OBJECTIVE: Obstructive sleep apnea (OSA) is a common sleep disorder characterized by repeated upper airway collapse during sleep. The epiglottis, often overlooked in conventional assessments, has emerged as a site of significant obstruction in OSA. This review aims to highlight current knowledge of the epidemiology, pathophysiology, diagnosis, and treatment of epiglottic collapse in adults with OSA.
DATA SOURCES: PubMed, Embase, Web of Science, and Cochrane literature search.
METHODS: We conducted a detailed review of publications related to epiglottic collapse, specifically on study design,…

BACKGROUND: Although the impact of obstructive sleep apnea syndrome (OSA) on cardiovascular morbidity is well documented, ocular complications remain an uncertain topic. Eye diseases represent a problem of modern society, being one of the main causes of disability and decrease of quality of life.
OBJECTIVE OF THE STUDY: The objective of this review is to analyze the specialized literature regarding ocular manifestations in OSA patients and the ophthalmological conditions associated with this sleep disorder. Underlying pathophysiological mechanisms and the impact of OSA treatment on ocular out…

BACKGROUND: Sleep apnea is a common disorder characterized by recurrent episodes of upper airway obstruction or impaired respiratory drive, leading to disrupted sleep and significant cardiopulmonary consequences. While anatomical and neuromuscular factors are well-established contributors to obstructive sleep apnea (OSA), and central sleep apnea (CSA) is often linked to cardiac or neurological conditions, the impact of endocrine disorders, particularly hypothyroidism, is frequently overlooked.
CLINICAL PRESENTATION: We describe a case of severe sleep apnea that exhibited persistent sleep apne…

Obstructive sleep apnea syndrome (OSAS) is a common yet often underdiagnosed sleep-related breathing disorder characterized by recurrent upper airway obstruction during sleep. We present a case of a 24-year-old, non-obese male patient with severe OSAS who exhibited an atypical clinical picture dominated by long-standing fatigue, poor concentration, and digital clubbing. Diagnostic evaluation included polysomnography, which confirmed severe OSAS, and ancillary testing to exclude pulmonary, cardiac, and gastrointestinal causes of clubbing. Continuous positive airway pressure (CPAP) therapy led t…

All areas of medicine are focused on developing a personalized approach to diagnosis and treatment of specific conditions. This is based on the fundamental concept that all subjects with apparently the same disorder are different. There are multiple reasons for these differences. These include differences in the sequence of DNA, differences in the environment, differences in epigenetics, some of which may be driven by environmental differences and differences in the microbiome. These different factors will result in variations in multiple aspects of the phenotype. This includes different pathw

Obstructive sleep apnea (OSA) is a heterogeneous condition characterized by diverse endotypic traits and clinical phenotypes. A recent randomized controlled trial evaluating a glucagon-like peptide-1 receptor agonist showed promising results, potentially making it the first on-label drug treatment for OSA. Phase II/III clinical trials investigating combinations of noradrenergic and antimuscarinic agents, as well as carbonic anhydrase inhibitors, are ongoing. Future drug treatments for OSA, either as monotherapy or combined with other treatment modalities, will be personalized and based on trea…

INTRODUCTION: The efficacy of non-pharmacotherapeutic treatment of obstructive sleep apnea, a highly prevalent condition with serious cardiometabolic and neurocognitive health consequences, is well established. Supplementing traditional treatment strategies with medications can improve symptoms and reduce side effects. Efforts to identify medications that target the causes of sleep apnea have met with mixed success. However, this remains a worthwhile objective for researchers to pursue, given the potential benefit pharmacotherapy could bring to those patients who reject or struggle to adhere t…

Background Childhood obstructive sleep apnea (OSA) is a common disorder in children mostly due to adenotonsillar hypertrophy. Therefore, adenotonsillectomy is the mainstay of treatment. However, the outcome of adenotonsillectomy is limited in some patients who develop persistent OSA (POSA). We aim to evaluate the prevalence, risk factors, and treatments of POSA in the pediatric population in Qatar. Methodology This is a retrospective review of medical electronic records of patients aged 1-18 years, who underwent adenoidectomy and/or tonsillectomy at Sidra Medicine (Doha, Qatar) between June 20…

Medication-induced central sleep apnea (CSA) is one of the eight categories of causes of CSA but in the absence of awareness and careful history may be misclassified as primary CSA. While opioids are a well-known cause of respiratory depression and CSA, non-opioid medications including sodium oxybate, baclofen, valproic acid, gabapentin, and ticagrelor are less well-recognized. Opioids-induced respiratory depression and CSA are mediated primarily by µ-opioid receptors, which are abundant in the pontomedullary centers involved in breathing. The non-opioid medications, sodium oxybate, baclofen, …

About 40% of cases of cognitive impairment (CI) are associated with modifiable risk factors, such as inactivity, hypertension, diabetes and obesity. Recently, sleep disorders, including obstructive sleep apnea syndrome (OSA), have been considered among these factors. OSA is one of the most widespread conditions among patients with CI. The pathogenesis of cerebral lesions in OSA is complex. Timely diagnosis and complex therapy of patients with OSA can reduce the risk, reduce the severity of CI and slow their progression. Along with non-drug methods of treatment, the use of the drug Cortexin wit…

PURPOSE: The existence of a bidirectional relationship between poor sleep and pain intensity has been studied, and good sleep quality has been found to be a key factor underlying pain control. The purpose of this prospective cohort study was to observe if OSA treatment provides a reduction in temporo-mandibular disorder (TMD) pain and headache attributed to TMD in patients with obstructive sleep apnea (OSA) after 18 months of OSA treatment.
METHODS: A prospective cohort study was conducted on consecutive patients suffering from OSA. Patients underwent polysomnography and TMD examination accor…

Obstructive sleep apnea (OSA) has long been studied in patients with obesity and type 2 diabetes mellitus (T2DM), due to the fact that both disorders are commonly associated with an increased body mass index (BMI). However, a link between OSA and non-obese diabetic patients is still not very elaborated, nor heavily explored. In this review, we elucidate some proposed mechanisms for the link between OSA and diabetic patients both with and beyond obesity, shedding the light on the latter case. One such mechanism is oxidative stress, a phenomenon of reactive oxygen species (ROS) imbalance seen in…

Sleep apnea is a serious comorbid condition affecting patients with heart failure. Present in 50% to 75% of heart failure patients, it is often underrecognized, underdiagnosed, and undertreated. Patients with sleep apnea and heart failure are at increased risk of adverse cardiovascular events and sudden death. Treatment of sleep apnea has shown mixed results in reduction of adverse outcomes by sleep apnea type and intervention strategy. Evolving home-based technologies and device therapies present an exciting frontier for patients with sleep apnea and heart failure and an opportunity to improv…

OBJECTIVE: The physiological mechanisms involved in cardio-respiratory responses to sleep apnea events are not yet fully elucidated. A model-based approach is proposed to analyse the acute desaturation response to obstructive apneas.
METHODS: An integrated model of cardio-respiratory interactions was proposed and parameters were identified, using an evolutionary algorithm, on a database composed of 107 obstructive apneas acquired from 10 patients (HYPNOS clinical study). Unsupervised clustering was applied to the identified parameters in order to characterize the phenotype of each response to…

INTRODUCTION: Fibromyalgia is characterized by pain all over the body, whose diagnosis and treatment are not fully understood. Obstructive sleep apnea syndrome (OSAS) is a disease that causes apnea, hypopnea and oxygen desaturation due to collapse in the upper respiratory tract and is characterized by excessive daytime sleepiness, fatigue and lack of attention. Symptoms and signs of OSAS and fibromyalgia are similar. In our study, we aimed to compare the association of fibromyalgia in female OSAS patients in terms of polysomnography and laboratory parameters.
MATERIALS AND METHODS: We aimed t…

BACKGROUND: There is growing evidence supporting an association between obstructive sleep apnea syndrome (OSAS) and systemic vascular disorders. However, the data on choroidal microvasculature are limited. In recent years, choroidal thickness (CT) and choroidal vascularity index (CVI) have been of considerable interest as objective markers of choroidal vascularity. We hypothesized that the imbalance of vascular regulation in OSAS may adversely affect the CT and CVI and may help to assess the vascular risk in these patients.
PURPOSE: This study aimed to evaluate the choroidal morphology in pat…

CONTEXT: Obstructive sleep apnea (OSA)-related hypoxemia stimulates release of acute-phase proteins and reactive oxygen species that exacerbate insulin resistance and lipolysis and cause an augmented prothrombotic and proinflammatory state which can leads to premature death.
AIMS: This study aims to study the prevalence of OSA in diabetic patients.
SETTING AND DESIGN: It was a cross-sectional study, done over a period of 1 year in a tertiary care hospital.
MATERIALS AND METHODS: A total of 149 type 2 diabetic patients were enrolled after taking written consent. All patients were subjected t…
