/carers to empty the bladder if needed. Sometimes the urethral lumen leading out of the bladder is blocked; sometimes, the bladder (detrusor) muscle itself or the autonomic motor nerves innervating the bladder are damaged, resulting in a failure of the detrusor muscle to work, leading to a failure of the bladder being able to empty adequately. Prior consensus as to the indications and timing of CISC has yet to be provided. This article aims to provide a multidisciplinary consensus view on this subject.

The most common cause of a retained Foley catheter is failure of the balloon to deflate. If noninvasive means are unsuccessful, balloon puncture is required. We present a safe and efficient technique using flexible video cystoscopy for balloon puncture and removal of any free fragments formed.

PURPOSE: In a previous study an external condom catheter was used to measure noninvasively bladder pressure during interruption of the flow rate. The pressure increase in the condom sometimes caused a sphincter contraction that made bladder pressure measurement unreliable. Therefore, we developed a new variable outflow resistance catheter to measure noninvasively bladder pressure without interrupting the flow rate.
MATERIALS AND METHODS: The new catheter consists of an incontinence condom connected to a set of various outflow tubes and a pressure transducer. A remotely controlled pneumatic va

Purple urine bag syndrome (PUBS) is a visually striking and a rare side effect in a patient with urinary tract infection where urine in the catheter bags and tubing turn purple. The urine in PUBS gets its color from the combination of two pigments, indirubin and indigo, which are breakdown products of tryptophan. The most important risk factors include long-term catheterization, female gender, chronic constipation, old age and being bed bound. Here, we present a case of PUBS in an elderly female with a history of bladder cancer that needed catheterization along with constipation.
