A Systematic Review of the Role of Runt-Related Transcription Factor 1 (RUNX1) in the Pathogenesis of Hematological Malignancies in Patients With Inherited Bone Marrow Failure Syndromes.

Somatic runt-related transcription factor 1 (RUNX1) mutations are the most common mutations in various hematological malignancies, such as myelodysplastic syndrome (MDS) and acute myeloid leukemia (AML). Mono-allelic RUNX1 mutations in germline cells may cause familial platelet disorder (FPD), an inherited bone marrow failure syndrome (IBMFS) associated with an increased lifetime risk of AML. It is suspected that additional RUNX1 mutations may play a role in the pathogenesis of hematological malignancies in IBMFS. This review aims to study the role of RUNX1 mutations in the pathogenesis of hem

Rapid Hematological and Molecular Response to Pegylated Interferon in WHO-Defined Pre-Fibrotic Myelofibrosis.

Background: Pre-fibrotic myelofibrosis (pre-PMF) is a rare subentity of myeloproliferative neoplasm (MPN) with limited overall survival and the potential for progression toward overt, symptomatic myelofibrosis or blastic transformation. To date, a consensus on the treatment of pre-PMF, especially in asymptomatic cases, has yet to be reached. Interferon therapy has been employed as an effective treatment for MPN for many decades, due to its immunomodulatory properties and disease-modifying effects. Methods: The objective of this monocentric study was to evaluate hematological, molecular, and cl

The Effect of Cancer Treatments on Telomere Length: A Systematic Review of the Literature.

BackgroundIt has been hypothesized that cancer treatments cause accelerated aging through a mechanism involving the shortening of telomeres. However, the effect of cancer treatments on telomere length is unclear.MethodsWe systematically reviewed the epidemiological evidence evaluating the associations between cancer treatment and changes in telomere length. Searches were performed in PubMed for the period of January 1966 through November 2016 using the following search strategy: telomere AND (cancer OR tumor OR carcinoma OR neoplasm) AND (survivor OR patient). Data were extracted and the quali

JAK/STAT signaling in hematological malignancies.

The Janus kinase (JAK)/signal transducer and activator of transcription (STAT) pathway is central to signaling by cytokine receptors, a superfamily of more than 30 transmembrane proteins that recognize specific cytokines, and is critical in blood formation and immune response. Many of those receptors transmit anti-apoptotic, proliferative and differentiation signals, and their expression and functions are critical for the formation of blood lineages. Several cancers, including blood malignancies, have been associated with constitutive activation of members of the STAT family, which normally re

Immunoglobulin A Myeloma in a Newly Diagnosed Sickle Cell Disease Patient.

Several neoplasms including hematological malignancies occurring in very few patients with sickle cell syndromes have been reported in literature, particularly as survival among these patients and diagnostic accuracy have continued to improve. Multiple myeloma (MM) has rarely been reported in patients with hemoglobin (Hb) S + C. We report a 65-year-old retired banker who was recently diagnosed with HbS + C. This patient developed MM with markedly elevated erythrocyte sedimentation rate, mild anemia, 80% bone marrow plasmacytosis, and elevated serum immunoglobulin A level, while plain X-ray of

Systemic mastocytosis with recurrent anaphylactic shock and multiple organ dysfunction failure.

BACKGROUND: Systemic mastocytosis (SM) is a rare neoplasm. The symptoms of this disease vary among patients. The authors describe a rare case of SM with recurrent anaphylactic shock and multiple organ dysfunction failure. METHODS: Hematologic investigation, bone marrow aspirate and biopsy, and cytogenetic analysis were performed. RESULTS: The patient was rescued with positive treatment, administered on prednisolone and H1/H2-receptor blocking agents. Corticosteroid and IFN-α treatment have no significant effect on tumor burden, but no more anaphylactic shock occurred. CONCLUSIONS: Cladribin

Atypical chronic myeloid leukemia with t(9;22)(p24,11.2), a BCR-JAK2 fusion gene.

We report here on a rare case of BCR-ABL1-negative atypical chronic myeloid leukemia with a t(9;22)(p24;q11.2)translocation and a BCR-JAK2 fusion gene, with resistance to the tyrosine kinase inhibitors imatinib and dasatinib. At two years of follow-up, the patient showed no hematologic response and was submitted to an allogeneic bonemarrow transplantation. Fifty-three days after the procedure, he died due to acute graft-versus-host disease. This BCR-JAK2 fusion gene has so far been found in only five patients in the whole world, with three clinical presentations: myeloproliferative neoplasm, ac

Blastic Plasmacytoid Dendritic Cell Neoplasm (BPDCN): 2025 Update on Diagnosis, Pathophysiology, Risk Assessment, and Management.

OVERVIEW: Blastic plasmacytoid dendritic cell neoplasm (BPDCN) is a rare, aggressive hematologic neoplasm that involves multiple organs, including the skin, bone marrow and blood, lymph nodes, and central nervous system. Violaceous tumors or plaques appearing in sun-exposed areas are characteristic and present in most patients. The disease typically affects adult men aged 60-70 years, although children and younger adults may also be affected. DIAGNOSIS: Diagnosis is based on biopsy of an involved site and is typically based on the identification of blastoid cells displaying the classical immu

Simultaneous blastic plasmacytoid dendritic cell neoplasm and myelofibrosis: A case report.

Blastic plasmacytoid dendritic cell neoplasm (BPDCN) is an extremely rare and aggressive tumor with an unknown pathogenesis. Myelofibrosis (MF) is a type of myeloproliferative neoplasm. MF can be secondary to several hematological malignancies, including chronic myeloid leukemia, myelodysplastic syndrome and hairy cell leukemia. In the present report, a rare case of BPDCN secondary to MF is described. A 70-year-old male patient developed a large purplish-red rash with recurrent symptoms. BPDCN was confirmed by immunohistochemistry of a biopsy specimen and flow cytometry of bone marrow cells. B

Chronic myelomonocytic leukemia with ETV6-ABL1 rearrangement and SMC1A mutation.

Chronic myelomonocytic leukemia (CMML) is a rare malignant neoplasm of the blood-forming cells in bone marrow characterized by persistent monocytosis. Although most patients with CMML show clonal genetic aberrations, there is no known cytogenetic or molecular genetic finding that is specific to CMML. We report a patient who had a clinical and morphological presentation consistent with CMML. The genetic work-up showed an ETV6-ABL1 fusion consequent to a 9;12 translocation, and a missense mutation in SMC1A (c.1757G>A, p.Arg586Gln). The SMC1A mutations are recurrent, albeit rare, in myeloid malig

OBJECTIVES: Determine ancillary test utilization for the workup of isolated eosinophilia in otherwise morphologically unremarkable bone marrow (BM). METHODS: We evaluated BM ancillary testing performed in cases with isolated eosinophilia and otherwise morphologically unremarkable BM. Cases with abnormal morphology (eg, dysplasia, basophilia) and/or findings suggestive of a disorder (eg, unexplained thromboses, lymphoma) are specifically excluded. RESULTS: A total of 132 cases met inclusion criteria. Ten cases had an ancillary testing abnormality that warranted a more specific hematologic dia

Utility of a Fluorescence Microscopy Imaging System for Analyzing the DNA Ploidy of Pathological Megakaryocytes Including 5q- Syndrome.

To investigate megakaryocyte (MK) DNA ploidy in various hematological diseases, fluorescence microscopy imaging system (FMI) can be used to analyze DNA ploidy with cell morphology at the single-cell level by using specialized image-processing software. Here we compared DNA ploidy obtained by FMI measured with that obtained flow cytometry (FCM). With FMI, we could evaluate the DNA ploidy in long-term preserved bone marrow smear samples after staining. We next analyzed the MK DNA ploidy in 42 bone marrow smear samples including 26 myeloid neoplasm cases, and we compared the DNA ploidy and platel

Detection of Hematological Malignancies Using N-NOSE (Nematode-NOSE).

Hematological malignancies often lack defined risk factors and present with non-specific symptoms, underscoring the urgent need for simple and reliable detection methods. To address this challenge, Hirotsu et al. innovated N-NOSE, a novel, non-invasive cancer screening test that utilizes the chemotaxis response of the nematode Caenorhabditis elegans to detect tumor-related odors in urine. In this clinical study, we assessed the performance of N-NOSE in patients with various hematological malignancies at diagnosis and during treatment. Urine samples were collected from 30 healthy individuals an

[Chinese guidelines for diagnosis and treatment of myelodysplastic neoplasms (2026)].

Myelodysplastic neoplasms (MDS) are a group of highly heterogeneous myeloid tumors originating from hematopoietic stem cells, clinically characterized by single or multiple lineage cytopenia with a risk of acute myeloid leukemia transformation. In recent years, significant progress has been made in basic research and the development of novel targeted therapy for MDS, and the 5th edition of the WHO classification has updated the subtypes of MDS. Therefore, the Chinese Society of Hematology, Chinese Medical Association has revised the "Chinese Guidelines for Diagnosis and Treatment of Myelodyspl

Pseudotumor Cerebri Syndrome and Essential Thrombocythemia: A Case Report.

Pseudotumor cerebri syndrome (PTCS) is characterized by elevated intracranial pressure in the absence of intracranial mass lesions, structural abnormalities, or infectious conditions. Although the exact pathogenesis of PTCS remains largely elusive, it is increasingly recognized as a multifactorial condition. Identifying secondary contributors is essential for guiding etiology-specific management and achieving an optimal treatment response. Essential thrombocythemia (ET) is a myeloproliferative neoplasm involving sustained thrombocytosis and an elevated risk of thrombotic complications. We pres

Periportal Extramedullary Hematopoiesis With Bile Duct Dilatation: A Case Report of a Rare Condition.

Extramedullary hematopoiesis (EMH) occurs in organs outside the bone marrow. In adults, it is usually linked to inefficient medullary hematopoiesis in the context of hematological diseases. It can manifest as hepatosplenomegaly and masses in different organs that can mimic neoplasms, often requiring histopathological analysis. Integrating clinical and radiological findings is crucial for accurate diagnosis and to avoid unnecessary invasive procedures. This report presents a case of an adult male patient with EMH and bile duct dilatation (a rare combination) in the context of accelerated phase
