Effects of exercise and respiration on hemodynamic efficiency in CFD simulations of the total cavopulmonary connection.

Congenital heart defects with a single functional ventricle, such as hypoplastic left heart syndrome and tricuspid atresia, require a staged surgical approach to separate the systemic and pulmonary circulations. Ultimately, the venous or pulmonary side of the heart is bypassed by directly connecting the vena cava to the pulmonary arteries with a modified t-shaped junction. The Fontan procedure (total cavopulmonary connection, TCPC) completes this process of separation. To date, computational fluid dynamics (CFD) simulations in this low pressure, passive flow, intrathoracic system have neglecte…

Seven-year clinical experience with the extracardiac pedicled pericardial Fontan operation.

BACKGROUND: Although improved perioperative outcomes with growth potential of the extracardiac pedicled pericardial Fontan (EPPF) operation have been suggested, no advantage has been demonstrated. METHODS: We retrospectively reviewed our institutional experience of 54 consecutive patients undergoing EPPF between June 1996 and August 2003. Clinical and echocardiographic follow-up was obtained yearly with a mean follow-up of 2.8 +/- 2.0 years. RESULTS: There were 29 males, median age 3.3 years (2-6.8). Median cardiopulmonary bypass time was 79 min (39-295). Fibrillatory arrest was used briefly

One and one half ventricle repairs.

Patients with functionally or anatomically borderline pulmonary ventricles can be managed by a biventricular repair, a Fontan procedure, or by an intracardiac repair in association with a cavopulmonary anastomosis. The latter repair is known as a one and one half ventricle repair. Extending the limits of a biventricular repair can be associated with a high early mortality. The Fontan is associated with late failure. One and one half ventricle repairs are an attempt to reduce early risk and late failure while still achieving separate pulmonary and systemic circulations. The two most common reas…

Transcatheter extracardiac Fontan completion: a minimally invasive pathway for single-ventricle care.

BACKGROUND: Transcatheter completion of the extracardiac Fontan provides a minimally invasive option for selected children with single-ventricle physiology who have undergone staged preparation at the time of the bidirectional cavopulmonary connection. METHODS: We performed a retrospective single-centre review of consecutive patients treated between 2022 and 2025. Candidate selection, cross-sectional imaging, diagnostic catheterisation, procedural steps, early recovery, and follow-up outcomes were collected. RESULTS: Five patients underwent transcatheter completion at a median age of 7 years

Principal component analysis identified neo-aortic diameter variations post Norwood surgery associated with the single ventricle performance and flow quality.

The purpose of this study was to investigate neo-aortic curvature and diameter variation using the principal component analysis in patients who underwent a Norwood procedure for hypoplastic left heart syndrome. We further assessed whether neo-aortic curvature and diameter features are associated with clinical outcomes, single right ventricle function and flow hemodynamic patterns derived by 4D-Flow MRI. 55 patients with Fontan circulation who underwent a Norwood procedure in infancy underwent cardiac MRI as part of surveillance of their Fontan circulation. Neo-aortic models segmented from the

Anomalous systemic vessel to the lung with hypoplastic left heart syndrome in a fetus.

We report a case of a fetus with hypoplastic left heart syndrome in addition to an anomalous vessel extending from the descending thoracic aorta to the basal segments of the lower left lung without sequestration. The concurrence of these 2 congenital abnormalities is extremely rare and unreported in the existing literature. The anomalous vessel to the left lung may cause increased pulmonary vascular resistance, which has implications for the long-term management of hypoplastic left heart syndrome via the Fontan procedure.

Thrombolytic therapy for superior vena caval thrombosis following superior vena cava-pulmonary artery anastomosis.

Immediate postoperative thrombosis of left superior vena cava-left pulmonary artery anastomosis in a modified Fontan procedure for single ventricle and pulmonary artery stenosis is described. Before thrombolytic therapy with streptokinase is initiated, confirmation of thrombosis by venography is mandatory to lessen the risk of cardiac tamponade or hemothorax. By this technique major surgical intervention is avoided, but close attention to the dose of streptokinase and the coagulation profile is essential. Measures to be taken if bleeding occurs with streptokinase therapy are described.

Quantitative analysis of extracardiac versus intraatrial Fontan anatomic geometries.

BACKGROUND: There exists large geometric variability among total cavopulmonary connections (TCPC) because of the patient-specific anatomies and the chosen surgical procedure. In this study we present quantitative comparison of the geometric characteristics of the extracardiac and intraatrial Fontan anatomies, the two commonly used TCPC procedures. METHODS: A method of centerline approximation of the three-dimensional geometries (skeletonization) was used to quantify the TCPC geometric parameters such as vessel areas, curvature, and collinearity. The TCPC anatomies of 26 patients, 13 extracard

Insufficiency of a Damus-Kaye-Stansel anastomosis in a Fontan patient: Transfemoral implantation of an Edwards Sapien 3 valve.

We present a 22-year-old patient with a univentricular heart who had already undergone five open heart surgeries including a Damus-Kaye-Stansel procedure, Fontan completion and tricuspid valve replacement. In addition, epimyocardial pacemaker implantation and repeated revisions had been necessary. He developed symptomatic free regurgitation of the pulmonary portion of his DKS anastomosis. To avoid additional high-risk open-heart surgery, we successfully implanted an Edwards Sapien 3 valve transfemorally in the pulmonary portion of the DKS anastomosis relieving insufficiency.

Estimating Mortality Risk for Adult Congenital Heart Surgery: An Analysis of The Society of Thoracic Surgeons Congenital Heart Surgery Database.

BACKGROUND: Adjustment for case mix is critical to accurate outcomes analysis in congenital heart surgery. Established tools encompass all age groups and are not specific to the growing population of adults undergoing congenital heart operations. We derived an empirically based adult congenital heart surgery (ACHS) mortality score. METHODS: In-hospital mortality was analyzed for the 152 most common procedures/procedural groups in adults 18 years of age and older in The Society of Thoracic Surgeons Congenital Heart Surgery Database (STS-CHSD) (2000-2013).
