This study reviewed a series of cemented total hip arthroplasty (THA) for dysplasia, with structural autograft fixed with poly-L-lactic acid screws. Grafted bone union was confirmed radiologically in every case, and there were no cases of early collapse or extravasation of grafted bone. Kaplan-Meier survivorship analysis of socket revision, radiologic loosening of the socket, and the appearance of a radiolucent line greater than 1 mm in the graft-socket interface as the end points indicated survival rates of 99%, 97.1%, and 63.5% at 10 years and 96.6%, 90.2%, and 56.1% at 15 years, respectivel

BACKGROUND: Implant anchorage in highly osteoporotic bone is challenging, since it often leads to osteosynthesis failure in geriatric patients with supracondylar femoral fractures. Cementation of screws is presumed to prevent such osteosynthesis failure. This study aimed to investigate the effect of a newly designed, cementable fenestrated condylar screw for plate fixation in a biomechanical setting.
METHODS: Eight pairs of osteoporotic cadaver femora with an average age of 77 years, ranging between 62 and 88 years, were randomly assigned to either an augmented or a non-augmented group. In bo

PURPOSE: The established technique for posterior C1 screw placement is via the lateral mass. Use of C1 monocortical pedicle screws is an emerging technique which utilizes the bone of the posterior arch while avoiding the paravertebral venous plexus and the C2 nerve root. This study compared the relative biomechanical fixation strengths of C1 pedicle screws with C1 lateral mass screws.
METHODS: Nine human C1 vertebrae were instrumented with one lateral mass screw and one pedicle screw. The specimens were subjected to sinusoidal, cyclic (0.5 Hz) fatigue loading. Peak compressive and tensile for

This study aimed to validate a patient-specific biomechanical simulation pipeline for predicting pedicle screw loosening risk in posterior spinal fusion. In particular, the research questioned whether this simulation, which integrates CT-derived bone properties, musculoskeletal force data, and finite element analysis, can outperform conventional CT attenuation measurements taken in both the vertebral body and along the planned screw trajectory. We conducted a retrospective database analysis, including patients who underwent lumbar spinal fusion with preoperative and postoperative CT scans. Scr

BACKGROUND: Transverse tarsal joint arthrodesis is commonly performed in the operative treatment of hindfoot arthritis and acquired flatfoot deformity. While fixation is typically achieved using screws, failure to obtain and maintain joint compression sometimes occurs, potentially leading to nonunion. External fixation is an alternate method of achieving arthrodesis site compression and has the advantage of allowing postoperative compression adjustment when necessary. However, its performance relative to standard screw fixation has not been quantified in this application. We hypothesized that

Locking plates have a rapidly growing process especially in the past decades and results are satisfactory especially in the osteoporotic bones compared to non-locking compression plates. There are many forms of failure in the fracture fixation of locking plates, and screw pull-out is one of the main failure reasons. In this study, we aim to investigate pull-out failure in locking plates using locking spongious screws. The pull-out force of an FDA approved locking plate system (LPS) and anonymous locking plate using the single lead head locking spongious screw (LPuLSS) was evaluated in vitro on

BACKGROUND: Reverse shoulder arthroplasty (RSA) is commonly used to treat patients with rotator cuff tear arthropathy. Loosening of the glenoid component remains one of the principal modes of failure and represents a significant complication that requires revision surgery. This study assessed the effects of various factors on glenoid baseplate micromotion for primary fixation of RSA.
MATERIALS AND METHODS: A half-fractional factorial design of experiment was used to assess 4 factors: central element type (central peg or screw), central cortical engagement according to length (13.5 or 23.5 mm)

High failure rates have been associated with nonlocking cancellous screws with a typical buttress thread in patients with osteoporotic bone. This study aimed to develop a novel thread design and compare its fixation stability with that of a typical buttress thread. Nonlocking cancellous screws with a novel thread design (proximal flank angle of 120 degrees, a flat crest feature, a tip-facing undercut feature) and nonlocking cancellous screws with a typical buttress thread were manufactured using stainless steel. Fixation stabilities were evaluated individually by the axial pullout and lateral

BACKGROUND: Osteoarticular allograft is now commonly used as a reconstructive biomaterial to replace bone defect following removal of aggressive bone tumors and for revision of failed arthroplasty. Good long-term clinical result of allograft replacement has been demonstrated. However, the rate of complications is high, including deep infection, allograft fracture, delayed union or non-union, and joint instability. Prevalence of allograft fracture ranges from 12-54 percent. Many studies recommend avoiding plate and screw fixation, or they advise the use of a modified screw fixation technique th

Comminuted proximal tibia fractures are an ongoing surgical challenge. This "proof of concept" study is the first step in designing a new percutaneous plate for this injury under toe-touch weight-bearing as prescribed after surgery. Finite element simulations generated design curves for overall stiffness, bone and implant stress, and interfragmentary motion using 3 fixations (no, 1, or 2 "kickstand" (KS) screws across the fracture gap) over a range of plate elastic moduli (EP = 5 to 200 GPa). Combining well-established optimization criteria to enhance callus formation (i.e. 0.2 mm ≤ axial inte

Graphene and its derivatives have been receiving increasing attention regarding their application in bone tissue engineering because of their excellent characteristics, such as a vast specific surface area and excellent mechanical properties. In this study, graphene-reinforced nanohydroxyapatite/polyamide66 (nHA/PA66) bone screws were prepared. The results of scanning electron microscopy observation and X-ray diffraction data showed that both graphene and nHA had good dispersion in the PA66 matrix. In addition, the tensile strength and elastic modulus of the composites were significantly impro…
