Dysarthria of motor neuron disease: longitudinal measures of segmental durations.

Motor neuron disease encompasses a group of terminal, demyelinating diseases affecting upper- and lower-motor neurons and producing muscular weakness resulting in a flaccid, spastic, or spastic-flaccid dysarthria of speech. The present study presents measurements of the temporal-acoustic characteristics of dysarthria in three subjects with Motor Neuron Disease over a two-year recording period. Changes seen over the course of the disease varied by type of motor neuron disease, though all types demonstrated some degree of neutralization of the prevocalic VOT, target vowel duration, and postvocal…

Postradiation lower motor neuron syndrome presenting as monomelic amyotrophy.

Monomelic amyotrophy developed 16 months, nine and 12 years after irradiation of the lumbosacral spinal cord for seminoma in one patient and for Hodgkin's disease in two others. In two patients, involvement was clinically limited to one leg, with a subacute course followed by plateau in the first case and with progressive worsening in the second one. In the third patient, the course was progressive with involvement of the other lower limb occurring five years later. From clinical and electrophysiological data, it seems probable that the disease process was a result of a selective injury to the

Diagnosis of motor neuron disease by neurologists: a study in three countries.

Ninety four neurologists in the United Kingdom, China, and West Germany responded to two structured questionnaires. The first assessed the diagnostic weighting assigned to a number of symptoms, signs, and clinical investigations ascertained from classical descriptions and case notes of patients with motor neuron disease (MND). The second tested the likelihood and consistency of diagnosis in a series of case summaries representing the clinical data of 10 patients with clinically and pathologically documented motor neuron disease. There was a wide measure of agreement concerning the common clini

Risk factors in motor neuron disease: a case-control study.

A case-control study of risk factors in 512 cases of motor neuron disease (MND) and 512 controls, affected by other neurological diseases, was performed. Clinical history showed a significant increase in gastric ulcer and mechanical injuries among MND cases, whereas the frequencies of operations, poliomyelitis, malignancies and autoimmune diseases were similar for cases and controls. The women affected by MND had a later menarche and an earlier menopause; therefore, the reproductive period was significantly shorter, supporting a possible role of sexual hormones in the pathogenesis of MND. The

Detecting lower motor neuron dysfunction of the pharynx and larynx with electromyography.

This study assessed the utility of clinical electromyography (EMG) for detecting lower motor neuron (LMN) or upper motor neuron (UMN) dysfunction affecting the intrinsic muscles of the larynx and pharynx. Twenty-nine subjects were examined; their clinical diagnoses included perioperative nerve injury, cerebral infarction, and lateral medullary infarction. Resting activity, motor unit action potential (MUAP) morphology, and MUAP recruitment were evaluated in every case. Medical records (excluding EMG data) were analyzed for clinical evidence of LMN or UMN dysfunction in the intrinsic muscles of

The F response represents the recurrent discharge of a small percentage of the motoneuron pool activated antidromically by any single impuls … gy and course) a change in the frequency of recurrent discharge and/or in the total number of lower motor neurons capable of backfiring occurs. Therefore, F response reflects the hyperexcitability state of the lower motor neurons in spasticity.

[Neuronal organization of field 4 of the motor cortex of the cat brain].

By means of the silver nitrate impregnation method after Golgi-Kopsch in kittens and young cats the field 4 in the cerebral motor cortex has been studied. The motor cortex of the field 4 possesses certain heteromorphism. Besides usual stellate and pyramidal neurons, that differ from real ones by some morphological signs: their body is often round, the apical dendrite is much thinner than the corresponding dendrite of a pyramidal neuron, it does not produce oblique branches along the course, never gets into the I layer, the spines arrange less densely. According to the mode of dendrites setting

Primary amyloidosis with peripheral neuropathy and signs of motor neuron disease.

A 51-year-old man had primary amyloidosis, with typical amyloid neuropathy and signs of motor neuron disease, including widespread fasciculation in limb muscles, tongue atrophy and fasciculation, swallowing and chewing difficulty, symmetric hyperreflexia, and bilateral Hoffmann's signs. Fasciculations, fibrillations, and positive sharp waves were found in electromyography of all muscles tested. Motor nerve conduction velocities were moderately slow. Lambda chains were detected in serum and CSF. Amyloid was found in sural nerve biopsy. This combination of amyloid neuropathy and features of amyo

[Conduction in the motor neurons of the peripheral nerves in hemiplegia of cerebral origin].

A study of motor nerve conduction velocities in median, ulnar, peroneal and tibial nerves bilaterally in 50 hemiplegic patients revealed a statistically significant slowing in the affected limbs compared with the unaffected side. The decrease of nerve conduction velocity was not related to any of the following factors: duration of stroke, degree of paralysis, degree of spasticity, side of paralysis, age of patients, and sex ef patients. The results can indicate that lower motor neurons are indeed affected by upper motor neuron lesions. The slowing of the nerve conduction velocity of motor peri…

Dementia and motor neuron disease: morphometric, biochemical, and Golgi studies.

In three patients dementia without neurofibrillary tangles or Pick bodies antedated amyotrophy by several years. The motor neuron disorder in two patients was characterized by terminal bulbar symptoms; in one it was similar to classic amyotrophic lateral sclerosis. In two patients, quantitative studies of selected regions of the cortex using a computerized image analyzer disclosed, as in patients with senile dementia of Alzheimer type, a marked reduction in the number of neurons, especially those larger than 90 mu 2. The findings differed from those in Alzheimer dementia, however, in that the

Presenile dementia with motor neuron disease -an additional case report-.

A 51-year-old male has suffered the gradual onset of difficulty with memory, concentration and cognition at age 50. He has progressively developed severe dementia accompanied by muscle wasting and fasciculation prominent in the upper arms and shoulders and the bulbar muscles, EMG and muscle biopsy revealed denervation patterns and a CT-scan showed moderate cerebral atrophy. Cases of presenile dementia with motor neuron disease reported in Japan have been reviewed and the possibility of a new clinico-pathologic entity is discussed.

The evidence for old age motor neuron death: A scoping review.

Age-related neuromuscular dysfunction is widely recognized; however, the extent to which aging is accompanied by motor neuron (MN) loss remains underexplored. This review evaluates evidence for MN loss with aging across species and anatomical motor pools. We synthesize PUBMED studies in humans, non-human primates, rats and mice within the spinal cord and brainstem. Attention was given to the methodological approach used to quantify MNs and categorized based on what MNs were quantified (specific pools, vertebral levels, lateral motor columns). Methods were assessed qualitatively, based on stere

Not ready to die yet! A patient perspective of motor neurone disease.

Palliative care expertise is appropriate at various points in a person's life or during a specific illness, but there is a widespread public and professional misperception that it is an 'end of life' service. This patient view is written jointly by Mark and by the consultant in his local palliative care service, who has been intermittently involved in his care and healthcare planning for a decade.
