Peripheral neuropathy in a turkey vulture with lead toxicosis.

Clinical, electromyographic, and pathologic findings characteristic of lead toxicosis were detected in a turkey vulture (Cathartes aura). The bird had generalized lower motor neuron dysfunction that progressed over 5 days. Electromyography revealed diffuse denervation potentials and a presumed decrement in the sciatic-tibial nerve conduction velocity. Histologic examination of peripheral nerves obtained at necropsy revealed changes that could be compatible with lead-induced neuropathy. Lead toxicosis was confirmed by determination of blood lead concentrations. Lead toxicosis causing neurologic…

Andrew Jackson's exposure to mercury and lead: poisoned president?

Historians have suggested that US president Andrew Jackson (1767-1845) experienced lead and mercury poisoning following his therapeutic use of calomel (mercurous chloride) and sugar of lead (lead acetate). To evaluate these claims, we performed direct physical measurement of 2 samples of Jackson's hair (1 from 1815, 1 from 1839). Following pretreatment and acid digestion, mercury was measured using cold vapor generation techniques, while lead levels were measured by electrothermal atomic absorption spectrophotometry. Mercury levels of 6.0 and 5.6 ppm were obtained from the 1815 and 1839 hair s…

Undiagnosed central anticholinergic syndrome may lead to dangerous complications.

This report describes two cases of central anticholinergic syndrome, the first after general anaesthesia and the other during a prolonged stay in the intensive care unit. The symptoms in both patients resolved soon after physostigmine administration. There was a delay in the diagnosis of central anticholinergic syndrome, which resulted in acute lung injury and unanticipated intensive care unit admission. It is suggested that in cases of abnormal mental recovery after anaesthesia or sedation, the diagnosis of central anticholinergic syndrome should be considered.

Lead poisoning secondary to hyperthyroidism: report of two cases.

With long-term exposure to lead, lead accumulates in bone, where it is stored for years. These quiescent lead stores are mobilised when increased bone turnover occurs, and latent lead toxicity may then become symptomatic. Although Graves' disease is a common cause of increased bone turnover, to date hyperthyroidism has been implicated in lead poisoning only twice. We describe herein two cases of hyperthyroidism, one caused by toxic multinodular thyroid enlargement, the second by Graves' disease, leading to lead poisoning. Treatment of hyperthyroidism with radioactive iodine cured both hyperthy…

Optimization of shocking lead configuration for transvenous atrial defibrillation.

INTRODUCTION: High atrial defibrillation energy requirements (ADER) in patients with chronic atrial fibrillation (AF) may limit the acceptan … fined as the voltage needed to defibrillate the atria in 10% to 90% of 20 consecutive shocks. Three lead systems had ADER lower than the RA (anode) to CS (cathode) configuration, which required a mean of 143+/-58 volts. These three were: RA-SVC (anode) to CS (cathode) 103+/-29 V; I-CS (anode) to RA (cathode) 129+/-39 V; and P-CS (anode) to RA (cathode) …

Initial experience with a co-radial bipolar pacing lead.

A new type of endocardial bipolar pacing lead has been designed to overcome the potential drawbacks of the conventional coaxial bipolar pacing lead. We prospectively evaluated the new co-radial bipolar pacing leads (Intermedics Thin-Line), which are thinner (5 Fr vs 6-8 Fr) than standard coaxial bipolar leads. X-ray visibility and lead handling were subjectively assessed (excellent, good, adequate, or poor) at implant; lead impedance, sensitivity threshold, and pacing threshold were measured at implant, then at 1, 3, 6, 12, and 18 months. The results were as follows: 103 patients (51 M; age 63…

A murine model of genetic susceptibility to lead bioaccumulation.

Previous reports have shown that blood lead levels in humans are associated with a polymorphic form of delta-aminolevulinate dehydratase (ALAD), an enzyme of heme biosynthesis that binds and is inhibited by lead. We hypothesized that ALAD levels may influence the distribution and accumulation of lead in the blood and target organs. To assess this, we studied strains of mice that differ in the numbers of copies of the ALAD gene. Our findings showed that mice with a duplication of the ALAD gene (DBA) accumulated twice the amount of lead in their blood and had higher lead levels in kidney and liv…

Congenital conditions that lead to osteoarthritis in the dog.

Several congenital arthropathies exist in the canine that can lead to the development of degenerative joint disease. Nevertheless, early diagnosis and treatment generally will afford the patient a favorable prognosis for limb function. There is still a great need to develop controlled studies to evaluate the long-term efficacy of many of the surgical and medical treatment modalities that are currently available and will be made available in the future to treat the patient with a congenital arthropathy.

Vegetative function diagnosis for early detection of lead intoxication.

In the search for the early effects of neurotoxic lead poisoning, vegetative function diagnosis is a potential approach, focusing on the behaviour of the cardiac rhythm. Four groups of male subjects (109 copper workers exhibiting mean lead levels in blood of 31.2 micrograms/dl after long-term lead exposure; 27 control subjects having a similar job in the iron and steel industry without neurotoxic exposure; 35 reference subjects from Magdeburg University Hospital without neurotoxic exposure, and 5 subjects to whom benefits have been awarded for disability resulting from lead intoxication) were …

Malposition of transvenous pacing lead in the left ventricle.

Transvenous pacemaker lead malposition in the left ventricle occurs rarely and requires a high index of suspicion for proper diagnosis. The case of a woman with unintentional lead placement in the left ventricle is presented. She had two episodes of transient neurologic deficits, possible secondary to embolic events, and was started on oral anticoagulants. Chest x-ray and electrocardiogram (ECG) suggested pacemaker lead malposition and transesophageal echocardiography revealed sinus venosus atrial septal defect. The lead was shown to cross the atrial septum and the mitral valve to the left ven…

Prevalence of lead intoxication in urban, adult medical inpatients.

Medical inpatients in inner-city hospitals have both potentially lead-related disorders and potential lead exposure. To see whether there is substantial unexpected lead intoxication in this population, we did a cross-sectional study of 117 consecutively admitted patients to a general medical ward of an inner-city university hospital. The mean (SD) blood lead level was 6.7 (2.8) micrograms/dL, with a range of 0 to 37 micrograms/dL. Twenty-one patients (18%) had mildly elevated lead levels (10 to 19 micrograms/dL) and 2 patients (2%) had moderately elevated lead levels (20 to 44 micrograms/dL). …

Antiphospholipid antibodies lead to increased risk in cardiovascular surgery.

BACKGROUND: Antiphospholipid (APL) antibodies are a heterogenous group of antibodies that have been associated with an increase in bleeding complications and a marked increase in thrombotic events, both of which result in significant patient morbidity and mortality. PATIENTS AND METHODS: A retrospective analysis of patients identified to be positive for APL via a university thrombosis registry who had cardiovascular surgery between 1989 and 1994. RESULTS: Seventy-one patients positive for APL antibodies were identified. Of those patients, 19 had cardiovascular surgical procedures (11 women a

Neurobehavioural evaluation of Venezuelan workers exposed to inorganic lead.

OBJECTIVES: To assess neurobehavioural effects of low exposure to lead, 43 workers from a lead smelter and 45 workers from a glass factory were evaluated with the World Health Organisation neurobehavioural core test battery (NCTB) in a cross sectional study. METHODS: The NCTB comprises a questionnaire and seven tests that measure simple reaction time, short-term memory (digit span, Benton), mood (profile of mood states), eye-hand coordination (Santa Ana pegboard, pursuit aiming II), and perceptual speed (digit-symbol). RESULTS: Smelter workers were employed on average for four years, and had…

Time to pregnancy among women occupationally exposed to lead.

A retrospective time-to-pregnancy study was conducted among women biologically monitored for exposure to lead. The women were participants of a previous study on spontaneous abortion. They were classified into exposure categories on the basis of questionnaire information, and individual blood lead (B-Pb) measurements. The adjusted incidence density ratios (IDR) of clinically recognized pregnancies were .93 (95% confidence interval [CI] .56 to 1.57) for very low (B-Pb < .5 mumol/L), .84 (CI .48 to 1.45) for low (B-Pb .5 to .9 mumol/L), and .80 (CI 0.42 to 1.54) for higher (B-Pb > or = 1.0 mumol…

Detection of occupational lead nephropathy using early renal markers.

Automotive use of leaded gasoline continues to be an important source of occupational exposure to lead in India and other countries. The present study assessed the renal function and markers of early renal damage of 22 mechanics at three automobile garages. Urinary N-acetyl-3-D-glucosaminidase activity and beta-2-microglobulin levels were significantly increased in auto garage mechanics with blood leads of 30-69 micrograms/dL. A significant correlation was observed between blood lead levels and urinary N-acetyl-3-D-glucosaminidase activity but not with urine beta-2-microglobulin levels. A marg…

Lead arsenate poisoning in a herd of beef cattle.

Lead arsenate poisoning was diagnosed in 2 beef heifers and was suspected in 6 other cattle from the same herd that had died previously and were not examined. Clinical signs in affected cattle included staggering, dehydration, hemorrhage, acidemia, and shock. Diagnosis was by arsenic and lead analysis of urine samples and kidney and liver tissue digests. Both examined heifers died within 4 days of onset of clinical signs. These cattle had been moved from an area with poor grazing conditions to a pasture with abundant forage. This pasture had an open shed that contained an open sack of lead ars…

Intrauterine cocaine, lead, and nicotine exposure and fetal growth.

Intrauterine tobacco, lead, and cocaine exposure often co-occur and may affect fetal growth and development, yet studies of gestational cocaine effects have not adequately measured lead or tobacco. In this anonymous survey, blood lead and urine cotinine levels were determined and mothers were queried about tobacco use. Eighteen cocaine-exposed mother-infant dyads had higher lead and cotinine levels than 46 random nonexposed dyads, regardless of reported cigarette smoking. Crude growth decrements in cocaine-exposed newborns were attenuated after control for lead and cotinine. Future studies of …
