Bile duct obstruction is associated with early postoperative upregulation of liver uncoupling protein-2 and reduced circulating glucose concentration in the rat.

OBJECTIVE: To evaluate whether upregulation of liver and muscle uncoupling protein 2 (UCP-2) is an acute phenomenon in obstructive jaundice and associated with secondary metabolic effects. METHODS: Male Sprague-Dawley rats were divided into four groups: bile duct ligated (BDL) and sham-operated pair-fed (PF), ad libitum fed (AL), and controls. BDL, PF, and AL rats were further divided into subgroups according to the interval postoperatively when they were reanesthetized and sampled for tissue and blood: 2, 4, and 8 d, respectively. Bilirubin, liver enzymes, glucose, free fatty acids, and insu

Choledochotomy for calculous disease in the elderly.

To determine the risk of operations on the biliary tract in the elderly, we retrospectively examined 56 patients 80 years of age or older undergoing choledochotomy for calculous disease and compared their outcomes with those of a contemporary group of 257 patients undergoing similar surgery. Thirty of 56 elderly patients (54%) required urgent surgery for acute cholangitis, acute cholecystitis, or obstructive jaundice (serum bilirubin greater than 5 mg/dL), compared with 97 of 257 younger patients (38%) (p = 0.029). Mortality in elderly patients was significantly higher (5 of 56) than in younge

The activity of ornithine transcarbamoylase and arginase during mechanical jaundice in the rat model.

BACKGROUND: The pathogenesis of morbidity and mortality associated with intervention of the biliary system in patients with obstructive jaundice is unknown. Mechanical jaundice initiates the development of morphological changes in hepatocytes with concomitant disturbances in metabolism. These are followed by changes in enzyme activity in hepatocytes and peripheral blood. MATERIAL AND METHODS: Wistar rats were divided into three groups: group 1, sham-operated controls; group 2, rats with permanent jaundice; and group 3, rats with temporary mechanical jaundice. The animals were examined at 2 we

Endoscopic diagnosis of squamous cell carcinoma of the pancreas invading the stomach.

CONTEXT: Squamous cell carcinoma of the pancreas is an unusual tumor due to the absence of squamous cells in the normal pancreas. Its clinical presentation is similar to that of adenocarcinoma of the pancreas and is usually diagnosed intraoperatively or at autopsy. CASE REPORT: We report a 50-year-old male who presented with obstructive jaundice and symptoms suggestive of gastric outlet obstruction. Contrast-enhanced computerized tomography revealed a heterogeneous enhancing mass in the head of pancreas and upper gastrointestinal endoscopy revealed an ulcerated polypoidal lesion in the stomac

Comparison of CT scanning and radionuclide imaging in liver disease.

Early experience with body CT suggested its usefulness in many diagnostic problems; jaundice, renal and pancreatic masses, and in the evaluation of relatively inaccessible parts of the body, such as the retroperitineum, mediastinum, and pelvis. Investigation of hepatic disease by CT was not unexpectedly compared to radionuclide liver scanning, the major preexisting modality for imaging the liver. In the evaluation of the jaundiced patient, CT rapidly assumed a major role, providing more specific information about the liver than the RN liver scan, as well as demonstrating adjacent organs. CT di

Common bile duct stent-stone complex: the silent threat of forgotten biliary stents-case series.

Choledocholithiasis, a frequent cause of biliary obstruction, occurs in 10%-15% of patients with gallstones. However, a stent-stone complex around a biliary stent is an extremely rare complication. Common biliary duct stent-stone complexes can be managed endoscopically or surgically. The authors report two rare cases of forgotten common bile duct stent-stone complexes presenting with acute cholangitis and obstructive jaundice, both of which necessitated surgical intervention. A stent-stone complex is a rare long-term complication of forgotten biliary stenting. Patients remain asymptomatic or p

Complete migration of a fully covered self-expandable metallic stent after endoscopic ultrasound-guided hepaticogastrostomy: A case report.

A male patient underwent gastrojejunal bypass surgery in 2017. In 2020, he was referred to our hospital for suspected obstructive jaundice. Subsequently, he was diagnosed with cholangiocarcinoma, and endoscopic retrograde cholangiopancreatography was attempted via balloon-assisted enteroscopy. However, the endoscope did not reach the duodenal papilla owing to the abdomen-small intestine adhesion. Therefore, endoscopic ultrasound-guided hepaticogastrostomy was performed using a dedicated plastic stent. After stent placement, obstructive jaundice and cholangitis promptly improved. However, we re

Neonatal jaundice is associated with increased risks of congenital anomalies of the kidney and urinary tract and concomitant urinary tract infection.

The link between neonatal jaundice and urinary tract infection (UTI) remains debated, with congenital kidney and urinary tract anomalies (CAKUT) potentially playing a role. This population-based study aimed to analyze the correlations between neonatal jaundice, CAKUT, and concomitant UTI. The study cohort consisted of 2,078,122 live births from 2004 to 2014. We linked several population-based datasets in Taiwan to identify infants with unexplained neonatal jaundice and their mothers. The primary outcome was the rate of CAKUT occurring within 3 years after delivery, and the presence of concomit

[Percutaneous transhepatic biliary drainage].

The authors describe their experience in 23 cases of biliary tract drainage by the transhepatic-percutaneous approach in the course of obstructive jaundice of diversified origin. This can be done for essentially three reasons, namely to alleviate jaundice preoperatively, to provide permanent bile drainage in patients not amenable to surgery, and to relieve excess pressure in surgical anastomoses of the biliary passages. On the basis of biological considerations (relationship between severity and duration of cholestasis on the one hand and postoperative mortality and morbidity on the other), an

Hemophagocytic lymphohistiocytosis with jaundice as first manifestation: A case report.

BACKGROUND: Hemophagocytic lymphohistiocytosis (HLH) is a rare but life-threatening condition. It is an immune-mediated disease that has a wide range of causes, elicits a hyperinflammatory response, and results in multiple organ damage. Clinical presentations vary, and in some cases, jaundice occurs as the first symptom. CASE SUMMARY: We report the case of a 71-year-old female patient who presented with jaundice. She was admitted to our hospital because of the occurrence of "jaundice for half a month", and upon examination, obstructive jaundice with choledocholithiasis and gallstones was sugg

"Evaluation of hepatobiliary disorders using magnetic resonance cholangiopancreatography in a low-resource setting in Kano, Nigeria: a retrospective study".

BACKGROUND: Biliary disorders are a major health concern in Nigeria, where magnetic resonance cholangiopancreatography (MRCP), a non-invasive imaging tool offering effective duct visualization without radiation, is emerging as a key diagnostic modality. This retrospective study in Kano explored its role in diagnosing biliary and pancreatic disorders. MATERIALS AND METHODS: This retrospective study reviewed MRCP images and records of patients with suspected biliary disorders at Aminu Kano Teaching Hospital (AKTH), Kano, Nigeria. A 1.5T magnetic resonance imaging (MRI) machine obtained MRCPs fo

Mucin-filled CBD, difficult to manage cholangitis.

Intraductal papillary mucinous neoplasms (IPMNs) are mucin-secreting cystic neoplasm of pancreas. They have a malignant potential. They are usually localised to the pancreas but occasionally can involve surrounding structures (1.9%-6.6%), like bile duct and duodenum, and are labelled as IPMN with invasion. Jaundice as a manifestation of IPMN is not common (4.5%). It can present as jaundice as a result of invasion of common bile duct (CBD) resulting in stricture formation or uncommonly as a result of fistulising to CBD with resultant obstruction of CBD by thick mucin secreted by this tumour. As

Diagnostic approach to the patient with jaundice.

Jaundice is caused by many disease processes ranging from benign to life threatening. History and physical examination remain important tools in evaluating the etiology of jaundice. The conjugation state of bilirubin, along with other laboratory tests judiciously ordered, can guide the provider toward category of illness. Hyperbilirubinemia may be categorized as to its etiology: unconjugated/prehepatic, intrahepatic, or extrahepatic/obstructive. Referral should be considered when likelihood of malignancy, chronic autoimmune condition, or need for intervention exists.

Extrahepatic biliary atresia associated with trisomy 18.

A case of extrahepatic biliary atresia (EBA) associated with trisomy 18 is presented. A 1-month-old boy was suspected to have Alagille syndrome with obstructive jaundice, a systolic heart murmur, growth retardation, and a small, pointed chin. However, surgery and chromosomal analysis revealed EBA associated with trisomy 18. Chromosomal examination must be performed in patients with jaundice and congenital anomalies. It is possible that EBA in trisomy 18 syndrome is due to a chromosomal disorder.

[Cholestasis caused by a choledochal botryoid rhabdomyosarcoma in a 22-month-old boy].

Botrioid rhabdomyosarcoma of the extrahepatic bile ducts is a rare cause of jaundice in children. It has a very poor prognosis and is rarely diagnosed preoperatively. We report a choledochal botrioid rhabdomyosarcoma in a 22-month-old boy who developed an obstructive jaundice. Radiographic explorations suggested cystic lymphangioma. The gallbladder, the cystic duct, the common bile duct and the pancreatic head were resected. The diagnosis was made on pathological examination; adjuvant chemotherapy followed. The patient was disease free 20 months following treatment.

Extrahepatic bile duct neurilemmoma mimicking Klatskin tumor.

Neurilemmoma rarely develops in the biliary tree. Here, we report a 39-year-old Iranian woman with neurilemmoma in the extrahepatic bile duct presenting with progressively deepening jaundice. On the basis of clinical and radiological features, this tumor was initially suspected as Klatskin tumor. Histologically, the tumor was a typical neurilemmoma. Immunostaining showed that tumor cells were strongly and diffusely positive for S-100 protein, which supported the diagnosis of neurilemmoma. Neurilemmoma should be considered in the differential diagnosis of obstructive jaundice.

Delayed presentation of porta hepatis injury following blunt abdominal trauma.

A 73 year old lady developed abdominal pain, anaemia and obstructive jaundice 18 days after a road traffic accident. The jaundice was due to compression of the biliary confluence by a haematoma which was caused by a laceration of the left portal vein. The portal vein was repaired (lateral venorrhaphy) and post-operative recovery was uncomplicated. Porta hepatis injuries are difficult to diagnose and delayed presentation is not uncommon. Significant morbidity and mortality may ensue if aggressive management is not adopted.

[Endoscopic methods in the diagnosis and treatment of mechanical jaundice of non-neoplastic etiology].

The authors analyse the results of treatment of 255 patients with obstructive jaundice of nonneoplastic origin. Various endoscopic methods were used. Endoscopic interventions were carried out in the first two days in 79.8% of cases. Laparoscopic cholecystostomy was conducted in 138 patients; jaundice was relieved in 3 to 20 days in 95% of cases. It is shown that the best results of operative treatment of patients with acute cholecystitis complicated by choledocholithiasis are produced in concurrent use of laparoscopic cholecystostomy and endoscopic papillosphincterotomy.
