Treatment options for moderate to severe atopic dermatitis.

Background: The treatment of chronic refractory moderate-to-severe atopic dermatitis (AD) has traditionally relied on broad-spectrum systemic anti-inflammatory agents. With the introduction of biologics and Janus kinase inhibitors (Jakinib), the step management of moderate-to-severe AD is rapidly changing; however, guidelines have yet to provide formal recommendations for how to best incorporate these agents into the treatment plan. Objective: To summarize the updated evidence-based medical treatment for AD, including a proposed position for biologics and Jakinibs in the treatment algorithm. M

Adolescents' Perspectives on Atopic Dermatitis Treatment-Experiences, Preferences, and Beliefs.

IMPORTANCE: For a considerable proportion of pediatric patients, atopic dermatitis symptoms persist into adolescence. Previous studies have focused mainly on (parents of) children, whereas little is known about adolescents with atopic dermatitis. OBJECTIVE: To explore the beliefs, experiences, and preferences of adolescents with atopic dermatitis toward their treatment. DESIGN, SETTING, AND PARTICIPANTS: We conducted a qualitative study employing focus group interviews of 15 adolescents (aged 12-18 years) who collected at least 1 prescription for topical corticosteroids in class 2 (moderatel

Adjuvant treatment with the bacterial lysate (OM-85) improves management of atopic dermatitis: A randomized study.

BACKGROUND: Environmental factors play a major role on atopic dermatitis (AD) which shows a constant rise in prevalence in western countries over the last decades. The Hygiene Hypothesis suggesting an inverse relationship between incidence of infections and the increase in atopic diseases in these countries, is one of the working hypothesis proposed to explain this trend. OBJECTIVE: This study tested the efficacy and safety of oral administration of the bacterial lysate OM-85 (Broncho-Vaxom®, Broncho-Munal®, Ommunal®, Paxoral®, Vaxoral®), in the treatment of established AD in children. METHO

The future of topical analgesics.

Topically applied analgesic therapies have been used throughout history to treat a variety of patient conditions that present with pain. Before modem pharmaceuticals became readily available, mud-based emollients, salves, cold therapies, and other natural remedies were often used. Now we have effective therapies and are developing advanced topical analgesics as we learn more about the physiology and pathophysiology of pain. The use of topical analgesics may be associated with fewer patient systemic side effects than are seen with oral, parenteral, or transdermally administered agents, making t

Treatment of actinic keratoses with 5% topical imiquimod: a multicenter prospective observational study from 93 Austrian office-based dermatologists.

BACKGROUND: While randomized, controlled trials have generated information about the safety and efficacy of imiquimod 5% cream in the treatment of actinic keratosis, still very little is known about the challenges and pitfalls of this therapy in the daily clinical routine. OBJECTIVE: To mirror the full picture of the actinic keratosis imiquimod routine therapy, ie, patient profile, in-therapy decisions, tolerability, and satisfaction. METHODS: The present observational, multicenter study included 463 patients from the offices of 93 non-hospital based Austrian dermatologists. Inclusion was so

[Acquired ichthyosis and haematological malignancies: five cases].

BACKGROUND: Acquired ichthyosis is a rare condition that can reveal an unsuspected haematological malignancy, thus allowing early diagnosis and management. If ichthyosis regresses under treatment for the haematological disorder, its recurrence reflects a turning point in the course of the disease and implies worsening of the prognosis. PATIENTS AND METHODS: The patients were examined at a joint dermatology/haematology consultation. The diagnosis of ichthyosis was based on clinical examination alone with no patients undergoing skin biopsy. RESULTS: Our series included three men and two women

Childhood psoriasis.

Psoriasis is a common dermatosis in children with about one third of all patients having onset of disease in the first or second decade of life. A chronic disfiguring skin disease, such as psoriasis, in childhood is likely to have profound emotional and psychological effects, and hence requires special attention. Psoriasis in children has been reported to differ from that among adults being more frequently pruritic; plaque lesions are relatively thinner, softer, and less scaly; face and flexural involvement is common and guttate type is the characteristic presentation. Whether onset in childho

Pattern of skin diseases in a geriatric patient group in Taiwan: a 7-year survey from the outpatient clinic of a university medical center.

BACKGROUND: Geriatric health care has become a worldwide concern, but relatively few statistical studies are available about geriatric skin diseases. Moreover, no information exists regarding skin disorders among the elderly population in Taiwan that has become a geriatric country. OBJECTIVE: To determine the characteristic pattern and the prevalence of various skin disorders for the elderly who visited the National Taiwan University in the last 7 years. METHODS: Using a database from the Dermatology Outpatient Clinic of the National Taiwan University Hospital, 1993-1999 file, we conducted a

Improved topical treatment of lamellar ichthyosis: a double-blind study of four different cream formulations.

Lamellar ichthyosis (LI) is characterized by generalized scaling of the skin and is often resistant to ordinary emollients. Recently, Locobase(R) fatty cream containing a mixture of 5% lactic acid and 20% propylene glycol (LPL) was found to be markedly effective in a pilot study. To consolidate this finding, a double-blind study comparing LPL with the corresponding mixture in Essex(R) (Diprobase(R)) cream (LPE) and Locobase(R) fatty cream containing either 5% urea or 20% propylene glycol was conducted in 20 patients with LI. Before and after applying the creams twice daily on each of the four

Histological Analysis Shows That a Moisturizer Formulated for Dermatoporosis Increases Elastin Content and Improves Skin Elasticity.

BACKGROUND: Dermatoporosis involves age-related alterations in dermal structure and cellular function, including changes in collagen, elastin, and markers of skin senescence. Changes in skin biomarkers (collagen I, elastin, senescence-associated beta-galactosidase, CD44, and CD47) were assessed following the use of a novel moisturizer, GC (Galderma Laboratories, L.P., Fort Worth, TX), by subjects with mature, aging skin. METHODS: This 12-week multisite study recruited female and male subjects aged 40 to 60 years of all races, ethnicities, and Fitzpatrick skin types. Subjects were required to

Effect of preoperative topical dexpanthenol moisturizer on the prevention of angular cheilitis after pediatric adenotonsillectomy: a prospective randomized controlled trial.

PURPOSE: To assess whether applying topical dexpanthenol to the oral commissures immediately before surgery reduces the incidence of postoperative angular cheilitis in children undergoing adenotonsillectomy. METHODS: A total of 101 pediatric patients who underwent adenotonsillectomy were included in this study. Patients were randomly divided into two groups based on whether or not dexpanthenol was applied to the lip corners immediately before surgery. Postoperative pain and presence of angular cheilitis were assessed and compared between the groups on the postoperative first day and first week

A novel facial moisturizer containing Saccharide isomerate accelerates skin barrier restoration following intense pulsed light treatment: a randomized split-face study.

BACKGROUND: Intense pulsed light (IPL) therapy is widely used for skin rejuvenation, but often impairs the skin barrier, causing discomfort and prolonging recovery. OBJECTIVES: To evaluate the efficacy and safety of a novel moisturizer containing saccharide isomerate in restoring skin barrier function and alleviating symptoms after IPL treatment in Chinese population. METHODS: In this split-face study, thirty subjects with skin aging received a single IPL treatment. Each participant applied the test moisturizer containing saccharide isomerate to one randomly assigned cheek and the control mo

1% Colloidal Oatmeal Cream/Baby Wash Regimen Improved Atopic Dermatitis in Pediatric Patients From First Use.

BACKGROUND AND OBJECTIVE: Atopic dermatitis (AD) affects 15% to 20% of children. Evidence supporting the effectiveness of colloidal oatmeal-based moisturizers in improving mild-to-moderate AD is accumulating. Data on use with a bathing routine, where compromised skin could be affected, is lacking. This study evaluated the effectiveness and tolerability of 1% colloidal oatmeal-containing cream and gentle baby wash in children with AD. METHODS: In this open-label, single-arm study of children 3 to 72 months old with mild-to-moderate AD, 1% colloidal oatmeal-containing cream was applied twice da

A novel, fixed-dose calcipotriol and betamethasone dipropionate cream for the topical treatment of plaque psoriasis: Direct and indirect evidence from phase 3 trials discussed at the 30th EADV Congress 2021.

Four posters about the novel, fixed-dose calcipotriol and betamethasone dipropionate cream (CAL/BDP cream) based on Poly-Aphron Dispersion (PAD) Technology were presented at the 30th European Academy of Dermatology and Venereology (EADV) Congress 2021 and are summarized here. CAL/BDP cream was compared in two randomized, phase 3 trials to vehicle and active comparator (CAL/BDP gel/topical suspension [TS]) in adults with plaque psoriasis (NCT03802344 and NCT03308799). Pooled data from both trials demonstrated significant greater efficacy in favour of CAL/BDP cream for all efficacy endpoints, in

Management of incontinence-associated dermatitis with a skin barrier protectant.

The skin performs many important protective functions, one of which is to act as a barrier to moisture, irritants and bacteria. Good management of patients' skin is a fundamental part of nursing care to prevent development of complex and distressing problems, such as pressure ulceration and incontinence-associated dermatitis (IAD). IAD is skin breakdown related to faecal and/or urinary incontinence, which requires adoption of a structured skin care regimen, including regular skin inspection, cleansing and the use of skin barrier protectants, to proactively protect the skin from irritant bodily

Phototherapy: Kids are not just little people.

Phototherapy is the delivery of treatments in the form of visible or ultraviolet light for the therapeutic care of a patient. Usage of phototherapy in children is affected by limited data in the medical literature, the inability of some children to stand still during the delivery of therapy, parental concerns regarding risks of therapy, and scheduling difficulties. Despite the limitation of data, there are publications in support of usage of phototherapy, especially for psoriasis, atopic dermatitis, and vitiligo in both children and adults. This contribution provides an overview of the utility
