Myofibroblast programming blocks differentiation of TLS-organizing fibroblastic reticular cells in pancreatic cancer.

Pancreatic ductal adenocarcinoma (PDAC) is strongly resistant to immunotherapy. However, recent evidence shows that some PDAC tumors contain tertiary lymphoid structures (TLSs) associated with improved survival. Why TLS forms in some tumors but not others remains elusive. Using a lymphotoxin beta receptor (LTBR) agonist, we observe the induction of TLS-aggregates in some murine PDAC tumor models but not others. The phenotypes of cancer-associated fibroblasts (CAFs) in TLS-resistant models are myofibroblastic (myCAF), whereas TLS-permissive models are enriched with reticular-CAF (rCAF) subsets.

PLIN3 overexpression in lung adenocarcinoma promotes M2 macrophage-myofibroblast transition in the tumor microenvironment.

BACKGROUND: Macrophage–myofibroblast transition (MMT) promotes tumor progression. Studies have primarily emphasized macrophage-intrinsic regulation; however, whether lung adenocarcinoma (LUAD) cells actively drive MMT in tumor-associated macrophages (TAMs) remains unclear. METHODS: We integrated LUAD single-cell RNA sequencing (scRNA-seq) and bulk transcriptomic datasets, applying MMT-infiltration-stratified differential analysis to identify tumor cell-derived upstream regulators of MMT. Multiple machine learning algorithms were used to construct an MMT-related risk score. SHAP analysis priori

Suppression of the myofibroblastic cancer-associated fibroblast phenotype to enhance anti-PD-1 response.

BACKGROUND: Cancer-associated fibroblasts (CAFs) are key modulators of tumor growth, the tumor immune context, and treatment response. CAFs take on diverse phenotypes with distinct functions. Myofibroblastic CAFs (myCAFs) regulate extracellular matrix remodeling, while inflammatory CAFs (iCAFs) modulate immune infiltration. Further studies are required to validate biomarkers of CAF phenotypes and to identify means to therapeutically manipulate CAFs. METHODS: Single-cell RNA sequencing analysis was used to identify markers of CAF phenotypes in CRC. CAF markers were validated and compared to cl

Immunomodulatory topographies regulate myofibroblast differentiation and influence fibrous encapsulation of glaucoma drainage devices.

Elevated intraocular pressure (IOP) is the primary driver of glaucoma, and lowering IOP remains essential for preventing vision loss. Glaucoma drainage devices (GDDs) help reduce IOP but often fail due to fibrosis. This study identifies surface micro topographies capable of modulating the fibrotic response to GDDs both in vitro and in vivo. Using the TopoChip high-throughput platform (2176 topographies), we fabricated poly(styrene-block-isobutylene-block-styrene) SIBS based micro topographies and screened them for their effects on human Tenon fibroblast adhesion and transdifferentiation, as we

Spatially Distinct Macrophage Subsets Drive Myofibroblast Heterogeneity and Maladaptive Fibrosis in Lupus Nephritis.

OBJECTIVES: Lupus nephritis (LN) is a severe complication of systemic lupus erythematosus (SLE), leading to progressive renal fibrosis and functional decline. Understanding the interplay between immune cells and stromal cells is needed to develop effective therapeutic strategies. Here, we investigated the landscape of macrophage-fibroblast interactions in human LN and validated these findings in mouse models. METHODS: We characterized distinct fibroblast subsets and their interactions with renal macrophages using single-cell RNA sequencing (scRNAseq) of 156 human LN biopsies and 30 healthy co

ASCL2-mediated macrophage-myofibroblast transition generates immunosuppressive CAF_7 in NSCLC bone metastases.

BACKGROUND: NSCLC frequently metastasizes to bone, where the microenvironment becomes immunosuppressive, limiting immunotherapy efficacy. Tumor-associated macrophages (TAMs) and cancer-associated fibroblasts (CAFs) contribute to immune evasion, but CAF subset origins and roles in bone metastases remain unclear. We investigated whether TAMs undergo macrophage-to-myofibroblast transition (MMT) to generate an immunosuppressive CAF subpopulation in bone lesions, driven by ASCL2. METHODS: We integrated single-cell RNA sequencing of primary lung tumors and bone metastases with pseudotime and regulo

Abdominal wall inflammatory myofibroblastic tumor initially misdiagnosed as leiomyoma: a rare case report.

Inflammatory myofibroblastic tumor is a rare mesenchymal neoplasm that most commonly arises in the lung and intra-abdominal soft tissues. Involvement of the anterior abdominal wall is exceptionally uncommon and can lead to diagnostic difficulty. We report a case of a 34-year-old male who presented with a painless, enlarging supraumbilical mass. Imaging demonstrated a heterogeneously enhancing lesion with intra-abdominal extension and close proximity to bowel loops. Core needle biopsy suggested a benign spindle cell tumor consistent with leiomyoma. Due to diagnostic uncertainty, complete surgic

Uterine leiomyoma-like inflammatory myofibroblastic tumor: two case reports and literature review.

Inflammatory myofibroblastic tumor (IMT) of the uterus is a rare low-grade malignant mesenchymal tumor characterized by its potential for recurrence and metastasis. The compact fibrous subtype can be easily misdiagnosed as a leiomyoma when it lacks a prominent inflammatory cell infiltrate. To enhance understanding of this disease and improve diagnostic vigilance, we present two cases of IMT exhibiting morphological features similar to leiomyomas for analysis. The ages of the two patients were 26 and 34 years, respectively. In Case 1, microscopic examination revealed densely packed spindle-shap

ALK-positive inflammatory myofibroblastic tumor in the pelvis of a child: a case report and literature review.

Inflammatory myofibroblastic tumor (IMT) is a rare neoplasm that primarily affects children and young adults. While typically found in the lungs, liver, and gastrointestinal tract, pelvic involvement is recognized but occurs less frequently than intra-abdominal IMT, particularly in pediatric patients. Here we report a case of a 2-year-old boy who presented with a brief history of vomiting and decreased appetite. Imaging revealed a cystic-solid mass in the pelvis with progressive enhancement of the solid component, leading to suspicion of a vascular soft tissue neoplasm. Surgical exploration id

Knockdown of METTL3 ameliorated myofibroblast activities in oral submucous fibrosis through upregulating DKK3.

BACKGROUND: Oral submucous fibrosis (OSF) is a precancerous disorder characterized by abnormal buccal mucosal fibroblast (BMF)-to-myofibroblast transition. Dickkopf-related protein 3 (DKK3) is implicated in fibrosis, but its role and regulatory mechanisms in OSF remain unclear. METHODS: Clinical OSF and normal tissues were analyzed for DKK3 and m6A regulator expression. Arecoline-treated BMFs were used to model OSF in vitro. The mRNA and protein expressions were analyzed by RT-qPCR, Western blot, immunofluorescence staining, and immunohistochemistry. Cell viability, migration, and invasion we

Inflammatory-myofibroblastic tumour with acute calculous cholecystitis in a sessile gall bladder in a young woman.

A woman in her 30s presented with acute cholecystitis. Blood and radiological investigations supported the clinical diagnosis. Diagnostic laparoscopy showed a thickened inflamed gall bladder (GB). Laparotomy was performed that showed a thickened sessile inflamed GB. There was no involvement of adjacent organs, ascites or metastasis. Histopathology showed an inflammatory myofibroblastic tumour (IMT) in a sessile GB. The tumour is classified as intermediate grade of malignancy by the World Health Organization. There are eight reported cases of IMT, all of which were successfully treated with res

Inflammatory Myofibroblastic Tumor of the Central Airways: Case Report and Review of Diagnostic and Therapeutic Strategies.

Inflammatory myofibroblastic tumors (IMTs) are rare mesenchymal neoplasms with intermediate malignant potential. While most commonly arising in the lung, airway involvement is exceedingly uncommon. Endobronchial IMTs typically manifest with symptoms of airway obstruction, including dyspnea, wheezing, and stridor. Diagnosis involves radiological imaging, bronchoscopic evaluation, and histopathological analysis. Additionally, assessment of anaplastic lymphoma kinase (ALK) status is particularly important in young and pediatric patients, as ALK-targeted therapies constitute a viable treatment opt

Pulmonary Inflammatory Myofibroblastic Tumor Mimicking Septic Pulmonary Embolism Following a Dental Infection.

Inflammatory myofibroblastic tumors are rare mesenchymal neoplasms that may mimic infections. A 67-year-old man developed a persistent fever after dental extraction. Chest computed tomography revealed multiple bilateral peripheral pulmonary nodules with feeding vessel signs, suggesting a septic pulmonary embolism. Despite intravenous antibiotics, his fever persisted, and blood cultures remained negative. A video-assisted thoracoscopic lung biopsy revealed spindle cell proliferation with inflammatory infiltrates. The patient's condition deteriorated rapidly, and he developed multi-organ failure

Clinicopathological Features and Immunochemical Staining of Inflammatory Myofibroblastic Tumor: A Retrospective Study of 48 Cases.

Inflammatory myofibroblastic tumors (IMTs) are rare neoplasms found in diverse anatomical sites, including the lungs, intestines, and bladder. Surgical resection is the primary treatment, with chemotherapy offering survival rates of 21.2 and 42.5 months in unresectable cases. Anaplastic lymphoma kinase (ALK) tyrosine kinase inhibitors (TKIs) have emerged as standard treatments. However, IMT presents diagnostic challenges owing to its morphological similarities with other conditions. While positive ALK expression aids diagnosis in approximately 50% of the cases, negative ALK expression complica

USP8-Rearranged Mesenchymal Tumors With Myofibroblastic Phenotype: A Comprehensive Clinicopathologic, Genetic, and Epigenetic Characterization.

USP8 is one of the members of ubiquitin-specific proteases deconjugating ubiquitin from target proteins. Besides USP6, it can be involved in tumorigenesis of mesenchymal neoplasms by binding to an activating fusion partner. Until now, USP8 fusion genes have been reported in calcified chondroid mesenchymal neoplasms, an inflammatory myofibroblastic tumor, a cardiac neoplasm, and a retroperitoneal sarcoma. In this study, we describe the clinicopathologic and genetic/epigenetic features of 7 USP8-associated tumors. The cohort included 5 male patients aged between 2 and 11 years, and 2 female pati

Fibromodulin selectively accelerates myofibroblast apoptosis in cutaneous wounds by enhancing interleukin 1β signaling.

Activated myofibroblasts deposit extracellular matrix material to facilitate rapid wound closure that can heal scarlessly during fetal development. However, adult myofibroblasts exhibit a relatively long life and persistent function, resulting in scarring. Thus, understanding how fetal and adult tissue regeneration differs may serve to identify factors that promote more optimal wound healing in adults with little or less scarring. We previously found that matricellular proteoglycan fibromodulin is one such factor promoting more optimal repair, but the underlying molecular and cellular mechanis…

NNMT Orchestrates Metabolic-Epigenetic Reprogramming to Drive Macrophage-Myofibroblast Transition in Hypertrophic Scarring.

Hypertrophic scar (HS) is a cutaneous fibrotic disorder characterized by persistent myofibroblast activation and excessive extracellular matrix deposition. Elucidating the origin and characteristics of myofibroblasts remains a central focus in the field. This study identifies a novel subtype of scar myofibroblasts originating from macrophage-myofibroblast transition (MMT). MMT cells constitute a significant proportion of HS myofibroblasts and drive HS progression. Multi-omics analysis uncovered nicotinamide N-methyltransferase (NNMT) as a metabolic orchestrator of MMT. Liquid chromatograph mas…
