McArdle's disease in childhood: report of a new case.

McArdle's disease (glycogenosis type V) is an inherited glycogen storage disease characterized clinically by myalgia, cramps and sometimes myoglobinuria, triggered by exercise. The onset of exercise intolerance is usually in late childhood or adolescence and diagnosis is exceptionally established during infancy. We report the case of a 6-year-old girl who had been complaining of aching muscles for a long time, and who presented after a near-drowning incident, with extensive muscle necrosis, probably secondary to myophosphorylase deficiency-induced cramps. These unusual manifestations led to th…

A fatality involving zolpidem.

An elderly woman residing in an independent-living retirement community was found dead in the bathtub. She had a history of depression and was prescribed Ambien (generic name zolpidem) for the treatment of insomnia. Two empty prescription bottles of Ambien were found; both were for 10-mg tablets with a total quantity of 60. A postmortem examination was conducted, and blood, urine, and gastric contents were submitted for toxicology screening. The cause of death was drowning. The only remarkable toxicology finding was zolpidem. Quantitative analysis by gas chromatography-mass spectrometry determ…

The spectrum of neuropathologic findings in deaths associated with seizure disorders.

The pathologic and neuropathologic findings in 90 autopsied cases of death associated with a seizure disorder or complication thereof were reviewed. Most (69%) of the deceased individuals were between 21 and 40 years of age; two thirds were male. In 58% of patients, no cause of death other than seizure disorder was found. The ultimate cause of death in those patients was assumed to be a cardiac arrhythmia or respiratory arrest. Drowning accounted for 19% of deaths, and 17% of patients died of other contributory causes such as suicide, exposure, or atherosclerotic coronary vascular disease. Asp…

[Malignant Transformation of Cerebellopontine Angle Epidermoid Cyst:A Case Report].

Intracranial epidermoid cysts are benign cystic lesions that typically exhibit slow growth. Their malignant transformation into squamous cell carcinoma is rare. We report a 77-year-old woman who was admitted to our hospital because of a near-drowning incident due to a seizure sustained in her bathtub. Magnetic resonance imaging(MRI)revealed an extra-axial tumor occupying the right cerebellopontine angle. The lesion appeared hyperintense in diffusion-weighted images and exhibited contrast enhancement after gadolinium injection. Cerebrospinal fluid examination revealed noninfectious meningitis, …

The increasing threat of personal watercraft injuries.

A trend of increasing number and severity of injuries associated with use of personal watercraft (PWC) has been noted as the use and popularity of PWC also rises. The rate of injuries secondary to PWC use is greater than that from other water sports. Multiple etiologies of injury have been reported, including closed head trauma, spinal injuries, facial fractures, chest trauma, abdominal injuries, and drowning. These injuries may occur from collision with other objects, falls from the PWC, and encounters with the hydrostatic jet stream. The most significant contributing factors to injury seem t…

Sudden unexpected death in young adults including four cases of SADS: a 10-year review from the west of Ireland (1985-1994).

Sudden unexpected death in young adults of 18-35 years account for an important subset of deaths in our autopsy population. The case notes and autopsy reports in 44 subjects were analysed during the period 1985-94 at the Department of Histopathology, University College Hospital, Galway to establish the underlying cause of death. Subjects who were involved in road traffic accidents, cases of drowning, and patients with known congenital heart disease, chronic renal failure and malignancy were excluded. We found that a large proportion of sudden deaths in young adults were secondary to epilepsy a…

Prehospital extracorporeal cardiopulmonary resuscitation for cardiac arrest patients in rural areas: a case report of two patients.

BACKGROUND: The prognosis of out-of-hospital cardiac arrest remains poor, especially for cardiopulmonary arrest patients in rural areas with longer transport duration to hospitals. CASE PRESENTATION: In June 2016, we began providing prehospital extracorporeal life support using a mobile operating room for emergency surgery. We report two patients who survived after receiving prehospital extracorporeal cardiopulmonary resuscitation and were discharged. A patient with cardiopulmonary arrest from accidental hypothermia due to drowning survived with good neurological outcomes after on-site extrac…

Use of Acoustic Transmitter-Equipped Remote Sedation to Aid in Tracking and Capture of Entangled California Sea Lions (Zalophus californianus).

Free-ranging California sea lions ( Zalophus californianus ) with marine debris entanglements were darted with a combination of medetomidine, butorphanol, and midazolam by using acoustic transmitter-equipped darts. Of the 15 animals sedated, 13 (87%) reentered the water and were tracked by using a unidirectional hydrophone. Sea lions that entered the water continued to surface and breathe postsedation. There were three mortalities (20%) during the course of this study due to the following: suspected drowning caused by entrapment under a dock, overdose due to inaccurate weight estimation, and t…

The impact of climate change on child health.

Human activity has contributed to climate change. The relationship between climate and child health has not been well investigated. This review discusses the role of climate change on child health and suggests 3 ways in which this relationship may manifest. First, environmental changes associated with anthropogenic greenhouse gases can lead to respiratory diseases, sunburn, melanoma, and immunosuppression. Second, climate change may directly cause heat stroke, drowning, gastrointestinal diseases, and psychosocial maldevelopment. Third, ecologic alterations triggered by climate change can incre…

Alkaloid delta agonist BW373U86 increases hypoxic tolerance.

Activation of delta opioid receptors increases survival time during acute, lethal hypoxia in mice. delta Agonists therefore present a promising avenue for therapeutic application to reduce the morbidity and mortality associated with clinical hypoxia in settings such as drowning, head injury apnea, and complicated childbirths. However, most delta agonists now available are peptides, and may have limited clinical utility. In the present study, we evaluate the neuroprotective ability of an alkaloid delta agonist, BW373U86. Alkaloid compounds, due to increased stability and increased systemic dist…

Eyes on the water: How local mariners can accurately detect a submerged human proxy in shallow nearshore coastal water with modern recreational fish finders.

It has recently been demonstrated that modern recreational fish finders with high-frequency (800 kHz) Sidescan transducers can detect submerged human bodies. This ubiquitous technology is widely used on the water and has the potential to be utilized for the rapid detection of submerged decedents in any waterbody. This research investigates whether, with minimal training, the boating public can use modern recreational fish finders to accurately locate a submerged human proxy in a shallow nearshore coastal location. One human-shaped plastic proxy was submerged, resting on the water bottom, at a

The Connection of Life with Respiration: Edmund Goodwyn's unexplored treasure of cardiopulmonary physiology.

The advent of artificial ventilation was largely sparked by the popular belief that drowning and other causes of asphyxia could induce death-like states known as suspended animation. While the mystical nature of such states befuddled some physicians into the early 1900s, an English medical student by the name of Edmund Goodwyn (1756-1829) published a thesis in 1786, which demonstrated that suspended animation was simply the physical manifestation of extreme hypoxia. Goodwyn's work advanced one of the earliest arguments in favor of artificial ventilation for the treatment of asphyxia over alter…

The Preference for Strangulation in a Sexually Motivated Serial Killer.

The existing literature on the killing method of choice for sexually motivated serial killers suggests strangulation as the preferable means of homicide, when the victim is female. When homicide victims are male, however, existing research suggests that firearms and blades are preferable methods of causing death. A case is presented here of a sexually motivated male serial killer who exclusively targeted males and who chose strangulation as his means of killing. Analysis suggests that not only is the psychological constitution of the killer an important factor in understanding how they kill vi
