Immediate reduction under general anesthesia and single-staged anteroposterior spinal reconstruction for fracture-dislocation of lower cervical spine.

Fracture-dislocation of the lower cervical spine is a severe traumatic lesion, most frequently resulting in tetraplegia. Treatment is usually painful and time consuming. This retrospective study evaluated the clinical curative effect of immediate reduction under general anesthesia and single-staged anteroposterior spinal reconstruction for fracture-dislocation of the lower cervical spine. Twelve cases of traumatic lower cervical spinal fracture-dislocation were retrospectively analyzed from January 2006 to December 2011. The injury level was C4/C5 in 4, C5/C6 in 5, and C6/C7 in 3 patients. The

Comparison between colloid preload and crystalloid co-load in cesarean section under spinal anesthesia: a randomized controlled trial.

BACKGROUND: Hypotension is a common problem during spinal anesthesia for cesarean delivery. Intravenous fluid loading is used to correct preoperative dehydration and reduce the incidence and severity of hypotension. Different fluid regimens have been studied but colloid preload and crystalloid co-load have not been compared. METHODS: In this randomized double-blind study, 210 patients scheduled for elective cesarean section under spinal anesthesia were randomly allocated to receive either 6% hydroxyethyl starch 130/0.4 500 mL before spinal anesthesia (colloid preload) or Ringer's acetate solu

Spinal anesthesia: should everyone receive a urinary catheter?: a randomized, prospective study of patients undergoing total hip arthroplasty.

BACKGROUND: The objective of this randomized prospective study was to determine whether a urinary catheter is necessary for all patients undergoing total hip arthroplasty under spinal anesthesia. METHODS: Consecutive patients undergoing total hip arthroplasty under spinal anesthesia were randomized to treatment with or without insertion of an indwelling urinary catheter. All patients received spinal anesthesia with 15 to 30 mg of 0.5% bupivacaine. The catheter group was subjected to a standard postoperative protocol, with removal of the indwelling catheter within forty-eight hours postoperati

Heterotopic pregnancy following IVF-ET: successful treatment with salpingostomy under spinal anesthesia and continuation of intrauterine twin pregnancy.

The extended use of assisted reproductive technologies is increasing the heterotopic pregnancies, leading to a potentially dangerous condition for the intrauterine pregnancy and the mother. We report a case of unruptured heterotopic pregnancy following IVF-ET at 6 weeks of gestation and the patient was treated with salpingostomy under spinal anesthesia. The intrauterine twin pregnancy course was uneventful with the delivery of healthy babies at 34th week by Cesarean section.

Epidural hematoma occurred by massive bleeding intraoperatively in cesarean section after combined spinal epidural anesthesia -A case report-.

We describe a case of acute lumbar epidural hematoma at the L2-3 level complicated by paraplegia, which occurred after coagulation disorder because of massive bleeding intraoperatively in cesarean section. The preoperative coagulation laboratory finding was in normal range and so we tried combined spinal epidural anesthesia. Uterine atony occurred in the operation, and there was persistant bleeding during and after the operation. After the operation, she complained of paresthesia on her both legs and was diagnosed with epidural hematoma (EDH) by radiologic examination. Emergency laminectomy on

Severity of locomotor and cardiovascular derangements after experimental high-thoracic spinal cord injury is anesthesia dependent in rats.

Anesthetics affect outcomes from central nervous system (CNS) injuries differently. This is the first study to show how two commonly used anesthetics affect continuously recorded hemodynamic parameters and locomotor recovery during a 2-week period after two levels of contusion spinal cord injury (SCI) in rats. We hypothesized that the level of cardiovascular depression and recovery of locomotor function would be dependent upon the anesthetic used during SCI. Thirty-two adult female rats were subjected to a sham, 25-mm or 50-mm SCI at T3-4 under pentobarbital or isoflurane anesthesia. Mean arte

Posterior reversible encephalopathy syndrome in an untreated hypertensive patient after spinal surgery under general anesthesia -A case report-.

Posterior reversible encephalopathy syndrome (PRES) is an unfamiliar term to anesthesiologists, and this is characterized by neurologic symptoms that include mental change, headache, seizure and visual disturbance and also abnormal neuroimaging finding. A 71-year-old female patient was operated on for posterior decompression and total laminectomy under general anesthesia for the spinal stenosis. After the operation, she developed generalized tonic-clonic seizure and a stuporous mentality in the recovery room. The magnetic resonance imaging (MRI) revealed swelling and increased signal intensity

Postdural puncture headache and back pain after spinal anesthesia with 27-gauge Quincke and 26-gauge Atraucan needles.

BACKGROUND AND OBJECTIVES: The purpose of this study was to determine whether the 26-gauge Atraucan needle shows any benefit on the incidence of postdural puncture headache (PDPH) and back pain as compared with the 27-gauge Quincke needle. METHODS: We investigated 388 patients, 171 men and 217 women, who were receiving spinal anesthesia for subumbilical surgery. We compared spinal anesthesia via the 27-gauge Quincke needle in 202 patients (group Q) and via the 26-gauge Atraucan needle in 186 patients (group A). RESULTS: The incidence of PDPH was 2.97% (6 of 202) in group Q and 2.69% (5 of 18

Acute normovolemic hemodilution combined with hypotensive anesthesia and other techniques to avoid homologous transfusion in spinal fusion surgery.

To avoid homologous transfusion in spinal fusion surgery, acute normovolemic hemodilution was combined with controlled hypotensive anesthesia. Patients were kept hemodiluted, not only during surgery, but also after surgery by delaying transfusion until the next morning or later. Transfusion decisions were made by clinical judgment rather than by preset value (ie, 10 g/dl). Other techniques used in this protocol were preoperative autologous blood donation and intraoperative salvage of shed blood. Only 4 of 119 patients (3.4%) required homologous blood, compared to 25 of 29 patients (86%) in 198

[Massive intracerebral hemorrhage and anterior spinal artery syndrome in etiologic relation to paraspinal anesthesia procedures? 2 case reports].

Complications in connection with operations also lead to the question of possible relations to the type of anaesthesia and their avoidability. The subject is treated demonstrating two cases of intracerebral haemorrhage and anterior spinal artery syndrome during and following catheter-epidural anaesthesia or spinal anaesthesia, respectively. No definite causal connection was established, even if the emergencies presumably would not have occurred without operation and anaesthesia. Complications are not completely avoidable, even if operation and anaesthesia as regards indication, execution and m

Comparison of Thoracic Segmental Spinal Anesthesia Using Isobaric Levobupivacaine With Fentanyl Versus Dexmedetomidine in Patients Undergoing Laparoscopic Cholecystectomy.

INTRODUCTION: Thoracic segmental spinal anesthesia is increasingly used as an alternative to general anesthesia for laparoscopic cholecystectomy because of its potential benefits in providing effective intraoperative anesthesia and improved postoperative recovery. The present study aimed to compare the hemodynamic effects, block characteristics, and postoperative analgesia effects of isobaric levobupivacaine combined with fentanyl versus dexmedetomidine in thoracic segmental spinal anesthesia. MATERIALS AND METHODS: In this prospective comparative study, 60 patients who underwent elective lap

ED95 determination of continuous norepinephrine infusion for preventing spinal anesthesia-induced hypotension in obese parturients during cesarean delivery.

BACKGROUND: Norepinephrine is an effective vasopressor for preventing and managing spinal anesthesia-induced hypotension during cesarean delivery. Nevertheless, data regarding the 95% effective dose (ED95) of continuous norepinephrine infusion in obese parturients remain scarce. METHODS: This prospective dose-finding study enrolled parturients scheduled for elective cesarean delivery under spinal anesthesia. Participants were stratified into two groups based on body mass index (BMI): non-obese group (BMI <30 kg/m2) and obese group (BMI ≥30 kg/m2). Intravenous norepinephrine infusion was initi

Fractionated versus Conventional Spinal Anesthesia for Prevention of Hypotension in Elderly Hip Fracture Surgery: A Randomized Controlled Study.

BACKGROUND: Spinal anesthesia (SA), though preferred for lower limb orthopedic surgery, produces rapid sympathectomy following bolus intrathecal injection, which may precipitate significant hypotension in elderly patients. Fractionated dosing may improve hemodynamic stability. This study compared fractionated versus bolus SA on hypotension requiring vasopressor support. METHODS: This prospective, randomized, double-blind trial included 118 American Society of Anesthesiologists I-III participants aged ≥60 years undergoing femur fracture surgery under SA. Participants were randomized to receive

Case Report and Literature Review of Spinal Anesthesia-Related Peroneal Nerve Injury Complicated with Diabetic Foot Peripheral Neuropathy.

Perioperative peripheral nerve injury (PPNI) is rare but potentially disabling. In patients with diabetic peripheral neuropathy (DPN), the threshold for nerve injury is lowered due to microvascular compromise and impaired myelin repair. We report a 48-year-old male with type 2 diabetes and chronic foot ulcers who developed immediate left foot drop following low spinal anesthesia for debridement. Electrophysiology revealed conduction block and focal demyelination of the peroneal nerve at the fibular head; high-frequency ultrasound showed no structural abnormality, while lumbar magnetic resonanc

Single-Shot Thoracic Spinal Anesthesia and Serratus Anterior Plane Block for Awake Nonintubated Thoracic Surgery: A Case Report.

Awake nonintubated thoracic surgery (NITS) is a valuable approach for patients at high risk of complications from general anesthesia. We present a case of a 67-year-old patient with cholangiocarcinoma and pulmonary metastases undergoing left lingular wedge resection. Given significant hepatic impairment, single-shot thoracic spinal anesthesia (TSA) and a serratus anterior plane block (SAPB) were used to ensure intraoperative and postoperative analgesia. The procedure was well tolerated, demonstrating TSA's potential advantages in minor thoracic surgery. This fusion approach may represent a saf

Thoracic spinal anesthesia for laparoscopic peritoneal dialysis catheter placement in older high-risk end-stage kidney disease patients.

Peritoneal dialysis (PD) catheter placement is considered a controversial procedure in patients with a history of abdominal surgeries or peritonitis. In these subjects, video laparoscopic (VLS)-assisted placement under general anesthesia (GA) is the gold standard procedure. However, older multimorbid patients are at high risk for complications in GA. In our opinion, thoracic spinal anesthesia (TSA) instead of GA could also be used in older multimorbid patients undergoing PD. Here, we report five cases of older multimorbid end-stage kidney disease (ESKD) patients aged 79.6 ± 3.5 years with a hi

Left-lateral position versus phenylephrine prophylactic treatment for hypotension following combined spinal epidural anesthesia during elective cesarean section.

BACKGROUD: Hypotension is one of the most common and dangerous complications following combined spinal epidural anesthesia during elective cesarean delivery. Many methods are used to prevent or treat the hypotension with pharmacological or non-pharmacological measures. Our aim was to assess left-lateral position and phenylephrine prophylactic treatment for the prevention or treatment of maternal hypotension. METHODS: A total of 127 pregnant women were enrolled to be analyzed. The primary outcome measure was the incidence of maternal hypotension and secondary outcome measures included maternal
