Automated Detection of Periprosthetic Joint Infections and Data Elements Using Natural Language Processing.

BACKGROUND: Periprosthetic joint infection (PJI) data elements are contained in both structured and unstructured documents in electronic health records and require manual data collection. The goal of this study is to develop a natural language processing (NLP) algorithm to replicate manual chart review for PJI data elements. METHODS: PJI was identified among all total joint arthroplasty (TJA) procedures performed at a single academic institution between 2000 and 2017. Data elements that comprise the Musculoskeletal Infection Society (MSIS) criteria were manually extracted and used as the gold

D-dimer for diagnosis of periprosthetic joint infection: A meta-analysis.

BACKGROUND: The D-dimer test is easily available to detect periprosthetic joint infection (PJI). This study aimed to estimate the diagnostic accuracy of the D-dimer test in PJI diagnosis and identify possible independent factors affecting the diagnostic value of this test. METHODS: MEDLINE and EMBASE databases identified literature until February 2020 that utilized the D-dimer test for PJI diagnosis. The pooled sensitivity, specificity, area under the curve (AUC), diagnostic odds ratio (DOR), positive likelihood ratio (PLR), and negative likelihood ratio (NLR) were calculated to evaluate the

Periprosthetic Joint Infection: A Review of Antibiotic Treatment.

A team approach among orthopaedic surgeons, infectious disease specialists, and patients is of paramount importance when treating periprosthetic joint infections (PJIs). Treatment usually includes various surgical approaches along with antibiotic treatment. Antibiotic selection requires a multifactorial decision that depends on the organism that is identified, its antibiotic-resistance profile, the extent of the infection, and factors associated with the host. Antibiotic duration is dependent on surgical intervention and the type of organism. Typically, patients are treated for 6 weeks after d

The Diagnosis and Treatment of Pacemaker-Associated Infection.

BACKGROUND: Approximately 105 000 cardiac electronic devices are newly implanted in Germany each year. Germany has the highest implantation rate with respect to population of any European country. Infections in cardiac implants are serious complications, with an associated in-hospital mortality of 5-15%. It is thus very important to optimize the diagnostic and therapeutic strategies by which such infections can be detected early and treated effectively. METHODS: This review is based on pertinent publications retrieved by a search in PubMed, with special attention to the current recommendation

Vancomycin elution, activity and impact on mechanical properties when added to orthopedic bone cement.

Infection incidence for total hip and knee arthroplasty (THA and TKA, respectively) is between 0.2% and 5% and results in approximately 100,000 device failures per year in the United States. Treatment requires prolonged systemic antibiotic therapy with additional surgical revisions. As a prophylactic measure against infection, antibiotics can be incorporated into bone cement during THA and TKA to provide drug administration at the implant site. Antibiotics in bone cement are only effective if they can elute out of the cement at a concentration that is active against common organisms. There is

[Leucocyte esterase as a reliable diagnostic, cost-effective and fast in knee infections].

BACKGROUND: The articular infection represents a challenge due to its complexity and its devastating effect when not treated promptly. We have various diagnostic studies: cultures, ESR, CRP, count of leukocytes, among others but none is specific, it takes more than 30 minutes to complete and require complex infrastructure. In this study we determine the sensitivity and specificity of the leukocyte esterase for detection of an infectious process joint in Mexican population. MATERIAL AND METHODS: From November 2015 to April 2016 was obtained synovial fluid from patients with diagnosis of knee i

Management of Periprosthetic Joint Infection: The More We Learn, the Less We Know.

BACKGROUND: Despite the tremendous and long-standing success of total hip arthroplasty and total knee arthroplasty as treatments for end-stage arthritis, periprosthetic joint infection (PJI) remains a rare but feared complication of these procedures. METHODS: This review highlights some of the difficulties inherent to studying PJI. These include the difficulty in powering studies to capture this relatively uncommon complication, as well as the heterogeneity in clinical presentation and manifestations associated with the diagnosis and treatment of PJI. CONCLUSION: We suggest an algorithm for

Cardiovascular implantable electronic device lead extraction: evidence, techniques, results, and future directions.

PURPOSE OF REVIEW: Cardiovascular implantable electronic devices are widely used to treat symptomatic arrhythmias, prevent sudden cardiac death, and improve symptoms and cardiac function. Continued population growth and expanding indications have resulted in a progressive increase in the number of cardiovascular implantable electronic device implantations. Mirroring this growth, an increasing number of leads require removal because of a variety of indications. Transvenous lead extraction continues to evolve with better techniques and risk-management strategies. This review highlights the indic

Novel developments in the prevention, diagnosis, and treatment of periprosthetic joint infections.

Periprosthetic joint infection (PJI) is one of the most challenging complications compromising the outcome of an otherwise successful operation. Considerable efforts have been invested in the recent years to address paradigm shifts in our understanding of the complex microbiological phenomena that contribute to the pathophysiology of PJI, such as microbial adherence, biofilm formation, and resistance to antibiotics. This article is an introduction to some of the recent advancements in the prevention, diagnosis, and treatment of PJI. It describes how industry, academic researchers, and governme

Evolution and impact of drive-line infection in a large cohort of continuous-flow ventricular assist device recipients.

BACKGROUND: Drive-line infections (DLIs) frequently complicate ventricular assist device (VAD) support. We sought to describe the detailed effects of DLIs over time in patients with continuous-flow VADs, including the onset, risk factors, organisms involved, association with invasive infections, and outcomes. METHODS: We reviewed data for patients with HeartMate II VADs (HMII) who were implanted at the Cleveland Clinic from October 2004 to September 2011 and followed through December 2011. DLIs were defined according to published criteria. RESULTS: DLIs developed in 45 of 194 HMII VADs over

Analysis of extracted cardiac device leads for bacteria type: clinical impact.

The use of cardiac implantable electronic devices (CIED) increased over time, followed by rise of CIED-related complications, mainly infections and malfunctions. A clear diagnosis of CIED infection is of pivotal importance. When infection is confirmed, transvenous lead extraction (TLE) becomes mandatory, with associated risks and mortality. Local lesions at the device pocket often return negative swabs and tissue specimens, but conservative interventions are inconclusive, raising risks of systemic dissemination of infection and difficulties of subsequent TLE any more. When local bacteriologica

[Is routine implant removal after trauma surgery sensible?].

We present a literature review about implant removal after operative extremity and spine fracture treatment. The indication for implant removal procedures has become less frequent in recent years, but is still more common in Europe than for example in North America. The time required to perform a implant removal can easily exceed the planned amount. Implant removal can result in significant complications like soft tissue damage, fractures, infections, and other problems. Not only because of these problems, the decision on whether or not to remove the implant should be made with great care. The

Implant-associated posttraumatic osteomyelitis: collateral damage by local host defense?

Infections following osteosynthesis or total joint replacement, also known as ''implant-associated posttraumatic osteomyelitis'', represent a major complication in orthopedic and trauma surgery. While the formation of bacterial biofilms on the implanted osteosynthesis materials is generally accepted as cause of the persistent infection, the molecular mechanisms leading to the progressive and destructive local inflammatory process and eventually to bone degradation, the osteolysis, have not been delineated. Here we provide evidence supporting the hypothesis that it is not the infection per se t

Toxic shock syndrome in plastic surgery patients: case report and review of the literature.

Toxic shock syndrome (TSS) is a rapidly developing disease, which may be lethal if not recognized and treated early. TSS unrelated to menstruation comprises an increasing proportion of the cases reported to the Centers for Disease Control during recent years, and a review of the literature reveals that TSS has is reported with increasing frequency in plastic surgical patients as well. The majority of reports relates to aesthetic plastic surgical procedures such as rhinoplasty, augmentation mammaplasty liposuction, and chemical peeling, but cases of TSS following reconstructive breast surgery w

The use of free gingival grafts in the treatment of peri-implant soft tissue complications: clinical report.

Soft tissue complications including gingival hyperplasia, peri-implantitis, inadequate vestibular depth, and insufficient attached tissue may occur adjacent to endosseous implants. In most clinical cases vestibuloplasty and grafting techniques to establish a wider zone of keratinized tissue are not needed. A clinical case is presented involving peri-implant mucosal areas exhibiting persistent inflammation and progressive recession. A free gingival palatal graft was used to correct the soft tissue defects and provide optimal peri-implant health in order to increase the long-term prognosis of th

Does a 24-h closed-suction drain reduce infection and hematoma after inflatable penile prosthesis surgery? A comparative study.

BACKGROUND: Three-piece penile prosthesis implantation (PPI) is the gold standard treatment for refractory erectile dysfunction. Although infection rates have declined with improved surgical techniques and antibiotic prophylaxis, postoperative hematoma remains an early complication that may predispose to infection and device malfunction. The role of short-term closed-suction drainage in reducing these complications remains controversial. AIM: To evaluate whether placement of a 24-h closed-suction drain reduces postoperative hematoma, infection, and mechanical complications following primary t
