Identifying homeless population needs in the Emergency Department using Community-Based Participatory Research.

BACKGROUND: Annually 1.5 million Americans face housing insecurity, and compared to their domiciled counterparts are three times more likely to utilize the Emergency Department (ED). Community Based Participatory Research (CBPR) methods have been employed in underserved populations, but use in the ED has been limited. We employed CBPR in an urban American hospital with a primary goal of improved linkage to care, reduced ED recidivism, and improved homeless health care. METHODS: A needs analysis was performed using semi-structured individual interviews with participants experiencing homelessne

"It's Not Like Taking Chocolates": Factors Influencing the Feasibility and Sustainability of Universal Test and Treat in Correctional Health Systems in Zambia and South Africa.

BACKGROUND: Sub-Saharan African correctional facilities concentrate large numbers of people who are living with HIV or at risk for HIV infection. Universal test and treat (UTT) is widely recognized as a promising approach to improve the health of individuals and a population health strategy to reduce new HIV infections. In this study, we explored the feasibility and sustainability of implementing UTT in correctional facilities in Zambia and South Africa. METHODS: Nested within a UTT implementation research study, our qualitative evaluation of feasibility and sustainability used a case-compari

Resolving Challenges in HIV Cure-Related Research: Protocol for a Modified Delphi Consensus-Building Process.

BACKGROUND: HIV cure-related research is expanding rapidly, bringing both new opportunities and ethical challenges. Historically, clinical trials for novel HIV treatments have underrepresented populations most affected by HIV, such as Black gay men and transgender women. This disparity is compounded by medical mistrust and historical mistreatment of racially and ethnically diverse individuals in the United States. Addressing these issues is crucial as we plan HIV cure-related clinical trials. We aim to build consensus on how to increase representation of groups most affected by HIV in cure-rel

Video-Algorithmic Patient Monitoring in Mental Health Inpatient Settings: Qualitative Study of Patient or Consumer, Clinician, and Vendor Perspectives.

BACKGROUND: Video-algorithmic patient monitoring (VAPM) combines remote, noncontact sensors and algorithmic analysis and is increasingly trialed in acute psychiatric and other care settings. While promoted for improving safety and reducing risk, it raises ethical concerns regarding safety, privacy and surveillance. Little is known about how those encountering VAPM in mental health care contexts anticipate its use and potential impacts, including where it has not yet been implemented. OBJECTIVE: This study aimed to explore the views of patients or mental health consumers, specialized mental he

Research and Knowledge Translation Priorities for the Prehospital Care of Older Adults: A Modified Delphi Study.

OBJECTIVES: Older adults account for a substantial proportion of emergency medical services (EMS) encounters. Despite increasing recognition of older adults as a distinct population within EMS, a minimal amount of work has been conducted to describe knowledge and practice gaps or to establish priorities for future research or translation. The objective of this study was to identify research and knowledge translation priorities with the greatest perceived potential to improve prehospital care for older adults. METHODS: We conducted a modified Delphi study involving a multidisciplinary stakehol

Challenges and opportunities for the implementation of the Three Rs in Canadian vaccine quality control.

A case-study approach was used to identify opportunities and challenges to the implementation of the Three Rs in vaccine testing in Canada. Data was obtained through interviews with 16 Canadian stakeholders involved in the production, testing and evaluation of vaccines. Participants identified inconsistent regulatory testing requirements, the lack of biological functionality of some in vitro methods, the benchmarking of in vitro against in vivo assays, and high caution towards method changes as major challenges to implementation. Opportunities to implementation were identified as the desire fo

Speeding up respiratory diagnosis: clinical drivers, barriers, and system readiness for rapid molecular point-of-care testing in Saudi Arabia.

BACKGROUND: Acute respiratory tract infections place substantial strain on healthcare systems, particularly during seasonal respiratory surges and mass-gathering events. Delayed confirmation of respiratory pathogens contributes to diagnostic uncertainty, unnecessary antimicrobial use, delayed infection prevention decisions, and inefficient patient flow. Rapid molecular point-of-care testing (mPOCT) offers near-PCR-level diagnostic accuracy with turnaround times compatible with real-time clinical decision-making; however, implementation and integration into routine clinical pathways remain vari

Development of an Online Scenario-Based Tool to Enable Research Participation and Public Engagement in Cystic Fibrosis Newborn Screening: Mixed Methods Study.

BACKGROUND: Newborn screening aims to identify babies affected by rare but serious genetic conditions. As technology advances, there is the potential to expand the newborn screening program following evaluation of the likely benefits and drawbacks. To inform these decisions, it is important to consider the family experience of screening and the views of the public. Engaging in public dialogue can be difficult. The conditions, screening processes, and associated moral and ethical considerations are complex. OBJECTIVE: This study aims to develop a stand-alone online resource to enable a range o

Evolving determinants of implementing low-risk childbirth quality improvement in Japan: a longitudinal qualitative study.

BACKGROUND: International evidence suggests that reducing user costs increases maternity care utilization but not necessarily care quality, underscoring the need for transparent, comparable measurement. In Japan, evidence-based maternity care for low-risk childbirth has not been systematically evaluated, and limited claims-data capture hinders assessment, benchmarking, and sustained quality improvement. We aimed to explore how determinants (facilitators and barriers) of implementing a multifaceted quality improvement intervention for low-risk childbirth emerge and evolve over time. METHODS: T
