Developing the supportive care bundle for mothers of infants hospitalised in the neonatal intensive care unit: protocol for a multimethod study.

INTRODUCTION: Neonatal admission to the neonatal intensive care unit (NICU) represents an acute crisis for the family, particularly for the mother. During this period, mothers face a broad spectrum of care-related, emotional, informational and supportive needs; when these needs are not adequately addressed, serious negative consequences for maternal mental health and the mother–infant bond may ensue. Despite the significance of this issue, a comprehensive and culturally adapted care bundle for this population remains notably absent within Iran's healthcare system. The present study aims to dev

Retinopathy of prematurity in South Africa: an assessment of needs, resources and requirements for screening programmes.

AIMS: Retinopathy of prematurity (ROP) is a major cause of blindness in children in middle-income countries. In 1995, it accounted for 10.6% of blindness in children in schools for the blind in South Africa. This study was undertaken to estimate the number of premature babies at risk and to investigate policies, practices and screening programmes. MATERIALS AND METHODS: 17 level 1-3 neonatal units were visited in four provinces. Published literature reports were reviewed and staff interviewed. RESULTS: 13,000-15,000 surviving premature babies are at risk of ROP each year. Shortage of equipme

Multicenter randomized clinical trial of home uterine activity monitoring: pregnancy outcomes for all women randomized.

OBJECTIVE: Our purpose was to evaluate the impact of home uterine activity monitoring on pregnancy outcomes among women at high risk for preterm labor and delivery. STUDY DESIGN: Women at high risk for preterm labor at three centers were randomly assigned to receive high-risk prenatal care alone (not monitored) or to receive the same care with twice-daily home uterine activity monitoring without increased nursing support (monitored). There were 339 women with singleton gestations randomized with caregivers blinded to group assignment. The two groups were medically and demographically similar

Prenatal behavior of multiples: implications for families and nurses.

Each twin and higher order multiple leads an extraordinary life before birth. The literature has revealed that the intrauterine environment differs for every multiple fetus and helps to shape the individuality of each multiple-birth child. From early in pregnancy, it appears that a multiple develops his or her own temperament and each set of multiples establishes an individualized pattern of tactile communication between or among themselves. The same behaviors, traits, and intermultiple interactions seen in pregnancy also have been observed in infancy and beyond. Intermultiple communication co

Hyperbilirubinemia: new approaches to an old problem.

Hyperbilirubinemia continues to be a common problem of the term and near-term neonate. Each year more than 60% of infants born in the United States develop jaundice, making it difficult at times to differentiate jaundice due to pathologic reasons from jaundice due to physiologic ones. Now, the knowledge that bilirubin may even have a beneficial role as an antioxidant has caused a great deal of renewed interest in the management of jaundice. The American Academy of Pediatrics has published revised guidelines for management of neonatal jaundice that have resulted in much discussion. The article

Bilious vomiting in the newborn: rapid diagnosis of intestinal obstruction.

Bilious vomiting in newborns is an urgent condition that requires the immediate involvement of a team of pediatric surgeons and neonatologists for perioperative management. However, initial detection, evaluation and treatment are often performed by nurses, family physicians and general pediatricians. Bilious vomiting, with or without abdominal distention, is an initial sign of intestinal obstruction in newborns. A naso- or orogastric tube should be placed immediately to decompress the stomach. Physical examination should be followed by plain abdominal films. Dilated bowel loops and air-fluid l

Parasitic conjoined twins: a case report.

Parasitic twins are a rare phenomenon and a challenge for caregivers. Occurring as a result of abnormal processes in the first several weeks of embryologic development, this condition is oftentimes diagnosed by prenatal ultrasound. This unusual condition may go undiagnosed until the time of delivery, however, despite the availability and use of advanced ultrasound techniques. The challenges presented by this condition are multiple and complex. The following case report demonstrates some of these challenges.

The rhythm of a preterm neonate's life: ultradian oscillations of heart rate, body temperature and sleep cycles.

The objectives are to characterize oscillations of physiological functions such as heart rate and body temperature, as well as the sleep cycle from behavioral states in generally stable preterm neonates during the first 5 days of life. Heart rate, body temperature as well as behavioral states were collected during a daily 3-h observation interval in 65 preterm neonates within the first 5 days of life. Participants were born before 32 weeks of gestational age or had a birth weight below 1500 g; neonates with asphyxia, proven sepsis or malformation were excluded. In total 263 observation interva

A quality improvement initiative to improve transport team mobilization for emergent neonatal transfers.

INTRODUCTION: A considerable number of neonatal deaths can be attributed to delays in accessing appropriate medical care or the absence of reliable systems for transferring newborns to advanced care centers. Infants requiring urgent transfer to specialized neonatal units are particularly vulnerable, often due to the limited capabilities of peripheral birthing facilities, the absence of structured neonatal transport networks, and the reliance on unsafe or uncoordinated transfer methods. Approximately 25% of neonatal transfers are time-sensitive and require specialized neonatal teams to respond

In-line filters in central venous catheters in a neonatal intensive care unit.

Nosocomial sepsis remains an important cause of morbidity in neonatal intensive care units. Central venous catheters (CVCs) and parenteral nutrition (TPN) are major risk factors. In-line filters in the intravenous (IV) administration sets prevent the infusion of particles, which may reduce infectious complications. We randomized infants to in-line filter (for clear fluids and lipid emulsions) or no filter placement. Sepsis, nursing time and costs were assessed. IV sets without filters were changed every 24 h, IV-sets with filters every 96 h. Of 442 infants with a CVC, 228 were randomized to fi

Substance Abuse Disorder: Prenatal, Intrapartum and Postpartum Care.

The growing opioid crisis in the United States affects childbearing women and their infants at an alarming rate. Substance use disorders in pregnancy have transitioned from a topic barely addressed to one that has become mainstream in the issue of pregnancy management. Opioid use can include appropriate use of a prescribed medication, the misuse of street drugs, and maintenance on an opioid agonist treatment such as methadone. Identifying this population of childbearing women is critical to be able to organize the appropriate resources and to provide a comprehensive multidisciplinary evidence-

Incremental Hospital Costs Associated With Comorbidities of Prematurity.

PURPOSE: Preterm birth (PTB), defined as birth at a gestational age (GA) of less than 37 weeks, is associated with increased hospital costs. Lower GA at birth is negatively correlated with the presence of neonatal comorbidities, further increasing costs. This study evaluated incremental costs associated with comorbidities of PTB following spontaneous labor at 24-36 weeks. DESIGN: Birth records from January 2001 to December 2010 at the Medical University of South Carolina were screened to identify infants born at GA 23-37 weeks after uncomplicated singleton pregnancies and surviving to dischar

Nonpharmacological techniques to reduce pain in preterm infants who receive heel-lance procedure: a randomized controlled trial.

INTRODUCTION: The heel-lance (HL) method for blood collection from the newborn is controversial for the pain it causes. This is the first randomized controlled trial on the management and reduction of pain using the music of Wolfgang Amadeus Mozart ("Sonata K. 448") in premature infants hospitalized in a neonatal intensive care unit (NICU). This study has compared nonpharmacological techniques with standard procedure for reducing pain during HL procedure. METHODS: Thirty-five premature infants were enrolled, each for 3 HL procedures, of which each was randomized to 1 of the 3 study arms. Arms

Postpartum care in the neonatal intensive care unit, PeliCaN: a randomized controlled trial.

BACKGROUND: Postpartum parents of hospitalized infants in neonatal intensive care units have higher rates of chronic disease and pregnancy complications than parents of well newborns. Neonatal intensive care unit parents may prioritize remaining at their infants' bedsides over their healthcare. OBJECTIVE: This study aimed to determine whether embedding doulas and certified nurse midwives for postpartum care in the neonatal intensive care unit (PeliCaN) would reduce the time to receive postpartum healthcare, with the hypothesis that the intervention would shorten the time and improve the compr

Systematic Review of the Effect of Topical Application of Human Breast Milk on Early Umbilical Cord Separation.

OBJECTIVE: To evaluate the efficacy of topical application of human breast milk to reduce umbilical cord separation time. DATA SOURCES: We used a three-step search strategy. First, we searched six electronic databases from inception through July 16, 2018: PubMed, Cochrane, CINAHL, Embase, Scopus, and ProQuest Dissertations and Theses Global. We used the following search terms: infant, newborn, baby, babies, colostrum, breast milk expression, breast milk, breastmilk, mother milk, human milk, umbilical cord, and umbilicus. We included published trials in English without any time limit to optimi

Nutrition for the Surgical Infant with Gastroschisis.

Nutrition for the infant with gastroschisis is a complex topic and there is not a lot of uniformity in the literature to formulate evidence-based care. This article discusses more recent findings in the literature as we search for an effective method of feeding these infants. Issues with growth and development, illustrating the variety of outcomes, are also addressed.
