Exercise Rehabilitation for Heart Failure and Associated Cardiomyopathies.

PURPOSE OF REVIEW: To summarize and provide insight into the role of exercise rehabilitation for heart failure, cardiomyopathies and associated conditions. We provide an overview of the evolution of exercise training from “bed rest” to current guidelines and highlight emerging approaches. RECENT FINDINGS: Exercise training appears to be safe and provide benefits for patients with heart failure. Emerging evidence also suggests potential benefit in hypertrophic cardiomyopathy as well as angina with non-obstructive coronary arteries, though more data are needed for widespread implementation. Enhancing Cardiopulmonary Exercise Testing in Complex Congenital Heart Disease: The Role of Spirometry, Body Composition, and Handgrip Strength.

Cardiopulmonary exercise testing (CPET) is a cornerstone of functional assessment in congenital heart disease (CHD), yet there are additional important contributors to exercise limitation that are not directly measured by CPET. This narrative review highlights the complementary utility of spirometry, body composition analysis, and evaluation of handgrip strength in augmenting our understanding of functional capacity in CHD. Spirometry provides insights into restrictive lung disease patterns prevalent in post-surgical CHD populations; body composition analysis, especially lean mass assessment,

Pulmonary vasodilators and exercise in Fontan circulation: a systematic review and meta-analysis.

OBJECTIVE: In Fontan circulation, pulmonary arterial hypertension (PAH)-targeted therapies could improve the patients' exercise capacity. This study aimed to investigate the effects of PAH agents on different exercise parameters in stable Fontan patients by synthesising evidence of randomised controlled trials (RCTs). METHODS: A systematic search of PubMed, Cochrane Central Register of Controlled Trials and Web of Science databases, as well as of ClinicalTrials.gov, was performed. Primary outcomes were specific cardiopulmonary exercise test parameters: peak oxygen uptake (peak VO2), peak hear

Abnormal heart rate responses to exercise in non-severe COPD: relationship with pulmonary vascular volume and ventilatory efficiency.

BACKGROUND: Despite being a prognostic predictor, cardiac autonomic dysfunction (AD) has not been well investigated in chronic obstructive pulmonary disease (COPD). We aimed to characterise computed tomography (CT), spirometry, and cardiopulmonary exercise test (CPET) features of COPD patients with cardiac AD and the association of AD with CT-derived vascular and CPET-derived ventilatory efficiency metrics. METHODS: This observational cohort study included stable, non-severe COPD patients. They underwent clinical evaluation, spirometry, CPET, and CT. Cardiac AD was determined based on abnorma

Physiological underpinnings of exertional dyspnoea in mild fibrosing interstitial lung disease.

The functional disturbances driving "out-of-proportion" dyspnoea in patients with fibrosing interstitial lung disease (f-ILD) showing only mild restrictive abnormalities remain poorly understood. Eighteen patients (10 with idiopathic pulmonary fibrosis) showing preserved spirometry and mildly reduced total lung capacity (≥70% predicted) and 18 controls underwent an incremental cardiopulmonary exercise test with measurements of operating lung volumes and Borg dyspnoea scores. Patients' lower exercise tolerance was associated with higher ventilation (V̇E)/carbon dioxide (V̇CO2) compared with con…

Evaluation of the relationship of tricuspid regurgitation peak gradient/tricuspid annulus plane systolic excursion to exercise capacity, cardiac index, and ventilatory function during exercise in patients with COPD.

BACKGROUND: Chronic obstructive pulmonary disease (COPD) often causes cardiopulmonary dysfunction, which deteriorates exercise capacity. Cardiopulmonary exercise testing (CPET) and echocardiography are common tools for evaluating cardiovascular function. No studies have analyzed the correlation between echocardiography-derived parameters and cardiopulmonary response during exercise. OBJECTIVES: We analyzed the correlation between echocardiographic parameters such as tricuspid regurgitation peak gradient (TRPG), tricuspid annular plane systolic excursion (TAPSE), TRPG/TAPSE and CPET-derived pa

Effects of Supervised Exercise Training on Atrial Fibrillation: A META-ANALYSIS OF RANDOMIZED CONTROLLED TRIALS.

PURPOSE: Atrial fibrillation (AF) is associated with the comorbidities of a sedentary lifestyle. Endurance athletes also show an increased incidence of AF. The role of exercise in the treatment of AF is unknown so this study aimed to examine the effects of supervised exercise on AF. METHODS: A meta-analysis of randomized controlled trials (RCTs) examining supervised exercise training in participants with AF was performed. The primary outcome was AF recurrence and burden. Secondary outcomes included AF symptoms, quality of life, and cardiorespiratory fitness (CRF). RESULTS: Thirteen RCTs, inv

The role of invasive cardiopulmonary exercise testing in patients with unexplained dyspnea: a systemic review.

BACKGROUND: Dyspnoea is a common complaint that often remains unexplained with no diagnosis and poor management despite extensive, repetitive and costly testing. Invasive cardiopulmonary testing has been used in the evaluation of dyspnoea, however, its role is not yet well defined. We sought to perform a systematic review of the literature looking at the role of invasive cardiopulmonary testing in the evaluation of chronic dyspnoea and/or exercise intolerance. METHODS AND RESULTS: We performed a literature review in accordance with PRISMA, analysing articles published in peer-reviewed journal

Salutary Acute Effects of Exercise on Central Hemodynamics in Heart Failure With Preserved Ejection Fraction.

BACKGROUND: A warmup period of priming exercise has been shown to improve peripheral oxygen transport in older adults. We sought to determine the acute effects of priming exercise on central hemodynamics at rest and during a repeat exercise in heart failure with preserved ejection fraction (HFpEF). METHODS AND RESULTS: This is a post hoc analysis from 3 studies. Patients with HFpEF (n = 42) underwent cardiac catheterization with simultaneous expired gas analysis at rest and during exercise (20 W for 5 minutes, priming exercise). Measurements were then repeated at rest and during a second bout

Reduced exercise capacity in patients with systemic sclerosis is associated with lower peak tissue oxygen extraction: a cardiovascular magnetic resonance-augmented cardiopulmonary exercise study.

BACKGROUND: Exercise intolerance in systemic sclerosis (SSc) is typically attributed to cardiopulmonary limitations. However, problems with skeletal muscle oxygen extraction have not been fully investigated. This study used cardiovascular magnetic resonance (CMR)-augmented cardiopulmonary exercise testing (CMR-CPET) to simultaneously measure oxygen consumption and cardiac output. This allowed calculation of arteriovenous oxygen content gradient, a recognized marker of oxygen extraction. We performed CMR-CPET in 4 groups: systemic sclerosis (SSc); systemic sclerosis-associated pulmonary arteria

Sports practice predicts better functional capacity in children and adults with Fontan circulation.

BACKGROUND: Peak oxygen uptake (peak VO2) and its decline predict death or serious cardiovascular adverse events in patients with Fontan circulation. Our aim was to study VO2 in a population of Fontan patients with variable age and contemporary surgical correction, and find predictors of functional status which could lead to management changes. METHODS: Prospective cardiopulmonary exercise test was performed on a treadmill. Blood tests and transthoracic echocardiogram were performed on the same day. Dependent variables were defined as: VO2 at ventilatory threshold (VT) as a percentage of the

Maintaining the benefits following pulmonary rehabilitation: Achievable or not?

Pulmonary rehabilitation (PR) improves exercise capacity, health-related quality of life (HRQoL) and dyspnoea in patients with COPD and other lung conditions. Once PR is completed, the benefits gained begin to decline unless patients continue to exercise regularly. Due to limited evidence in other lung conditions, this review aims to examine the current evidence regarding maintenance exercise programmes for patients with COPD and to determine the types of programmes that are able to maintain the benefits gained from PR to 12 months and longer. A number of factors may affect the ability to main

The effects of 8-week water-running program on exercise capacity in children with juvenile idiopathic arthritis: a controlled trial.

Exercise capacity has been reported to be lower in children with Juvenile Idiopathic Arthritis (JIA). Therefore, the aim was to investigate the effects of an 8-week water-based exercise program on exercise capacity in children with JIA. Forty-two children with JIA were divided into two groups as: exercise group [n = 21, water-running, moderate-intensity exercise (60-70%), two times/week], and control group (n = 21, no additional treatment other than the prescribed medication). All children were assessed at baseline and post-intervention in terms of physical and disease-related characteristics,

Whole body vibration training in patients with COPD: A systematic review.

In recent years, several studies have shown that whole body vibration training (WBVT) may be a beneficial training mode in a variety of chronic diseases and conditions such as osteoporosis, fibromyalgia, multiple sclerosis, or chronic low back pain. However, a systematic review on the effects of WBVT in patients with chronic obstructive pulmonary disease (COPD) has not been performed yet. An extensive literature search was performed using various electronic databases (PubMed, Embase, LILACS, and PEDro). They were searched from inception until September 20, 2014, using key words like "COPD" and

Exercise training and rehabilitation in pulmonary arterial hypertension: rationale and current data evaluation.

Pulmonary arterial hypertension (PAH) is characterized by reduced functional capacity and health-related quality of life. Despite the progress made in recent years in disease-targeted therapies with improvement in prognosis, PAH patients often experience progressive exercise intolerance. Exercise rehabilitation is beneficial and has become a standard of care in patients with systolic heart failure and chronic obstructive pulmonary disease. However, although PAH patients present with similar exertional limitation and often share common hemodynamic, ventilatory, and skeletal muscle derangements,

Fitter Fontans for future-Impact of physical exercise on cardiopulmonary function in Fontan patients.

BACKGROUND: In Fontans, exercise tolerance is poorer compared to their healthy peers. Higher V ˙ O 2 p e a k represents a strong predictor for mortality and morbidity in these patients. Cardiac rehabilitation programs have been shown to improve cardiopulmonary function in Fontans. More habitual physical activity should therefore lead to a better exercise tolerance. METHODS: We performed cardiopulmonary exercise testing in 24 Fontan patients who had engaged in physical activity for a minimum of 3 h per week over their lifetime. As a control we performed cardiopulmonary exercise testing in 20…

Physical fitness and cardiovascular regulation: mechanisms of orthostatic intolerance.

We studied three groups of eight men each--high, mid, and low fit (peak O2 consumption 60.0 +/- 0.8, 48.9 +/- 1.0, and 35.7 +/- 0.9 ml.min-1.kg-1)--to determine the mechanism of orthostatic intolerance in endurance athletes. Tolerance was defined by progressive lower body negative pressure (LBNP) to presyncope. Maximal calf vascular conductance (Gmax) was measured. The carotid baroreflex was characterized using both stepwise R-wave-triggered and sustained (2 min) changes in neck chamber pressure. High-fit subjects tended to have lower LBNP tolerance than mid- and low-fit subjects but similar b

Acute effects of fan therapy on exercise-induced breathlessness in healthy adults and people living with chronic respiratory disease: A systematic review.

BACKGROUND: Exercise-induced breathlessness is a common complaint in the general population and a highly prevalent symptom in adults living with chronic diseases, often acting as an important barrier to engaging in physical activity. Fan therapy can reduce breathlessness at rest, yet its acute effects during exercise have not been systematically explored. METHODS: A literature search encompassing three electronic databases (PubMed, Scopus, Embase) from inception to January 2025 was conducted to identify randomized controlled trials comparing fan therapy during and after an acute exercise bout
