Two fatal cases of amiodarone-induced acute, confluent, necrotic hepatitis are described. The patients, aged 28 and 60, had received a high loading dose of amiodarone. After the first and second day respectively following the administration of amiodarone, the patients developed jaundice, hepatomegaly, high serum transaminases, a prolonged prothrombin time and low cholesterol concentration. Because safe, effective treatment for established viral hepatitis is not available, physicians need to be acquainted with recent advances in prophylaxis. Immune globulin (Gamastan, Gammar) is used for both preexposure and postexposure prophylaxis of hepatitis A, and side effects are rare. Two vaccines (Heptavax-B, Recombivax HB) are licensed for hepatitis B, one is a plasma-derived vaccine, the other a yeast-recombinant vaccine. Indirectly, these also control hepatitis D (delta agent), which needs hepatitis B virus to develop.

We report the cases of 2 female patients aged 69 and 61 yr, suffering from fulminant hepatitis induced by pirprofen, a new nonsteroidal antiinflammatory drug. The duration of pirprofen administration before the onset of hepatitis was long, 7 and 9 mo, respectively. Hepatitis was not preceded or accompanied by hypersensitivity manifestations. The liver lesion consisted of massive, predominantly centrilobular hepatic cell necrosis and microvesicular steatosis. One patient died of liver failure. We present a case of halothane-associated-hepatitis in a 34-year-old woman. She received halothane anesthesia twice within a 6-day interval, 2 weeks prior to the onset of symptoms. Liver histology was nonspecific, and unusual in showing not only lobular hepatitis but also granulomatous changes. Since the clinical picture and the laboratory values were more and the liver histology less in keeping with the diagnosis, we emphasize that the former two parameters should play a vital role in establishing or discarding the diagnosis of halothane-induced hepatitis.
