Should high-risk adolescents have Papanicolaou tests?

BACKGROUND: The current American College of Obstetricians and Gynecologists guidelines state that cervical cancer screening should begin at age 21 years, regardless of sexual or obstetric history. However, previous studies have demonstrated that there is a small but significant subset of high-risk adolescents with extensive sexual and obstetric history who harbor a significant squamous cervical lesion. The objective of the current study was to use histologic and demographic data from adolescents (aged <21 years) who received Papanicolaou (Pap) tests to determine whether they benefited from ear

Screening for cancer of the cervix with simultaneous pap smear and colposcopy. The efficacy of pap smear and colposcopy.

OBJECTIVE: Some Japanese institutes have been performing a population screening program for cervix cancer involving the simultaneous use of Pap smear and colposcopy. This program may be a good model for evaluating the efficacy of Pap smears and colposcopy. METHODS & MATERIALS: The subjects included 2,000 women who underwent primary screening at the Kanagawa Health Service Association. RESULTS: 1) The incidence of ACF (atypical colposcopic findings) was 3.6%, whereas that of abnormal Pap smears (ASC-US and above) was 1.1%; 2) Of 88 women who showed abnormal findings on Pap smear and/or colpos

Persistent inflammation on Pap smear: does it warrant evaluation?

OBJECTIVE: Due to the low sensitivity of Pap smear, premalignant lesions of the cervix can be missed in women with inflammatory Pap smears. However, it is not practically possible to subject all women with inflammatory Pap smear to colposcopy. This study was carried out with the aim to evaluate whether women with persistent inflammation on Pap smear need further evaluation with colposcopy. MATERIALS AND METHODS: Four hundred and twenty women were screened at a tertiary level hospital with Pap smear. Women with inflammation on Pap smear were given treatment as per WHO guidelines and Pap smear

[A woman with a special cervix].

A 40-year-old woman (nulligravida) presented with abnormal cervical cytology. At colposcopy a double external uterine os was discovered with a levonorgestrel intra uterine contraceptive device placed in the opening on the left side. Ultrasound revealed an uterus bicornis bicollis. Despite frequent examinations, the abnormality had not been discovered earlier. Therefore, contraception and cervical cytology sampling had most likely been inadequate.

[Exploring the issue of cervical cancer prevention and control in women living with HIV/AIDS].

Shared risk factors mean that women engaging in unprotected sex are at risk of contracting both the human immunodeficiency virus (HIV) and the human papillomavirus (HPV). As HIV suppresses bodily immune systems, an HIV-positive woman with HPV will likely see a faster progression of cancer cells around the cervix. Therefore, it is important to prevent and control cervical cancer in HIV-positive women. Findings from the literature on this topic include: 1) HIV-positive women face a relatively greater risk of: HPV surveillance, contracting high-risk HPV subtypes, contracting more subtypes of HPV,

Hyperchromatic crowded cell groups in gynaecological liquid-based cytology samples.

Cervical cytology screening is a complex and demanding procedure. Correct diagnosis depends on accurate interpretation of cells, including dense clusters known as hyperchromatic crowded cell groups (HCCG). These groups are frequently encountered in liquid-based cytology (LBC) samples and can be difficult to identify as a specific cell type. Although usually benign and thus often overlooked, they may occasionally represent severely abnormal cells. Thus, their correct interpretation is vital for accurate reporting. Such groups are responsible for false-positive and false-negative reporting and h

Endometrial aspiration cytology for diagnosis of peritoneal lesions in extrauterine malignancies.

OBJECTIVE: To evaluate the usefulness of endometrial aspiration cytology for assessing malignant cells of extrauterine origin. STUDY DESIGN: Endometrial cytology was performed on 224 patients with primary ovarian cancer, 10 with fallopian tube cancer and 45 with peritoneal tumors. RESULTS: Of 224 patients with ovarian cancer, 53 (23.7%) had positive endometrial cytology. Positive rates were: stage I, 4.3%; stage II, 25.0%; stage III, 39.7%; stage IV, 34.5%. Histologic positive rates were: serous, 28.7%; mucinous, 11.4%; clear cell, 23.1%; endometrioid and unclassifiable adenocarcinomas, 28.0

Atypical glandular cells.

The 2001 Bethesda System has radically altered the classification of glandular abnormalities. The recognition of the cytologic features of atypical glandular cells on cervicovaginal smears is important because a significant number of patients will be found to have an underlying cancerous or dysplastic lesion of the exocervix, endocervix, or endometrium. The differential diagnosis of AGC on cytology is diverse and accurate classification is necessary because the most appropriate form of follow-up depends on the specific subcategorization of the atypical glandular cells. Because the level of int…

Well-differentiated villoglandular adenocarcinoma of the uterine cervix: cytomorphologic observation of five cases.

We retrospectively examined the cytologic findings of well-differentiated villoglandular adenocarcinoma (VGA) treated in our hospital. Cervical smears of four cases and a touch preparation of another case of VGA formed the materials for the study. The cytologic features were correlated with the histomorphology of VGA. Architecturally, long slender papillae and cohesive branching epithelial sheets with smooth borders and a lack of feathery edge were observed. Crowding and overlapping of nuclei were noted. The nuclei were uniform, small, and round to oval-shaped, with evenly distributed granular

Neurilemmoma: an unusual benign tumor of the cervix.

BACKGROUND: A Neurilemmoma (also called Schwannoma) is a benign, slowly growing neoplasm of the Schwann cells which may occur in association with any nerve. Its finding in the cervix of the uterus is extremely rare. CASE: At a routine annual exam, a 47-year-old woman was found to have a tumor on the posterior lip of the cervix. The patient was completely asymptomatic. The tumor was excised using a large electrical loop (LEEP) and found to be a benign Neurilemmoma. Subsequently a CT scan of the pelvis did not reveal any other abnormalities and no further treatment was contemplated. CONCLUSION

Exfoliative cytology of atypical polypoid adenomyoma. A case report.

BACKGROUND: An atypical polypoid adenomyoma (APA) is a well-defined entity. It occurs in the endometrium, lower uterine segment and endocervix. It is usually composed of atypical complex glands with squamous metaplasia admixed with myofibromatous stroma. CASE: A 35-year-old female presented with one-year history of irregular menstrual periods. A diagnosis of adenocarcinoma in situ was rendered on her cervicovaginal smear. Pelvic examination revealed an enlarged uterus due to a leiomyoma. Colposcopic examination revealed a 0.6-cm, sessile, polypoid mass at the junction of the endocervix and ec

Carcinoma of the endometrium.

Carcinoma of the endometrium is the most common gynecologic malignancy. The majority of women present with stage I disease, and the most common presenting symptom is postmenopausal bleeding. Early detection of endometrial cancer is important, because up to 90 percent of patients with stage I disease can be successfully treated. Certain risk factors, such as obesity, hypertension and diabetes mellitus, are associated with the development of this malignancy. Office endometrial sampling has a sensitivity of up to 97 percent for diagnosing carcinoma of the endometrium and can often eliminate the n

Conservative treatment of mild/moderate cervical dyskaryosis: long-term outcome.

There is some controversy about the management of women with mild or moderate dyskaryotic cervical smears. To assess the strategy of an established cervical cytology screening programme (Grampian region, northeast Scotland) we identified 500 women who had had mild or moderate cervical dyskaryosis in 1978 or 1979, and 500, matched by age, who had had a normal smear at that time. Follow-up smear results and any subsequent investigation by colposcopy, cone biopsy, or hysterectomy, with biopsy result were recorded. Of the 500 women who initially had an abnormal smear, 300 (60%) had a smear that wa
