Sensory Decline: Real-World Tactics for Managing Vision and Hearing Loss.

Sensory decline, particularly vision and hearing loss, is nearly universal in aging dogs and cats yet remains underrecognized in general practice. These changes are often mistaken for cognitive dysfunction or normal aging, but they significantly affect quality of life, behavior, and the human-animal bond. This review provides busy clinicians with practical, evidence-based strategies for identifying and managing sensory decline, including screening approaches, diagnostic tools, and real-world management tactics. Emphasis is placed on proactive care, client education, and the interplay between s

[Screening for sleep disorders in internal medicine as potential comorbidities of systemic autoimmune diseases and improving patients' quality of life].

Sleep medicine and internal medicine share a global and transdisciplinary vision of human physiology and illnesses, with an approach guided by the complaint and semiology. In France, approximately 13 to 18 million individuals suffer from a sleep disorder: these disorders therefore represent a public health problem. Their comorbidities with systemic autoimmune diseases are frequent. As such, this article suggests an approach to screening for sleep disorders in daily clinical practice of internal medicine leading, when appropriate, to request specialized diagnostic and/or therapeutic care in sle

Pentosan Polysulfate-Associated Macular Disease in Patients With Interstitial Cystitis.

Recent studies have implicated long-term pentosan polysulfate use with vision loss from a newly described macular condition. Affected patients report difficulty with reading and adjusting to dim lighting, and they occasionally develop severe visual disability. Macular changes resemble those seen in age-related macular degeneration, potentially leading to misdiagnosis. The objectives of this Current Commentary are to summarize studies evaluating the association between pentosan polysulfate use and macular disease, to educate pentosan polysulfate prescribers about the clinical manifestations of

Machine Learning in the Detection of the Glaucomatous Disc and Visual Field.

Glaucoma is the leading cause of irreversible blindness worldwide. Early detection is of utmost importance as there is abundant evidence that early treatment prevents disease progression, preserves vision, and improves patients' long-term quality of life. The structure and function thresholds that alert to the diagnosis of glaucoma can be obtained entirely via digital means, and as such, screening is well suited to benefit from artificial intelligence and specifically machine learning. This paper reviews the concepts and current literature on the use of machine learning for detection of the gl

Retaining Diabetic Eye Exams in Medicare Advantage Star Ratings: A Policy Commentary on Prevention, Equity, and Quality Measurement.

In 2025, the Centers for Medicare and Medicaid Services (CMS) proposed removing the diabetic eye examination measure from the Medicare Advantage Star Ratings program, raising concern about the future visibility of preventive vision care within chronic disease management. Following public comment and stakeholder engagement, CMS retained the measure in the 2026 final rule. This editorial examines the policy implications of that decision for quality measurement, prevention, health equity, and value-based care. Diabetic retinopathy remains a major and growing cause of preventable vision loss in th

Emerging strategies targeting genes and cells in glaucoma.

Glaucoma comprises a heterogeneous set of eye conditions that cause progressive vision loss. Glaucoma has a complex etiology, with different genetic and non-genetic risk factors that differ across populations. Although difficult to diagnose in early stages, compromised cellular signaling, dysregulation of genes, and homeostatic imbalance are common precursors to injury and subsequent death of retinal ganglion cells (RGCs). Lowering intraocular pressure (IOP) remains the primary approach for managing glaucoma but IOP alone does not explain all glaucoma risks. Orthogonal approaches such as large

Working under Pressure: Scleroderma Presenting with Bilateral Exudative Retinal Detachment in the Context of Scleroderma Renal Crisis: A Case Report.

This is an observational case report to detail a novel case of scleroderma renal crisis presenting as bilateral exudative retinal detachments in a patient with newly diagnosed systemic sclerosis. An otherwise healthy 58-year-old female presented primarily with vision complaints and was found to have malignant hypertension (230/120 mm Hg) and bilateral exudative retinal detachment on dilated fundus examination and macular OCT scan. Further history revealed sclerodactyly, mild dysphagia, and dyspnea. She was diagnosed with diffuse systemic sclerosis and Sjogren's syndrome complicated by an episo

Update on glial antibody-mediated optic neuritis.

Optic neuritis (ON) refers to inflammatory demyelinating lesions of the optic nerve, which can cause acute or subacute vision loss and is a major cause of vision loss in young adults. Much of our understanding of typical ON is from the Optic Neuritis Treatment Trial. Glial autoantibodies to aquaporin-4 immunoglobulin (AQP4-IgG) and myelin oligodendrocyte glycoprotein immunoglobulin (MOG-IgG) are recently established biomarkers of ON that have revolutionized our understanding of atypical ON. The detection of glial antibodies is helpful in the diagnosis, treatment, and follow-up of patients with

[Amblyopia].

Amblyopia is a developmental disorder of the entire visual system, including the extra-striate cortex. It manifests mainly by impaired visual acuity in the amblyopic eye. However, other abnormalities of visual function can be observed, such as decreased contrast sensitivity and stereoscopic vision, and some abnormalities can be found in the "good" eye. Amblyopia occurs during the critical period of brain development. It may be due to organic pathology of the visual pathways, visual deprivation or functional abnormalities, mainly anisometropia or strabismus. The diagnosis of amblyopia must be c

The worldwide epidemic of diabetic retinopathy.

Diabetic retinopathy (DR), a major microvascular complication of diabetes, has a significant impact on the world's health systems. Globally, the number of people with DR will grow from 126.6 million in 2010 to 191.0 million by 2030, and we estimate that the number with vision-threatening diabetic retinopathy (VTDR) will increase from 37.3 million to 56.3 million, if prompt action is not taken. Despite growing evidence documenting the effectiveness of routine DR screening and early treatment, DR frequently leads to poor visual functioning and represents the leading cause of blindness in working

Visual dysfunction in HIV-positive patients without infectious retinopathy.

Persons with HIV disease are susceptible to various manifestations of retinal damage, such as infectious retinopathies (e.g., cytomegalovirus [CMV] retinitis and toxoplasmosis) and noninfectious complications (microangiopathic infarctions or cotton-wool spots [CWS]); CWS being quite common in AIDS patients. Until recently, little research focused on noninfectious ocular pathology in HIV disease. These disorders may all affect normal functioning of the visual system while funduscopic examination results appear normal. A review of the psychophysical changes, color and contrast sensitivity, perip

The effect of health insurance coverage on medical care utilization and health outcomes: Evidence from Medicaid adult vision benefits.

Increasing the proportion of adults that have regular, comprehensive eye exams and reducing visual impairment due to uncorrected refractive error and other common eye health problems are federal health objectives. We examine the effect of vision insurance on eye care utilization and vision health outcomes by taking advantage of quasi-experimental variation in Medicaid coverage of adult vision care. Using a difference-in-difference-in-difference approach, we find that Medicaid beneficiaries with vision coverage are 4.4 percentage points (p<0.01) more likely to have seen an eye doctor in the pas

Vision and driving: the United States.

Minimal visual standards for obtaining driving licensure in the United States principally use 2 measures: visual acuity and visual field. Although research studies have established a correlation between performance on these measures and safe driving, the correlations are weak and mostly retrospective. These measures remain in place in screening centers largely because they (especially visual acuity) are practical. A newer test of visual attention, called the useful field of view, may be more predictive of safe driving than the traditional measures, but it has not been widely applied in licensi

Unexplained vision loss following silicone oil removal: A systematic review and meta-analysis.

BACKGROUND/OBJECTIVES: Sudden, unexplained visual acuity loss has been reported following silicone oil (SO) removal. This systematic review and meta-analysis aimed to estimate the pooled incidence of unexplained vision loss following SO removal (UVLSOR) and identify associated risk factors. SUBJECTS/METHODS: We searched Ovid MEDLINE, Embase, and Cochrane Library from inception to November 2025 for studies reporting unexplained vision loss after SO removal following vitreoretinal surgery. Paired independent reviewers conducted screening, data extraction, and risk of bias assessment using the R

The 6-Item Vision-Related Quality of Life and Limitations Questionnaire: Evaluation of Psychometric Properties.

PURPOSE: We developed the Vision-related Quality of life and Limitations Questionnaire (VQL-6), a screening instrument to signal a need for additional care resulting from reduced vision-related quality of life in patients with chronic ophthalmic diseases. The aim of the present study was to evaluate psychometric properties. METHODS: A Dutch population-based sample of 2032 participants (mean age, 55 ± 19 years) completed the VQL-6 and other questionnaires on vision-related quality of life, executive functioning, attention, mental health, and symptom validity. In addition, we recruited a sample

Diabetic retinopathy education and screening at the community pharmacy in Puerto Rico.

OBJECTIVE: Diabetic retinopathy (DR), the most common diabetes-related eye disorder, is associated with poor glucose control and is the most frequent cause of adult-onset blindness. The purpose of this study was to demonstrate that pharmacists can play an effective role in identifying individuals at risk for developing DR, educating them about DR, facilitating access to diagnostic eye exams, and referring patients diagnosed with vision problems or other conditions for treatment and continuing care. METHODS: This two-phase project was conducted at two independent community pharmacies in Puerto

Volume perimetry: measurement in depth of visual field loss.

PURPOSE: Volume scotomas are three-dimensional regions of space that are not visible to the observer. Volume perimetry maps volume scotomas. Volume scotomas predicted from combining monocular visual fields assume known fixation locus (mainly foveal). However, fixation loci are not always known, especially with central field loss (CFL). Here we demonstrate methods for measuring and calculating volume scotomas and discuss their practical implications. METHODS: Three patients (bitemporal hemianopia, binasal scotoma, and CFL) were evaluated. Slices through the volume scotomas were measured at thr
