[Proximal humeral fracture complicated by axillary artery lesion--a case review].

Although proximal humeral fractures are common diagnosis in emergency, cases of axillary artery injuries are rare. Authors present a patient, who underwent high energy trauma during a traffic accident like a pedestrian. Comminuted proximal humeral fracture was complicated with an occlusion of the third part of the axillary artery. There was necessary an urgent surgery, ostesynthesis with locking plate and reconstruction of the axillary artery by interposition of expanded polytetrafluoroethylene (ePTFE) graft. Postoperative course was uncomplicated, patient had no symptoms of ischaemia of extre

Decision making in glenohumeral arthroplasty.

Prosthetic replacement arthroplasty for glenohumeral arthritis is a well-developed and well-described technique with good and excellent results. The surgeon is faced with many decisions to make, however, regarding choice of implant, implant fixation, soft tissue management, and options for glenoid resurfacing. In general, when the precise cause of the arthritic condition is identified, the choices become more straightforward. For advanced osteoarthritis of the shoulder joint in an older patient with asymmetric posterior erosion of the glenoid, a total shoulder arthroplasty renders the best rel

Late vascular complication after fracture of the proximal humerus.

A case of late vascular complication after a fracture of the proximal humerus is presented. The main clinical feature was neurological loss of the brachial plexus, while angiography showed no rupture or false aneurysm. The long delay before surgical intervention caused irreversible damage to the nerves. Early diagnosis and surgical intervention are emphasized.

Outcomes following revision of the revision total elbow arthroplasty.

BACKGROUND: There is very little information in the literature on the outcomes of revision of revision total elbow arthroplasty (RRTEA). Our aim was to report the outcomes of this rarely performed procedure. METHODS: We retrospectively identified all patients who had undergone RRTEA between 2007 and 2016. Outcomes were assessed clinically using a number of validated systems, and radiographs were reviewed for prosthesis alignment, cementation by Morrey grading, and heterotopic ossification. RESULTS: We identified 22 patients who underwent RRTEA. Of these patients, 14 were available for assess

Arthroscopic and open shoulder stabilization using absorbable implants. A clinical and radiographic comparison of two methods.

The aim of this study was to compare the clinical and radiographic results in patients with recurrent unidirectional, post-traumatic shoulder instability (dislocations/subluxations). All the patients had a Bankart lesion and underwent reconstruction using either an open or an arthroscopic technique and absorbable implants. Thirty-three consecutive patients (36 shoulders) were operated on by one surgeon. Group A comprised 18 shoulders which underwent an open Bankart reconstruction using absorbable 3.7-mm TAG suture anchors. Group B comprised 18 shoulders which underwent a combination of an intr

A new locking plate for unstable fractures of the proximal humerus.

UNLABELLED: In a prospective study, 28 patients with 29 proximal humeral fractures were treated with the Locking Proximal Humerus Plate from 2001-2002. Most of these fractures were complex, Type B (n = 15) and Type C (n = 9) fractures, according to the AO classification. Followups included radiographs, clinical examinations, and recording Constant scores. After 1 year, the average Constant score for all fractures was 74.6 (range, 37-96). For Type A fractures, it was 82.6 (range, 60-96), for Type B it was 78.3 (range, 37-95), and for Type C it was 64.6 (range, 40-91). Complications related to t

Three-dimensional Volumetric Filling Ratio Predicts Stress Shielding in Short-stem Anatomic Total Shoulder Arthroplasty.

INTRODUCTION: Three-dimensional (3-D) CT volumetric filling ratio (VFR) is a better predictor of proximal humerus stress shielding after short-stem anatomic total shoulder arthroplasty (aTSA) than using plain radiographs. METHODS: Forty-four patients with short-stem aTSAs, preoperative CT scans, and a minimum 3-year radiographic follow-up were included. Patients were divided into group A (stress shielding) and group B (no stress shielding) based on the radiographic analysis. Standard implant filling ratios were measured on plain radiographs. The 3-D VFR of the metaphyseal and diaphyseal segme

Reverse total shoulder arthroplasty for the management of fractures of the proximal humerus: a systematic review.

Reverse total shoulder arthroplasty (RTSA) represents a good solution for the management of patients with fracture of the proximal humerus with associated severe osteoporosis and RC dysfunction. A systematic review of the literature according to the PRISMA guidelines was performed matching the following keywords: "reverse total shoulder arthroplasty"; "reverse total shoulder prostheses"; "fractures"; "fracture of the proximal humerus." Medline, EMBASE, Google Scholar, and Ovid database have been screened. Ten studies were considered in the qualitative analysis. No randomized prospective contro

What is the concordance rate of preoperative synovial fluid aspiration and intraoperative biopsy in detecting periprosthetic joint infection of the shoulder?

BACKGROUND: The accuracy of preoperative synovial fluid culture for microbe detection in shoulder periprosthetic joint infection (PJI) is poorly described. To evaluate the utility of preoperative culture data for early pathogen identification for shoulder PJI, we determined the concordance between preoperative synovial fluid culture results and intraoperative tissue culture results. METHODS: Fifty patients who met the 2014 Musculoskeletal Infection Society criteria for shoulder PJI between January 2016 and December 2019 were retrospectively reviewed for clinical and demographic data. Conservative treatment, plate fixation, or prosthesis for proximal humeral fracture. A prospective randomized study.

BACKGROUND: Proximal humerus fracture is the third most common fracture type after hip and distal radius fracture in elderly patients. A comprehensive study by Palvanen et al. demonstrated an increase in the annual fracture rate of 13.7% per year over the past 33 years. Should this trend continue, the fracture rate would triple over the next three decades. The increasing incidence of low-energy fractures raises questions about the optimal treatment in terms of functional outcome, pain, and rehabilitation time, as well as the economical impact. Despite the high incidence and costs of proximal h

[Compound fractures of the proximal humerus].

This paper stresses the importance of considering reconstructive measures when dealing with complex fractures, including fracture dislocations of the proximal humerus. Mainly, we try to use "biological" techniques with minimal exploration and internal fixation for stabilization. Indications for internal fixation are compared with prosthetic replacement and primary fusion.

Long-term results of uncemented humeral components in shoulder arthroplasty.

The purpose of this study was to determine the long-term results of uncemented humeral components in shoulder arthroplasty. We reviewed 37 shoulder arthroplasties with press-fit humeral components with a mean follow-up of 9.2 years (range, 5.8-13.6 years). The mean Constant score was 57 points (range, 16-95 points), and the results in 70% of the patients remained satisfactory. No complications or revisions occurred related to the uncemented humeral component. On radiographic analysis, none of the humeral components showed subsidence. Of the components, 22 (59%) showed radiolucency, 12 (32%) ha

Hemiarthroplasty for glenohumeral arthritis with massive rotator cuff tears.

Rotator cuff deficiency with associated glenohumeral arthritis poses a formidable clinical challenge. Humeral head replacement with maintenance of the coracoacromial arch is the current treatment of choice. Properly performed arthroplasty can provide pain relief and modest gains in motion.

Controlling Leg Length Change During Total Hip Arthroplasty Using Three-Dimensional Modeling and a Single Intraoperative Distance Measurement.

BACKGROUND: Leg length inequality following total hip arthroplasty (THA) is a major cause of patient dissatisfaction. This study evaluated the accuracy and feasibility of a workflow combining computed tomography-based preoperative planning with a single intraoperative linear distance measurement for leg length restoration in primary THA. METHODS: A consecutive series of 40 patients undergoing primary, mixed reality-assisted THA using a minimally invasive superior capsulotomy approach was analyzed. Preoperative three-dimensional (3D) planning calculated the distance between the prosthesis shou

Radiographic stability of ingrowth humeral stems in total shoulder arthroplasty.

BACKGROUND: Cemented and uncemented stem types are available for TSA. An early uncemented stem designed for bone ingrowth had radiographic loosening of approximately 10% at intermediate followup (mean 4.6 years). Subsequent stem modifications included circumferential metaphyseal porous coating to enhance ingrowth and reduce loosening rates. QUESTIONS/PURPOSES: We asked whether the radiographic loosening rates would be lower after this design modification and whether the pain score and ROM would be improved. PATIENTS AND METHODS: We reviewed the medical records and radiographs of 76 patients

Treatment of proximal humeral fractures with reverse shoulder arthroplasty in elderly patients.

BACKGROUND: Proximal humeral fractures in four or even only three parts, with metaphyseal hinge distances of <8 mm, represent a serious and widely debated problem. Reduction is complex and plating is often instable, especially in elderly patients. Failures, sometimes involving necrosis of the head, are frequent. Hemiarthroplasty has long been used for 3- or 4-part complex fractures, even in young patients, although often with sub-optimal results, due to reabsorption of tuberosities. This complication has partly been overcome with reverse shoulder prostheses which, although more invasive than p
