Introduction: Emergency department differentiation of pulmonary oedema from chronic obstructive airways disease causing acute breathlessness is inaccurate 25% of the time despite clinical acumen, clinician-reported chest x-ray and ECG. This research investigates whether a basic lung ultrasound protocol (LUS) could improve identification of pulmonary oedema in breathless elderly patients. Method: Researchers prospectively sampled patients over 60 years, describing any breathlessness on presentation to a suburban emergency department. LUS studies were acquired by experienced or novice sonologist

Several types of cardiovascular complications can occur during treatment with targeted therapies: heart failure, QTc lengthening, arterial and venous thrombosis. A clinical examination, ECG and cardiac ultrasound are essential before starting treatment with targeted therapy. Patients with no medical history, who are asymptomatic with a normal ECG and a left ventricular ejection fraction (LVEF) greater than 50% can begin molecular targeted therapy (MTT). Patients must be assessed by a cardiologist before the introduction of MTT if they have a history of ischemic or valvular heart disease, heart

The prevention of perioperative myocardial ischemia is one of the cornerstones of anesthetic techniques. From the perspective of anesthesiologists, the traditional relation between oxygen supply and demand is improved mainly by reducing demand. Cardiologists, however, look at the problem from the other side of the equation. For the cardiologist, myocardial infarction is an entirely thrombotic event caused by changes in procoagulants, regardless of increased demand for oxygen. It is conceivable that signs of perioperative myocardial ischemia, which are related to increased oxygen demand, are eq…

To examine the changes in birth cohort prevalence rates and severity of congenital heart disease, we studied children with congenital heart disease born to blacks, whites, and Mexican-Americans in Dallas County from 1971 through 1984. Diagnoses were made by pediatric cardiologists' clinical evaluations, echocardiography, catheterization, surgery, or autopsy. During this study period, 2,509 of 379,561 liveborn infants were diagnosed, a prevalence rate of 6.6/1000. The rates for whites was significantly higher than for blacks or Mexican-Americans--7.2/1,000, 5.6/1,000, and 5.9/1,000, respectivel…

Pregnancy-related cardiovascular disease with stroke remains a considerable source of higher maternal morbidity and mortality occurs in periods of pregnancy, delivery, and postpartum. It is essential to counsel the mother before pregnancy by an expert cardiologist and obstetric team to discuss any event related to preexistent cardiac or past preeclampsia for estimation of maternal and fetal risks. In pregnancy, the cardiac state includes hypertensive disorders, ischemic heart disease, valvular disease, and postpartum stroke. The incidence of stroke is increasing in pregnancy, particularly in p

The most common types of supraventricular tachycardia are caused by a reentry phenomenon producing accelerated heart rates. Symptoms may include palpitations (pulsation in the neck), chest pain, lightheadedness or dizziness, and dyspnea. It is unusual for supraventricular tachycardia to be caused by structurally abnormal hearts. Diagnosis is often delayed because of the misdiagnosis of anxiety or panic disorder. Patient history is important in uncovering the diagnosis, whereas the physical examination may or may not be helpful, and usually necessitates use of a Holter monitor or an event recor

BACKGROUND: Cardiac defects can be the presenting symptom in patients with mutations in the X-linked gene FLNA. Dysfunction of this gene is associated with cardiac abnormalities, especially in the left ventricular outflow tract, but can also cause a congenital malformation of the cerebral cortex. We noticed that some patients diagnosed at the neurogenetics clinic had first presented to a cardiologist, suggesting that earlier recognition may be possible if the diagnosis is suspected.
METHODS AND RESULTS: From the Erasmus MC cerebral malformations database 24 patients were identified with cereb

BACKGROUND AND AIMS: The fetal diagnosis of complex congenital heart disease (CHD) leads to a crisis for the affected couple. How much more so when a decision is made to terminate the pregnancy? In making that decision do the parents understand the anomaly, its consequences and possible outcome and does that information influence their decision process?
METHODS AND SUBJECTS: Questionnaires were developed. They were forwarded to parents seen in a private obstetric ultrasound practice, diagnosed with, or specifically referred, because of the diagnosis of a complex cardiac anomaly in the fetus.

BACKGROUND: Congenital heart disease (CHD) remains a leading cause of neonatal morbidity and mortality worldwide, with a significant proportion of cases remaining undetected before birth. While pediatric cardiologist-performed echocardiography represents the diagnostic gold standard, access to specialist services is limited across many neonatal care settings globally. Neonatologist-performed point-of-care echocardiography has emerged as a bedside diagnostic tool; however, the evidence supporting its diagnostic performance and clinical impact requires systematic evaluation.

A 78-year-old woman with a history of hypertension, hyperlipidemia, and asthma was referred to her cardiologist for chest pain and dyspnea on exertion. After performing stress echocardiography, it was determined that closure of the fistula with coils was indicated. This case highlights that coronary fistula could be an unusual entity of angina, clinically improving after the occlusion procedure.

Tetralogy of Fallot is the most common cyanotic congenital heart defect. Advances in surgical technique and postoperative care have improved survival which is now very good. Patients now face long-term morbidities such as reduced exercise tolerance and arrthymias. Cardiologists caring for these patients are confronted with decisions regarding best care practices. This article will review the evidence available on repair and postoperative follow-up for patients with Tetralogy of Fallot with pulmonary stenosis.

Interventions in patients with 'abnormal' anatomy are a challenge for every invasive cardiologist. Understanding of the underlying geometry by 3D reconstruction of a pre-acquired image is facilitating the orientation of the operator. The report describes how to perform catheter ablation of atrial fibrillation in a patient with complete situs inversus using the magnetic navigation system in conjunction to electroanatomical mapping.

Echocardiography is a well established, non-invasive diagnostic technique that has assumed a prominent place in the armamentarium of the pediatric cardiologist. The principles and techniques of echocardiography are explained. In addition, the measurements and interpretation are described, as well as the deductive approach that leads to a specific diagnosis. The purpose of this discussion is to provide the pediatrician with an understanding of the appropriate application, value, and limitation of the echocardiogram.

BACKGROUND: Tele-echocardiography has the potential to bring real-time diagnoses to neonatal facilities without in-house pediatric cardiologists. Many neonates in rural areas, smaller cities, and community hospitals do not have immediate access to pediatric sonographers or echocardiogram interpretation by pediatric cardiologists. This can result in suboptimal echocardiogram quality, delay in initiation of medical intervention, unnecessary patient transport, and increased medical expenditures. Telemedicine has been used with increased frequency to improve efficiency of pediatric cardiology care…

The cardiologist involved in the evaluation of the competitive athlete requires knowledge of the normal variations seen on clinical examination and laboratory studies. There is limited information in the literature, currently, that provides guidelines for decision making in the face of cardiac abnormalities. This paper outlines our experience with athletes and cardiac disease. Recommendations are given as guidelines for specific activity allowances.
