[Middle third clavicle fractures--diagnosis, complications and treatment].

Clavicle fractures compose 2.5% to 5% of all fractures, and usually occur due to a direct blow to the shoulder. The incidence of these fractures persistently increases. Most of the fractures are in the middle third--a result of the unique structure of the clavicle and its ligamentous and muscular attachments. The fracture diagnosis includes focused physical examination, radiographic evaluation, and comprehensive evaluation of the patient, considering the fact that the fracture can be the result of significant energy absorption and can hide other injuries. Usually treatment is non-operative, an

Humeral torque in professional baseball pitchers.

BACKGROUND: Spontaneous fracture of the humeral shaft in throwers is a rare but well-known phenomenon. Although it has been hypothesized that the biomechanics of the throw cause such fractures, it is not clear how or when the fractures occur in the pitching motion. METHODS: The torque acting about the long axis of the humerus was calculated in 25 professional baseball pitchers throwing in game situations. RESULTS: Peak humeral axial torque reached a mean value of 92 +/- 16 Nm near the time of maximum shoulder external rotation at the end of the cocking phase. This torque tended to externally

Upper extremity fractures in restrained front-seat occupants.

BACKGROUND: The aim of our study was to analyze the injury mechanism of upper extremity fractures in car crashes, to create a basis for developing prophylactic devices. METHODS AND RESULTS: During 1985-1995, 3,260 restrained car drivers (1,228 front-seat passengers) were injured in 9,380 crashes involving cars. A total of 179 drivers (5.5%) (front-seat passengers: n = 53, 4.3%) sustained fractures of the arm. The hand (25%), wrist (23%), and forearm (23%) were affected most often, and the elbow (9%), upper arm (10%), and shoulder (10%) were seldom affected. No considerable differences of the

Current treatment strategy for proximal humerus fractures.

Fractures of the proximal humerus present a unique challenge in orthopedic practice due to the complex anatomy and biomechanical complexity of this region. The spectrum of injuries occurring here ranges from nondisplaced stable fractures to complex displaced fractures or fracture-dislocations. Historically, the treatment of these fractures has been the subject of much debate and treatment modalities have mainly involved a conservative approach, with surgical management being relatively rare. In recent decades, there has been a paradigm shift in the treatment of proximal humerus fractures not o

Fractures of the lesser tuberosity of the humerus.

BACKGROUND: Fractures of the lesser tuberosity are rare injuries, and little is known of their epidemiology. Operative treatment is generally recommended for displaced fractures; however, the outcome of this method of treatment has not previously been studied. The aims of our study were to determine the approximate incidence of lesser tuberosity fractures, as well as the functional outcome following operative treatment in a consecutive series of patients. METHODS: Over an eight-year period, we studied the demographic details of a consecutive series of twenty-two adult patients who had a fract

Operative treatment of complex intra-articular scapular fractures: long-term functional outcomes in a single-center study.

BACKGROUND: Scapula fractures occur in the setting of high-energy trauma. Operative treatment of complex intra-articular scapular fractures can result in adequate surgical and functional outcomes. MATERIALS AND METHODS: Twenty-seven patients with complex, displaced intra-articular scapular fractures with or without involvement of the scapular body, were treated operatively in a single trauma center between 2010 and 2021. Associated injuries such as ipsilateral upper limb fractures and nerve injuries were identified in the majority of the patients. Fixation with anatomical medial and lateral b

Outcome following management of unstable lateral end clavicle fractures with locking plate and coracoid anchor augmentation.

BACKGROUND: Management of complex lateral end clavicle fractures with coraco-clavicular ligament disruption can be challenging. METHODS: We prospectively analysed 19 (17 M:2F) patients from January 2014 to June 2016. Six patients had intra-articular fractures (Edinburgh-3B2) and the remaining were extra-articular (3B1). All patients had open reduction internal fixation with lateral end locking plate augmented with a coracoid anchor. All patients were evaluated at the final follow-up by American Shoulder and Elbow Surgeon score (ASES), Disabilities of the Arm, Shoulder and Hand (DASH), Oxford

Risk factors associated with secondary displacement in fractures of the humeral greater tuberosity.

BACKGROUND: The incidence of secondary displacement in fractures of the greater tuberosity of the humerus remains high, irrespective of whether conservative or surgical treatment is administered. However, the specific risk factors contributing to secondary displacement of the greater tuberosity of the humerus have not been previously reported. The primary objective of this study was to analyze the risk factors associated with secondary displacement of the greater tuberosity of the humerus and to summarize corresponding guidelines for clinical diagnosis and treatment. METHODS: A retrospective

Stemless vs. stemmed reverse total shoulder arthroplasty as primary treatment in the elderly: A meta-analysis.

BACKGROUND: Reverse total shoulder arthroplasty (RTSA) is widely used for cuff-deficient and degenerative shoulder conditions in older adults. While stemmed implants offer long-term success, stemless designs have gained attention due to bone preservation and ease of revision, but evidence remains limited. METHODS: A systematic search was performed in PubMed, Scopus, Cochrane Library, and Google Scholar till June 2025. Five studies met the inclusion criteria. Key outcomes included satisfaction, Visual Analog Scale for pain, functional scores, shoulder range of motion (ROM), fracture-related co

No Association Between Growth Hormone Therapy and Upper Extremity Physeal Tension Injuries: A Matched Case-control Study.

BACKGROUND: Recent evidence has identified a strong association between growth hormone therapy and physeal injuries in the lower extremity; however, few studies have investigated this association in the upper extremity. QUESTIONS/PURPOSES: (1) Do pediatric patients with physeal tension injuries of the shoulder and elbow have higher odds of having exposure to recombinant growth hormone therapy than matched controls? (2) Are the odds of having exposure to recombinant growth hormone therapy in physeal tension injuries different when stratified by shoulder and elbow injuries? METHODS: Using a ma

[The radiographic diagnosis of posterior dislocation of the shoulder (author's transl)].

In spite of its clinical features, the diagnosis of posterior dislocation of the shoulder is often missed. The authors considered that this is due to an inadequate analysis of the x-rays. They show that the diagnosis can be made on an antero-posterior x-ray and describe the findings in fourteen cases. They note a modification of the gleno-humera line and the features of the associated impaction fracture. A lateral view is necessary to make the diagnosis certain, an axillary view being preferred to a trans-thoracic late-femoral head or of the posterior glenoid margin.
