Dupuytren's disease: controversial aspects of management.

Surgical literature is replete with descriptions of Dupuytren's contracture and the various operations which surgeons have utilized to corre … al observations, meager as they may be. Prejudice for surgical biology as a means of enlightenment, intuition, and, at times, empirical reasoning has been admitted in an attempt to stimulate imagination. The rest is left to the reader.

Hot Electron-Informed Quantum Plasmonics in Nanoparticle-on-Mirror Nanocavities.

We develop a novel approach to ultrafast optical modulation of quantum-mechanical phenomena at the interface of plasmonic metals. Focusing on efficient and versatile nanoparticle-on-mirror plasmonic nanocavities, we discuss indirect control of plasmonic properties through laser-induced hot electron injection. Overcoming the limitations precluding the observations of laser-driven mesoscopic phenomena in the time domain with state-of-the-art amplified sources, our proposed experimental approach can be readily realized without irreversible optical damage and holds immense potential for the future

Engaging Nephrologists in "Green Kidney" Conversations: A Technology-Driven Endeavour.

BACKGROUND: Kidney replacement therapies (KRTs) such as dialysis have a significant environmental impact, yet clinician engagement with sustainable kidney care remains limited. Our objective was to develop a mobile phone application to raise awareness of clinicians about Green Nephrology. METHODS: We developed and piloted a web-based mobile application designed to raise awareness of "Green Nephrology" through short, interactive information modules. The app was introduced at the World Congress of Nephrology 2025. It presented users with real-world dilemmas and evidence-based facts about the en

SparStVR - exploring sparse 3D histology data in virtual reality.

Visualization of three-dimensional image stacks from serial sectioned histology or from other modalities poses a challenge, and solutions are often limited to projections or to rotated views of the 3D stacks. Here, we present SparStVR, a virtual reality application for interactive 3D image data visualization. A virtual environment with textured 3D models and, when desired, embedded measurements is generated from a stack of labeled masks and stacks of color images. In addition, modeling whole sample from sample masks and embedding multimodal measurements from heatmaps are supported. The possibi

Solvation Lies Within: Simulating Condensed-Phase Properties from Local Electronic Structures.

In transitions between different environmental settings, a molecular system inevitably undergoes a range of detectable changes, and the ability to accurately simulate such responses, e.g., in the form of shifts to molecular energies, remains an important challenge across physical chemistry. Based on an exact decomposition of total energies from Kohn-Sham density functional theory in a basis of spatially localized molecular orbitals, the present work outlines a robust protocol for sampling the effect of solvation within homogeneous condensed phases by focusing on perturbations to local electron

Views of Facial Attractiveness of Faces of Individuals With and Without an Intellectual Disability.

BACKGROUND: Little is known about people with intellectual disabilities' views of facial attractiveness and and how desirable they feel they are (self-desirability). METHOD: Twenty-four adults with intellectual disabilities and twenty-five adults without disabilities were recruited from further education colleges and voluntary community organisations. Participants were asked to rate the attractiveness of facial images of typically developing individuals and individuals with Down syndrome. This was followed by a semi-structured interview, exploring participants with intellectual disabilities'

A personal overview of epidemic models for mosquito-borne infections.

In this paper, we present a brief, nonexhaustive overview of the features we consider most relevant in models for mosquito-transmitted infections, comparing them with the corresponding features in models for directly-transmitted infections. In particular, we focus on the basic reproduction number $ \mathcal{R}_0 $ and its relations with the exponential growth rate, the probability of an outbreak, and the final size of the epidemic. The concept of a threshold for epidemic spread based on the basic reproduction number ($ \mathcal{R}_0 > 1 $) was first introduced by Ronald Ross for malaria. Later

Haptic Feedback Systems for Lower-Limb Prosthetic Applications: A Review of System Design, User Experience, and Clinical Insights.

Systems presenting haptic information have emerged as an important technological advance in assisting individuals with sensory impairments or amputations, where the aim is to enhance sensory perception or provide sensory substitution through tactile feedback. These systems provide information on limb positioning, environmental interactions, and gait events, significantly improving mobility in amputees and their confidence about using such devices. This review summarizes recent progress in haptic feedback systems by providing a comparative analysis of different feedback approaches, evaluating t

Robotic retroperitoneal tumor resection: a video vignette.

The utility of robotic platform remains relatively unexplored in the context of retroperitoneal tumors. The location and complexity of neurovascular anatomy pose additional challenges for surgical planning. Herein, we present the case of a 56-year-old male with a 9 × 7-cm retroperitoneal tumor located within the right iliopsoas muscle, without intraforaminal extent. The surgery was performed using the da Vinci Xi Surgical System (Intuitive Surgical, Inc.). The patient was placed in the left lateral position. Intraoperative and postoperative recovery was uneventful. A transient sensory deficit

A Flexible Clinician-Friendly Interface for Upper Limb Rehabilitation with Industrial Robots.

This paper presents a flexible software with a Graphical User Interface (GUI) designed to assist clinicians in upper limb robotic rehabilitation sessions. The flexibility comes from the fact that, since it relies on the Robot Operating System 2 (ROS2) framework, it is potentially compatible with any collaborative industrial robot, with relatively low integration effort. The software collects and analyzes data related to each session to identify critical movement directions for each patient, detecting improvements or regressions through statistical tests, thus reducing subjectivity. Moreover, i

Recognition of Unseen Combined Motions via Convex Combination-based EMG Pattern Synthesis for Myoelectric Control.

Electromyogram (EMG) signals recorded from the skin surface enable intuitive control of assistive devices such as prosthetic limbs. However, in EMG-based motion recognition, collecting comprehensive training data for all target motions remains challenging, particularly for complex combined motions. This paper proposes a method to efficiently recognize combined motions using synthetic EMG data generated through convex combinations of basic motion patterns. Instead of measuring all possible combined motions, the proposed method utilizes measured basic motion data along with synthetically combine

A Novel Protocol for Feasibility and Safety in Early Discharge with ACS.

Acute coronary syndrome is the primary reason for admission to cardiology services and involves significant resource consumption. Currently, low-risk patients are recommended for discharge within the first 48-72 h, enabling early initiation of cardiac rehabilitation with proper follow-up. This is crucial not only as an efficiency marker but also because current evidence has shown it to be a safe and feasible strategy. Thus, this work aims to provide a comprehensive review of the current evidence on early discharge in acute coronary syndrome and propose a new, intuitive, objective, and easy-to-

Pre-hospital LVO detection: One size does not fit all.

A range of techniques are being developed with the goal of leverage physiologic data to detect large vessel occlusion (LVO) in the prehospital setting. The output of most every technique is a continuous (or ordinal) variable, but a threshold is applied to yield a simplified binary outcome (i.e., high-likelihood LVO vs low-likelihood LVO). The mathematically optimal threshold is typically selected to balance sensitivity and specificity. Though intuitive, this approach may not appropriately account for the relative significance of false negatives and false positives. Most importantly, relying on

An anti-Hebbian model for binocular visual plasticity and its attentional modulation.

Monocular deprivation during the critical period impairs the cortical structure and visual function of the deprived eye. Conversely, transient occlusion of one eye in adults enhances the predominance of that eye. This counter-intuitive effect of short-term monocular deprivation is a form of homeostatic plasticity. However, whether this sensory plasticity requires attention, and the underlying neural mechanisms remain unclear. Here, through a psychophysical experiment, we demonstrate that the deprivation effect is dramatically attenuated in the absence of attention. We develop a neural computat

Bayesian Gower agreement for categorical data.

In this work I present two methods for measuring agreement in nominal and ordinal data. The measures, which employ Gower-type distances, are simple, intuitive, and easy to compute for any number of units and any number of coders. Influential units and/or coders are easily identified. I consider both one-way and two-way random sampling designs, and develop an approach to Bayesian inference for each. I apply the methods to simulated data and to two real datasets, the first from a one-way radiological study of congenital diaphragmatic hernia, and the second from a two-way study of psychiatric dia

Gesture encoding in human left precentral gyrus neuronal ensembles.

Understanding the cortical activity patterns driving dexterous upper limb motion has the potential to benefit a broad clinical population living with limited mobility through the development of novel brain-computer interface (BCI) technology. The present study examines the activity of ensembles of motor cortical neurons recorded using microelectrode arrays in the dominant hemisphere of two BrainGate clinical trial participants with cervical spinal cord injury as they attempted to perform a set of 48 different hand gestures. Although each participant displayed a unique organization of their res

Decoy cells detected in the urine of a patient with complex karyotype Myelodysplastic neoplasms who underwent umbilical cord blood transplantation: a case report.

BACKGROUND: Currently, few literature reports document cases of decoy cells in the urine of umbilical cord blood transplant patients. The majority of the literature indicates that decoy cells are frequently identified in the urine of kidney transplant recipients. CASE PRESENTATION: This case report describes a patient with Myelodysplastic Neoplasms featuring a complex karyotype who underwent umbilical cord blood transplantation. Postoperative urinary cytology revealed decoy cells, and subsequent BK virus nucleic acid testing was positive. However, the routine use of antiviral drugs by the phy

Grasp Force Assistance via Throttle-based Wrist Angle Control on a Robotic Hand Orthosis for C6-C7 Spinal Cord Injury.

Individuals with hand paralysis resulting from C6-C7 spinal cord injuries frequently rely on tenodesis for grasping. However, tenodesis generates limited grasping force and demands constant exertion to maintain a grasp, leading to fatigue and sometimes pain. We introduce the MyHand-SCI, a wearable robot that provides grasping assistance through motorized exotendons. Our user-driven device enables independent, ipsilateral operation via a novel Throttle-based Wrist Angle control method, which allows users to maintain grasps without continued wrist extension. A pilot case study with a person with

CE: Overcoming Movement-Evoked Pain to Facilitate Postoperative Recovery.

Severe postoperative movement-evoked pain (MEP) can be immobilizing, instilling in patients the fear that further activity will produce unbearable pain. This impedes healing and restoration of function while also extending time to recovery. Therefore, it is critical to manage MEP effectively through timely evaluation and comprehensive care planning. This article builds on recent calls to standardize testing of MEP to inform care planning in a way that both reduces pain and improves functioning. Subsequent reassessment of MEP can guide the refinement of therapy. Although this approach may seem
