A scoping review for published guidelines for use of ketamine in chronic non-cancer pain.

Off-label ketamine infusions are often used to manage chronic non-cancer pain (CNCP), including neuropathic pain, central sensitisation and opioid dose reduction. There are variable treatment regimens and routes of delivery based on clinical trials but there is the distinct lack of any clearly defined treatment guidelines. The goal of the scoping review was to determine whether there has been progress with the availability of published guidelines. Our inclusion criteria were publications that contained ketamine infusion, chronic pain and guidelines or protocols. We excluded publications that c…

Sudden Appearance of Metastatic Cancer on Magnetic Resonance Imaging in a Patient with Back Pain.

BACKGROUND: Magnetic resonance imaging (MRI) is highly sensitive and specific for detecting bone metastases. Patients with HIV have an increased risk of certain cancers, and MRI plays an important role in detecting metastatic disease. However, diagnostic accuracy may vary depending on the use of contrast medium and imaging sequences. CASE REPORT: A 69-year-old woman with HIV presented with an insidious onset of lumbar radiculopathy. She underwent lumbar spine MRI without contrast, which revealed degenerative changes. Despite conservative management, she continued to have intractable pain and

Embedding expectation effects on the management of cancer pain and related symptoms: A focus review.

As of 2022, an estimated 18 million individuals in the United States were living as cancer survivors, a number projected to rise to approximately 26 million by 2040. Advances in treatment and early detection have improved the 5-year cancer survival rate from 35% in 1960 to 65% today. However, cancer treatment often takes its own toll, and multi-modal treatments can have life-long consequences themselves, particularly in the form of chronic pain, which is known to compound the physical and emotional burden of cancer. In this review, we examine the complex burden of cancer-related pain, using br

Ileocolonic Intussusception Secondary to Colon Cancer: A Rare Cause of Abdominal Pain In Adults.

Intussusception, defined as the telescoping of one segment of the gastrointestinal tract into an adjacent one, is a rare cause of abdominal pain in the adult population due to underlying benign or malignant pathology. With the liberal use of CT in the evaluation of patients with abdominal pain, the diagnosis became more reliable. Resection of the bowel segment is the recommended treatment in most cases. We are presenting the case of a 76-year-old male patient who presented with a three-week history of abdominal pain and diarrhea. The evaluation was consistent with ileocolic intussusception. Ro

Characteristics and Outcomes of Patients with Cancer Pain Placed in an Emergency Department Observation Unit.

Pain remains an undertreated complication of cancer, with poor pain control decreasing patients’ quality of life. Traditionally, patients presenting to an emergency department with pain have only had two dispositions available to them: hospitalization or discharge. A third emerging healthcare environment, the emergency department observation unit (EDOU), affords patients access to a hospital’s resources without hospitalization. To define the role of an EDOU in the management of cancer pain, we conducted a retrospective study analyzing patients placed in an EDOU with uncontrolled cancer pain fo…
